Each of these wells was found.to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped June 26~', 2009. The corrosion inhibitor/sealant was pumped June 30~' & July 16~', 2009. (CPA) representative, performed safety valve system (SVS) inspections on 9 wells located on Kuparuk 3Q pad. Mr. Crisp noted that the surface safety valve (SSV) on Well 3Q-14 had been defeated some time prior to the inspection - there was a fusable cap installed preventing the closure of the SSV. The fusable cap is designed to allow the SSV to close in case of a fire at the wellhead. A copy of the inspection report is attached. You are reminded that Conservation Order 348, Rule 5 modifies the requirements of 20 AAC 25.265 ("Automatic Shut-in Equipment") as follows: (a) Each well shall be equipped with a Commission approved fail-safe automatic surface safety valve system (SVS) capable of preventing uncontrolled flow by shutting off jlow at the wellhead and shutting down any artificial lift system where an over pressure of equipment may occur. (b) The safety valve system (SVS) shall not be deactivated except during repairs, while engaged in active well work, or if the pad is manned. If the SVS cannot be returned to service within 24 hours, the well must be shut in at the wellhead and at the manifold building. 1. Wells with a deactivated SVS shall be identified by a sign on the wellhead stating that the SVS has. been deactivated and the date it was deactivated. 2. A list of wells with the SVS deactivated, the dates and reasons for deactivating, and the estimated re-activation dates must be maintained current and available for Commission inspection on request. Notice' of Violation Kuparuk 3Q pad July 1, 2003 Page 2 of 2 CO 348 replaced the requirements of earlier Kuparuk field rules by providing for the elimination of subsurface safety valves (SSSV) in certain wells. The elimination of the SSSV increases the reliance on the surface safety equipment. The Commission is concerned with the recent increased numbers of defeated safety valve systems discovered during routine inspections. Failure to maintain an operable SVS is a violation of State regulations. Within 14 days of receipt of this letter, you must provide the Commission with an explanation of how this event happened and what will be done in the future to prevent its occurrence in CPA-operated fields. In accordance with Area Injection Order No. 2, excess non-hazardous fluids developed from well operations and those fluids necessary to control the fluid level in reserve pits will be pumped down the surface/production casing annulus. The annulus will be left with a non-freezing fluid during any extended shutdown in disposal operations.
