Submit a well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed. Item 1 a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. Item 4b: TPI (Top of Producing Interval). Item 8: The Kelly Bushing and Ground Level elevation in feet above mean sea level. Use same as reference for depth measurements given in other spaces on this form and in any attachments. Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). Item 20: Report true vertical thickness of permafrost in Box 20. Provide MD and TVD for the top and base of permafrost in Box 28. Item 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. Item 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, or Other (explain). Item 28: Provide a listing of intervals cored and the corresponding formations, and a brief description in this box. Submit detailed description and analytical laboratory information required by 20 AAC 25.071. Item 30: Provide a listing of intervals tested and the corresponding formation, and a brief summary in this box. Submit detailed test and analytical laboratory information required by 20 AAC 25.071. The well produces with a high GOR and has been shut-in since August 2008. Request: Plug and abandon wellbore in preparation for sidetrack to restore production. Well History: Well 02-22A is a July 2000 CTD sidetrack targeting ZiB. The high initial gas rate combined with declining oil production caused the GOR to climb above 30,000 scf/bbl within 6 months. In order to find the origin of the high gas production in the well, a RST/BORAXlfEMP log was run in December 2000. The log didn't show evidence of leaks at the liner lap and no apparent channeling. The continuous 830' perforation interval provided no gas shut-off options. In April 2003, two thin Z2 oil lenses (9216'-9228', 9304'-9312') were perforated to increase the oil production. The additional perfs failed to improve the oil production and the well continued producing as a cycle well with decreasing on time (15% for 2004). A Memory CNL log was run in October 2006, from 9100' - 9800' to update GOC. The new GOC pick was chosen at 8899' TVDSS (9498' MD). In July 2005, a SCMT log to find the cement top was run. The TOC was calculated at 8358' behind the 2 3/8" liner, approximately 400' above the Top of the Sag River (8778'). In the GLM #1, a latch broke while the gas lift valve was being set in April 1996. Different attempts to fish the valve in the past were unsuccessful and made it necessary to test the integrity of the well by setting a retrievable plug and through a CMIT TxIA. A MIT-T passed to 3,000 psi, a CMIT TxIA passed to 3,000 psi and MIT-OA passed to 2,000 psi in September 2009. Due to the restriction in the size of the liner and the high top of cement, this well is not a coiled sidetrack candidate and it will require a rotary sidetrack. Other Requests: Given rig floor height limitations, a MASP < 3000 psi, and the fact that the three ram drilling BOP configuration is preferred to a two ram configuration with a spool, BPXA will execute the provisions of 20 AAC 25.285 (f) (1) regarding the initial testing of the BOPE as follows when existing wellhead or testing equipment prevents the testing of the lowest set of pipe rams: That lowest set of rams will be tested at the earliest opportunity during path- forward de-completion work, i.e., after the completion has been pulled from the well. Safety concerns and spacing limitations with the wellhead height and tubing spool preclude us from installing a spacer spool in the three-ram drilling BOP stack. Scope of Work: The pre-rig work will include a P&A of the reservoir zone with cement and making a tubing cut in prepare well for rotary rig sidetrack operations. With a rig on the well, operations will include pulling the tubing from the pre-rig cut and completing the P&A work if required, a 9-5/8" BP will be set in position for milling the window at +/- 5850 MD. Preparations will be made for sidetracking. y... /~(~ ~ C)X>U bro~~ lvt,// /+~ ~Qm~/~' 7f'~ ~ ~' /`7r .~.~(~ ~G?r?`~~larl l.J~ T~ ~~.~/-r~ 02-22A Sundry Application 1 4/19/2010 Pre-Rig Operations: • C~ 1.
