Dear Mr. Maunder: Enc1ûsed please find a spreadsheet with a list of wells from the Kuparuk field (KRU). Some of these wells were found to have a void in the conductor by surface casing annulus. The volume of annular void subsequently was filled with a corrosion inhibiter engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. Supervisor DATE: May 18, 2001 FROM: John Crisp, SUBJECT: Petroleum Inspector Safety Valve Tests Kuparuk River Unit 3K: Pad May 18,2001- I traveled to Phillips Alaska's Kuparuk River Field to witness Safety Valve System tests. Ernie Barndt was Phillips Rep. in charge of SVS testing. Testing was conducted in a safe & efficient manner. I witnessed one failure, SSV on 3K-27 would not pass the DP test. The valve was serviced & passed the re-test The ^OGCC test report is attached for reference. I inspected 14 well houses on lQ Pad w/no problems witnessed. 'SUMMARY: I witnessed SVS testing in Kuparuk River Field. 14 wells, 28 components tested. 1 failure witnessed. 3.57% failure rate. 14 well houses inspected. Install Tankco diverter system. 3. Drill 12¼" hole to 9-5/8" surface casing point according to directional plan. 4. Run and cement 9-5/8" casing. 5. Install and test blow out preventer. Test casing to 2000 psig. 6. Drill out cement and 10' new hole. Perform leakoff test. 7. Drill 8½" hole to logging point according to directional plan. 8. Run open hole logs. 9. Continue drilling 8½" hole to provide 100' between plug back total depth and bottom of Kuparuk sands. 10. Run and cement 7" casing. Pressure test to 3500 psig. j 11. Downsqueeze A~ctic Pack and cement in 7" x 9-5/8" annulus. 12. Nipple down blow out preventer and install temporary ~m-s tree. 13. Secure well and release rig. 14. Run cased hole logs. 15.
