In 1986 the well had a rig workover to downsize tubing. During that operation the well was temporarily abandoned due to high fluid losses. In 1990 a rig workover installed a selective to isolate the A and C sands and the well was brought back online. 1B pad requires that a SSSV to be installed due to proximity to camp and the current tubing configuration only allows for a K-valve type to be installed. Since the K- valve requires back pressure applied from the surface to operate, some production is choked back. The T- PPPOT failed during pre-FEL diagnostics on 8/25/2013. Itqpassed a CMIT-TxIA on 9/6/2013. This rig workover will eliminate the K-valve and install a new SSSV that does not require back pressure to operate. The incremental production rate associated with this change is 81 BOPD. During rig preparatory work on 3/8/2014 a CMIT-TxIA to 2,500 psig failed. A MIT-IA to 2,500 psig failed on 3/9/2014. The leak rate is 0.25 gpm. This rig workover will pull the existing 4" x 3 '/2" tubing string above the selective, locate the leak in the IA, fix the leak with a stacked packer or MetalSkin casing patch,and install a 3 '/2"tubing string with a SSSV installed. Each of these wells were found to have a void in the conductor. These voids were filled with cement and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10 -404, Report of Sundry Operations. The cement was pumped in the fall of 2011. The corrosion inhibitor /sealant was pumped on various days in January and February 2012. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. The cement was. tagged at 7800' with 15,000 pounds. It was also tested to 1600 pounds of pressure fOr 25 minutes with no leakoff. The' top of the perfs was at 7945 There was 1450 pounds of pressure on the final squeeze of three bbis of cement. The 3M well head was removed and replaced with a 5M well head. B-6 Sec 9, TllN, R10E, UM Permit #81-75 Monday August _3, 1981, I traveled this date to Prudhoe Bay after receiving a Call fr~ Joe McInnis, Arco drilling foreman, requesting an inspector to witness the upcoming BOPE test. After arriving at the rig Joe informed me that while cementing the 10 3/4" surface pipe the cement had "flash set." After cleaning the cement out of the casing a cement bond log was run. I looked at the log and it showed good bond from T.D. at 2849' to 2640'. It was then decided to perforate the casing at 2410' and then set a cement retainer at 2350'. 1981~ After setting the retainer the D.P. was stung an attar was made to establish circulation. Circulation could not be established and 600 sks. of Artic set II was squeezed into the formation. The rig crew then ran 115' of 1" pipe between the conductor pipe and the 10 3/4" surface pipe and 180 sacks of Attic set II was pumped. Cement returns were received at the surface. W~nesday ~ugust 5, 1981: The BOPE test began at 7:00 p.m. and was ConclUded at 9:00 b"'.m. As the attached A.O.G.C.C. BOPE test report shows there were no failures.
