a well must be submitted to the AOGCC, no matter when the analyses are conducted per 20 AAC 25.071.
Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each
segregated pool is a completion.

Pursuant to 20 AAC 25.070, attach to this form: well schematic diagram, summary of daily well operations, directional or inclination survey, and
other tests as required including, but not limited to: core analysis, paleontological report, production or well test results.
Report measured depth and true vertical thickness of permafrost. Provide MD and TVD for the top and base of permafrost in Box 29.
Attached supplemental records should show the details of any multiple stage cementing and the location of the cementing tool.
If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the
producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced,
showing the data pertinent to such interval).

up Fox energy cement equipment. Run new submersible water well pump once electrician arrived. Fix wifi, wire new mix
tank to generator. Fill mix tank and test components. Rig pump lines to Leg 4 for RU on Di-5.
05/13/2023 - Saturday
PTW, JSA with crew. Continue mixing up mix tank. Fill 3x 400 bbl uprights with FIW water. Spot and line cuttings boxes with
10mill visquine. Run and prime fox energy injection pump skid and cement pump.Circulate water and test flow meters.
Build manifold on Di-05 from OOA, OA, IA, LS, SS to surface return diffuser tanks. Prep mixing pits with FSW310 surfactant
wash. Circulate 2x bottoms of in OA, IA, LS, SS. OOA would not inject or circulate. Pressure up 00A to 550 psi and watch for
bleed down. Dropped to 515 psi in 5 minutes. Spot 30 bbls of FSW310 on each OA and IA. Shut in well and prep for cement
job in the AM.

PTW, JSA with crew. Crews function check equipment. Break circulation through strings. Pressure test iron to 1000 psi. Set
pressure trips to 500 psi. Mix cement and circulate down LS taking returns up OA with 15.6 ppg cement slurry at 2.4
bbls/min. Shut down at 24 bbls away and divert returns from OTT to cuttings box. Confirm good cement to cuttings box @
31.5 bbls pumped. 6.5 bbls over calculated displacement. Close OA and open IA. Mix and pump 28 bbls of 15.6 ppg slurry
at 2-4 bbls/min. Cement seen at 25 bbls away. Shut down and swap from OTT to cuttings box. Circulate 1 additional bbl.
Close IA and open SS. Stay lined up to cuttings box. Circulate 2 bbls of 15.6 ppg slurry and confirm cement returns. Shut
down cement pump. Close SS. Open OOA to return tank and come online down LS to perform 5 minute hesitation squeeze
with 2 bbls of slurry. Hold and watch pressure, 540 psi for 5 minutes. Close in OOA and LS. All wellhead valves closed.
Open manifold bypass valves and clean up iron to cuttings boxes. Clean up cement skid and perform multiple sheen checks
with sheen kit. No sheen observed. Rig down wellhead iron. Start rigging up injection line to Di-17 LS and difuser tank.
05/15/2023 - Monday
PTW, JSA. Rig up on Di-17 injection well to Tubing LS and IA. Pressure up IA to 2000 psi and Injected 222 bbls of produced
fluid from Di-05 surface cement job down LS. Start Rigging up on Di-16 for intermediate cement job.

Attachments:
I traveled to the Dillon platform to witness the E-line conveyed TOC (top of cement) tag on well 05. Previously another inspector
had witnessed the MIT on the well but couldn’t witness the tag. They used a 1.75-inch bailer assembly and tagged the cement
plug at 5,918 ft MD inside the 9-5/8". Plug was founded on an existing packer after a variance was granted by AOGCC. No
sample observed in the bailer once at surface.

This letter confirms your conversation with Commission Engineer Tom Maunder last week regarding the
multiple Applications for Sundry Approval (Form 403) you submitted in mid October regarding changing
the monitoring frequency on the 16 wells on Dillon Platform and 23 wells on Baker Platform. Production
and injection operations were halted on these platforms in 4th Q 2002 and 2nd Q 2003 respectively.
Attached, please fmd the Sundry Applications, which are being returned without approval..¡ because the
relief you request needs to be sought through a different procedure.
The intent of submitting sundry notices for each well was to establish a monthly pressure and general
monitoring frequency for all wells on Dillon and Baker. There is a requirement in place for some of the
water injection wells that pressures and rates are to be monitored daily and reported monthly. Since the
platforms are unmanned and not operating, it is not possible to obtain daily information. When
operations were curtailed, a monthly monitoring frequency was established in the Plans of Development
(POD) annually filed with the Division of Oil and Gas and Unocal desires to assure that this monitoring
scheme is accepted by the Commission.
As noted in the conversation, changing the monitoring frequency where presently in place and
establishing a monthly monitoring frequency in general is not an action that is best accomplished with
multiple sundry notices. A letter application requesting the desired monitoring schedule should be made
under the applicable Area Injection Orders (AlO 7 for Baker and AIO 8 for Dillon) and Conservation
Orâers (CO 44 and CO 54) for the platforms/unit. The Commission looks forward to receiving your letter
applications. If prior monthly monitoring information has not been submitted to the Commission, that
information should be ubmitted forthwith.

Summary of Operations for August 1975:
August 19-21:
Skidded rig, removed Kobe pump and standing valve, and
displaced the hole with salt water. Cut the short string
tubing, nippled down the tree, nippled up BOP's and
pressure tested. Pulled the dual 3-1/2" tubing strings,
Kobe bottom hole assembly and 2-7/8" tubing tailpipe.
August 22:
Cleaned out casing to plug back depth of 10,825'. Set
retrievable bridge plug, installed new tubing head,
pressure tested to 1500 psi and retrieved bridge plug.
August 23:
Ran Gamma Ray correlation log and perforated the follow-
ing intervals at 4SPF:

10,502-10,507'
10,617-10,622'
10,720-10,725'
10,767-10,772'
August 24,27:
Cleaned out hole to plug back depth. Ran BJ Type 73KAl00
electric submersible pump on 3-1/2" Buttress tubing.
Landed tubing and nippled down BOP's. Nippled up tree
and tested to 3000 psi. Displaced salt water and re-
leased the rig.
24-hour test results: 919 BOPD, 1800 BWPD, 175 MCFD, 190 GOR.

cement.
July 24-26:
Perforated from 10,355-10,370' and 10,380-10,395' with
2 JSPF. Set packer ~t 10,280' and acidized new perforations
with 500 gallons of 15% HC1 acid. Tested,new perforations
on two 3-hour tests. Both tests were 100% water, no gas
or oil.

Propose to pull tubing and existing downhole equipment. Clean out
the liner to plug back depth. Cementsqueeze non-productive, high
water cut regions. Will use 200-250 sacks of Class G cement.
Note:
Verbal approval for this operation was obtained July 18, 1974
from a telephone conversation with Hoyle Hamilton of Alaska
Division of Oil and Gas.
