Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Opera.tions. The cement was pumped June 16~` & 26~', 2009 a.nd the corrosion inhibitor/sealant was pumped June 23`d & 30~', 2009. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Commissioner IAppr°va' N°' by order of the commission Date Form 10-403 Rev 12-1-85 Submit in triplicate Attachment I Kuparuk Well 3C-16 Fracturing Procedure May 9, 1988 Summary' Well 3C-16 will be refractured in the A Sand with gelled diesel and propped with 16- 20 mesh Carbo-Lite proppant. A diesel/10% Musol soak is recommended prior to the treatment for asphaltene control. The treatment consists of a pre-pad for ISIP calculations, pad, and 16-20 mesh Carbo-Lite slurry stages pumped "on-the-fly" at 20 BPM. Proppant concentrations are ramped up from 2 to 10 PPG. Silica Flour is added only to the pre-pad and pad stages, for leak-off control. ..Pre Job $1ickline/CTU' 1. SI C Sand. Pull CVs at 6254', 6323', and 6437'. Run DVs. 2. Perform two A Sand only well tests. Set lift gas rate for GLR of 2000 scf/stb. 3. MIRU CTU. Cleanout well to PBTD at 6794' MD-RKB. Add breaker to the gel for a 12 hour break. To minimize formation damage, take returns throughout the job. 4. Perform two A Sand only well tests. Set lift gas rate for GLR of 2000 scf/stb. 5. Pull GLVs at 2279', 3798', 4793', 5735', and 6153'. Run DVs. 6. Pressure test the tubing. Pre Job Checklist: 1. Inspect all tanks for cleanliness. 2. Check diesel for contamination prior to gelling, i.e. "black diesel", water, etc. 3. Check proppant for fines due to excessive handling. Request sieve analysis of proppant and obtain proppant sample. 4. Record proppant weight before and after the fracture treatment. 5. Procedure: RU fracturing equipment. Install "Tree Saver". Pressure test all lines. 2. Pressure up annulus to prevent buckling the tubing. Set pressure to account for thermal effects, achieving a maximum of 3500 psi during the treatment. 3. Pump 100 bbl Pre-pad stage. Shut down for ISIP calculations. * Check tank straps against metered volume. Correct flush volume accordingly. 4. Set pumps to trip off at 7250 psi wellhead pressure. Rate should be reduced if pressure exceeds 7000 psi. 5. Pump fracture treatment per attached schedule: , , . * All slurry stages should be mixed "on-the-fly". * Use silica flour in the PRE-PAD and PAD ONLY at 40 Ibs/Mgal. If coring is conducted, a brief description of each core and a one cubic inch size chip from each foot of recovered core is required. Samples of drill cuttings and a mud log are not required. A continuous directional survey is required as per 20 AAC 25.050(b)(5). If available, a tape containing the digitized log information shall be submitted on all logs for copying except experimental logs, velocity surveys and dipmeter surveys. M~ny rivers in Alaska and their drainage systems have been classified as important for the spawning or migration of anadromous fish. Operations in these areas are subject to AS 16.05.870 and the regulations promulgated thereunder (Title 5, Alaska Administrative Code). Selected intervals will be logged. A 20", 2000 psi annular diverter will be installed on the 16" conductor while drilling the surface hole. The 13-5/8", 5000 psi, RSRRA BOP stack will be installed on the 9-5/8" surface easing. The stack and related equipment will be tested to 3000 psig upon installation and weekly thereafter. Expected maximum surface pressure is 2816 psi. Pressure calculations are based on virgin reservoir pressure. Surface pressure is calculated with an unexpanded gas bubble at surface, including temperature effects. Casing will be tested to 3000 psi prior to releasing the drilling rig. Non-freezing fluids will be left in uncemented annuli through the permafrost interval. %-1'6 will be 360' away from 3C-9 at the surface. There are no closer wellbores anywhere downhole. Reserve pit fluids will be disposed of by pumping them down the 7" x 9-5/8" annulus of this well.
