Access:ThepropertyisaccessedfromHolsteinAvenue(offPointMackenzieRoad).Vehiclescannot
directlyaccessthepropertyandyouhavetotraversethetrailonfootforjustoveramile.Theformer
trailtothepadisheavilyvegetated.Nearthebeginningofthetrailitismuckyandnotraisedabovethe
surroundingwetlands.Therampontothepadremainsingoodshape.Aroadtothepadwouldneedto
beconstructed,requiringclearingandplacementofgravel.Withclearinganduseofmudmatsa
trackedvehiclemaybeabletotravers.Thepaditselfhasalargeflatareathatcouldbeusedasastaging
areaorahelicopterpad.
Drainage:Thepadissurroundedbywhatappearstobejurisdictionalwetlands.Itappearsthatwater
mightrunoffthepadonthenortheastcornerofthepad.Therewasnoevidenceofrunofffromthe
padtothewetlandsinotherlocationsonthepad.Withinthepadtherearetwolowareas,thepossible
closedreservepitinthesouthwestofthepadandthelargersumpedareaonthenorthsideofthepad.
VisualObservations:Thepadisraisedabovethesurroundingwetlands.Thesouthsideofthepadwas
lightlyvegetatedwithplantscommonindisturbedareasͲgrasses,dandelions,clover,fireweed,
plantago.Inthesouthwestcornerofthepadmaybeaclosedreservepit.Itwasseveralfeetlower
thanthepad.Alderandequisetumarethedominantvegetationintheclosedpossibleclosedreserve
pit.Therewasnostandingwaterinthepossibleclosedreservepitatthetimeofthesitevisit.
Atestpitdirectlyoffthepadfrompossibleclosedreservepityieldedsheen.Thesheenhadsoftedges
andbrokeupbutseemedtocoalescebacktogetherbutitwasnotobviouslypetroleumorbiogenic
sheen.Vegetationinthisareawassimilartothevegetationoffthepadwithsedges,heath,spruce.
Smallbirchweresparselypresentaroundtheedgesofthesumpedarea.
Spanningmostofthenorthsideofthepadwasalargesumpedareathatmayhavebeenusedasa
reservepitaswell.Therewasnotstandingwaterthroughoutmostofthenorthsideatthetimeofthe
sitevisit,howevertestpitsforshovelsheentestingfilledimmediatelywithwater.Thisareahadalight
layeroffinesedimentandorganicmatunderlainbysandygravel.Nosheenorodorwerepresentin
severaltestpits.
Onthenortheastsideofthepadtherewasawaterwellpresentwithawatermeterattached.
Atthenortheastcornerofthepad,nearthesumpedarea,therewasvisualevidenceofrunofffromthe
pad.Therewasnowaterflowingfromthepadatthetimeofthesitevisitbutvegetationlaydownin
thatareaandthepresenceofstandingwaterindicatedtheremayberunofffromthepadinthatarea.
Nosurfacereleasesorleakingcontainerswereobservedonthepad.Somemetaldebriswasobserved
onthepad.
MiddleLakeUnit1AͲ2
PIDreadings:Aphotoionizationdetector(PID)wasnotusedtoscreenthewell.Thewelliscompletely
encasedincorrugatedmetalpiping.
Recommendations:ConsidersamplingthewaterwellͲpetroleumhydrocarbons,volatileorganic
compounds,metals,polynucleararomatichydrocarbons.Surfacewatersampling(sameanalyses)
suggestedlocationsidentifiedinsitefeaturesfigure.Anadditionalsampleisrecommendedinthe
possibleclosedreservepitifwaterispresent.CoordinationwithADFGandpermittingmaybe
necessary.Repairoftheroadtothepadmayrequirewetlandspermits.

SUBJECT: Middle Lake Unit 1 Orphan Well Site Survey
Well Names: Middle Lake Unit 1A (PTD 205-149)
Operator: None (Orphan Well). Previously Pacific Energy Resources LTD.
Objective: Locate and map orphan well and associated debris, reserve pits and other features
relevant to future well plugging and site clearance operations.
Attachments:
• Photos, including Aerial drone images of the site and features are located in the
AOGCC wellfile.
• DNR survey map with GPS coordinates of wells and other features.
Observations:
I traveled by car to the access trail at the end of Holstein Rd to locate and assess the Middle Lake Unit 1
Orphan Well in preparation for eventual plug and abandonment. I was met by a survey crew from
Alaska DNR (Joe Donohue and Randy Guidu) and the three of us walked ~1.25 miles west through the
Susitna Flats State Game Refuge to reach the wellsite. The date of the site visit was selected such that
the area would be free of snow cover, but before new vegetation concealed the debris and wells.
The access trail was very wet and is not suitable for any kind of vehicle, other than snow machines in
winter. The access ramp at the east end of the drillsite is in good condition and the drillsite itself is
raised above the surrounding wetlands. It appears suitable as a helicopter landing zone and heavy
equipment staging area.
The well itself was enclosed in a large , +/-10’ diameter corrugated culvert and was not accessible.
There was also a perceived water well located on the drillsite. The site was cleared of most debris,
except for some large steel cables.
Photos were taken, including aerial drone photos, and a map created showing GPS location of the
various features at the drilsite. These photos and map are located in the AOGCC wellfile.
Photo looking east shows Middle Lake Unit 1A in the center of the drillsite, with the ~1.25 mile access
trail through the wetland to the end of W. Holstein Ave.
