Each of these wells was found to ha.ve a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped June 16~' & 26~', 2009 and the corrosion inhibitor/sealant was pumped June 23`d & 30~', 2009. The atta.ched spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Please ca11 Perty Klein or MJ Loveland at 907-659-7043, if you have any questions. Sincerely, ~~f~ MJ Loveland ConocoPhillips Well Integrity Projects Supervisor • • ConocoPhillips Alaska Inc. Dear Mr. Kewin: Enclosed is the approved application for permit to drill the above referenced well. If coring is conducted, a brief description of each core and a one cubic inch size chip from each foot of recovered core is required. Samples of drill cuttings and a mud log are not required. A continuous directional survey is required as per 20 AAC 25.050(b)(5). If available, a tape containing the digitized log information shall be submitted on all logs for copying except experimental logs, velocity surveys and dipmeter surveys. Many rivers in Alaska and their drainage systems have been classified as important for the spawning or migration of anadromous fish. Operations in these areas are subject to AS 16.05.870 and the regulations promulgated thereunder (Title 5, Alaska Administrative Code). Prior to commencing operations you may be contacted by the Habitat Coordinator's Office, Department of Fish and Game. Selected intervals will be logged. A 20", 2000 psi annular diverter will be installed on the'ld" conductor while drilling the surface hole. The 13-5/8", 5000 psi, RSRRA BOP stack will be installed on the 9-5/8" surface casing. The stack and related equipment will be tested to 3000 psig upon installation and weekly thereafter. Expected maximum surface pressure is 3005 psi. Pressure calculations are based on virgin reservoir pressure. Surface pressure is calculated with an unexpanded gas bubble at surface; including temperature effects. Casing will be tested to 3000 psi prior to releasing the drilling rig. Non-freezing fluids will be left in uncemented annuli through the permafrost interval. 3C-8 will be 120' away from 3C-6 at surface. There are no closer wellbores anywhere downhole. Reserve pit fluids will be disposed of by pumping them down the 7" x 9-5/8" annulus of this well. The 7" x 9-5/8" annulus will be left with a non-freezing fluid during any interruptions in the disposal process. (continued on attached sheet) 23.
