Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped July 27, 2008. The corrosion inhibitor/sealant was pumped July 3 and July 30, 2008. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Supervisor State of Alaska Alaska Oil and Gas Conservation Commission DATE: January 26, 2001 FROM: Chuck Scheve; SUBJECT: Petroleum Inspector Safety Valve Tests Kuparuk River Unit 30 Pad Friday, Jan.,uary 26, 2001' I traveled to the Kuparuk River Field and witnessed the 90-day retest of the safety valve systems on 30 Pad. As the attached AOGCC Safety Valve System Test Report indicates I witnessed the testing of 12 wells and 24 components with one failure observed. The Low pressure pilot on WAG injector 30-05 tripped below the required set point. This pilot was retested and passed at the the time of this test. Eamie Barndt conducted the testing today he demonstrated good test procedure and was a pleasure to work with All the wellhouses entered at this location were clean and all related equipment appeared to be in good condition. Summary:. I witnessed the 90-day SVS testing at 30 Pad in the Kuparuk River Field. After 200' of departure, there are no well bores within 200' of this proposed well. In accordance with Area Injection Order No. 2, excess non-hazardous fluids developed from well operations and those fluids necessary to control the fluid level in reserve pits will' be pumped down the surface/production casing annulus. The annulus will be left with a non-freezing fluid during any extended shutdown in disposal operations. The annulus will be sealed with 175 sx of cement followed by arctic pack upon completion of the fluid disposal.
