General' This form is designed for submitting a complete and correct well completion report and log on
all types of lands and leases in Alaska.
Item 1' Classificati°n of Service Wells: Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the

Commenced P&A operations 0900 hrs, 1-8-85. RU Halliburton. Mixed and
pumped 250 sx Class G cement (estimated TOC = 2700'). RD Halliburton.
Perf'd 1975' w/4 spf, 0° phasing. Established circulation down the 2-7/8"
tubing and up the 1-1/4" glycol line. Mixed and pumped Permafrost C until
cement returns out the 1-1/4" line. Switched lines to 2-7/8" x 7" annulus
and pumped cement until returns are seen. Cut off tree and wellhead.
Cemented 7" x 10-3/4" annulus w/40 sx Permafrost C cement. Weld on
abandonment marker. P&A operations completed 1500 hrs., 1-10-85.

Monday, January 7, 1985: I left Anchorage at 7:30 am by Alaska
Air-lineS---to '~rudhoe Bay, arriving at 9:10 am and at ARCO KRU CPF
1-23 Disposal Well at 10:15 am.
The purpose of abandonment of this well was due to questionable
tubing integrity and availability of a new CPF-1A Disposal Well.
Bottom plug was performed as follows, 30 bbls H90 ahead, followed
with 250 sacks 15.8 Class "G" cement + 2% CACL followed with 25
bbls H20 and 13 bbls diesel Estimated top of cement 2700 feet.
Cement was put away at 200 ~sig and was finished at 600 psig.
I was not present on the rest of the job due to problems they had
on another well and other work I was lined up with.
In summary, I witnessed the successful cementing of the bottom plug
on CPF-1-23 Disposal Well for ARCO. Ken Bonzo was the ARCO rep-
resentative.

~9 ARCO and the Well program to abandon CP~-1-23 was per-
formed according to the~Sundry Notice Form 10-403 dated and ap-
proved January 4, 1985. AOGCC Field Report and pictures taken of
marker attached.
Monda , Januar. y_ 14, 1985: Bobby Foster inspected the cellar and
p-~~-~ih~ c~sing with all the required information bead
welded on it, and picked up the pictures to verify same.

ARCO Alaska, Inc., proposes to plug and abandon this well due to q~stionable tubing integrity and the
availability of CPF-1A disposal well. The CPF-1 disposal well has open perforations from 3200' - 3230' in th,
Lower Ugnu formation. A 250 sx cement plug will be bullheaded into the formation. A hole will then be
punched into the tubing above the permanent packer at 2017'. Cement willie circulated down the tubing and up
the annulus all the way to surface. The wellhead and tree will then be [u~ off just below pad level. The
10-3/4" x 7" will then have a 50 sx surface plug set in it. A metal plate will be welded onto the casing
stubs with all the pertinent information as requested by State regulations welded on it. The cellar will be
filled and leveled.

Mr. James Week's August 24, 1984 letter, is approvable. This approval
conditioned upon'

l) receipt of a signed copy of the plan on EPA Form 7520-14 by
November 25, 1984; and
2) receipt of a plugging and abandonment report within 60 days after
plugging the well.
The plugging and abandonment report shall be certified as accurate by the
person who performed the plugging operation and shall consist of either'
l) a statement that the well was plugged in accordance with the
previously submitted plan; or
2) where actual plugging differed from the plan previously
submitted, an updated version of the plan on EPA Form 7520-14,
specifying the different procedures used.

As you know, ARCO will be plugging and abandoning well number~i...C...p..F_-_].,,..,,'~t
its Kuparuk River Unit. EPA's UIC Regulations require operators of these
types of wells to submit plugging plans for EPA approval prior to
abandonment. Enclosed is correspondence from ARCO dated August 24, 1984
which describes their plugging plan.
We have reviewed the plan and discussed it with ARCO's representative,
Mike Laue. We believe the plan is satisfactory; however, we would
welcome any comments you may have for incorporation in our approval. By
copy of this letter, I am also inviting any comments which ADEC may
have. I would appreciate receiving your comments by October 15.

RE: Kuparuk Central Production Facility (CPF) CPF-I Waste Injection Well
On the effective date of the Underground Injection Control Regulations for
the 5tare of Alaska (3une 25, 198#), ARCO Alaska, Inc. was operating the
Kuparuk CPF-I injection well as a Class IV well. The well is completed into
a zone of less than 10,000 mg/l TD5 and used for the disposal of hazardous
waste.
For your information the Class IV requirements for inventory'may be found in
Enclosures I & 2 and for plugging and abandonment in Enclosure 3. The
CPF-I injection well will be plugged and abandoned in accordance with
Enclosure 3 by December 2.5, 198#.

dwn @ 750 psi. Press dropped to 300 psi. Resume pumping @ 15 gpm. Increase
press to 1000 psi @ 30 gpm. Resume pumping, increase press to 1100 psi
@ 3-1/4 bbls. 130 b~ls in staged pressure up to 1200 psi, 1300 psi 1500 psi.
Start max.injecting~ 1600 psi, 6 bbl/min. Ran HP temp log to try ~ interpret
from log which perf's took fluid. Began step-down injection stages. 1200 psi'
@ 5 bbl/min f/30 min, 1000 psi @ 3-1/4 bbl~f/30 min, 800 psi @ 2-1/4 bbl.
Press stabilized @ 2 bbl/min, 600 psi @ 1-1/4 bbl/min & stabilized @ I bbl/mino
Pumped a total of 1312 bbls into perfs from start to finish. Injection fluid

Atlantic Richfield Company proposes to perforate selected intervals within the upper
Cretraceous Sands between 2800' and 3500'. The actual perforated intervals will be
reported on a subsequent report.
A 5000 psi wireline BOP/Lubricator will be utilized with all wireline work. All
perfOrations will be made 4 shots per foot. An injectivity test will be performed
to establish capacity of the well.

DP until jets plugged again. Filled hole thru annulus w/138 bbls mud. Hole
stood full, no gas. Perfd D.C's from'650'-652'. Pumped mud thru holes w/2300
psi @ 5 bbls/min rate. POH. PU new BHA Circ, reamed & washed to 1704'.
Lost returns. Filled annulus to keep hoie full. Pulle~ singles to 958' before
regaining circ. Staged in hole to 1784'. Circ out heavy mud. Washed & reamed
from 1734'-2250'. Ran DIL/SP/GR, BHC/Sonic/SP/GR, 4-arm CAL & FDC/CNL/GR logs.

Spudded well @ 1000 hrs 8/23/79. Drld 17-1/2" hole to 2249' Well started to unload
w/7 stds HWDP (651') & 6-D.C's (184'). Bit @ 835' left in h~le. Opened diverter
valves, closed Hydril, diverting mud & dry gas flow. Pumped approx 15 bbls of mud
in annulus to control well. Gas rate dropped off. Bit plugged. Found ice plug @
267'. Pumped in 250 bbls of 15.0 ppg mud to control well in 12-1/4" x DP annulus.
Gas rate declined. RU dual- BOP on top of HWDP. Started in hole w/I-I/4" tbg. Circ &
thawed ice inside 5" HWDP from 300'-813' Displ mud w/9 bbls of 145°F 50/50 glycol.
LD 1-1/4" tbg. Pumped 10 bbls glycol & ~4 bbls mud down DP until jets plugged again.
Filled hole thru annulus w/138 bbls mud. Hole stood full, no gas. Perfd D.C's from
650'-652'. Pumped mud thru holes w/2300 psi @ 5 bbls/min rate. POH. PU new BHA.
Circ, reamed, & washed to 1704'. Lost returns. Filled annulus to keep hole full.
Pulled singles to 958' before regaining circ. Staged in hole to 1784'. Circ out
heavy mud. Washed & reamed from 1734'-2250'. As of 8/31/79 preparing to run logs.

.September it 1979 - I left ARCO Base Camp at 1:00 PM and left ERA Heliport by
chopper to West Sak at 1:15 PM. Purpose of trip to witness BOP test for ANSD
on well C.P.F no. 1.
When I arrived on job I met M~ Hebert and Kieth Bullard, Supervisors for
ARO0. Circulating of the well was in progress, getting it ready to run
10 3/4' inch casing.
September 2,. 1.979 - The 10 3/4 casing was run to 2238' at 2:00 AM. The
cementing of~ the casing was done by 8:00 ~M. A top job had to be done, and
was finished by 10:00 AM. Nippling down was started. Nippling up BOP's
started about 3:00 PM.
September 3,. !979 - Nippling up BOP's was ccmpleted by 11:30 AM. The pusher
for Rowan Rig 34 %~s Orvin Graff. The test's of BOP's started at 11545 AM and
fir.~shed 2:15 ~4. The results of the BOP tests %~_re made out on an Alaska Oil
and Gas Field Inspection Report.
In Sun~: I witnessed the BOP tests for ARGD at West Sak~ well C.P.F No.1.
When test was ccmpleted there was no failures test's w~re satifactory.

Friday, .August 24, 1979
On this date at appr~tely 1:50 ~4 ~ormie Smith informed m~ that the water
disposal well, ~hich had been drilled to a depth of 2242 in preparation to
set surface pipe, h~ blown out while the rig crew was pulling the drill pipe
out of the hole (857' of drill pipe was still in the well). I caught Wien
flight 19 at 6:30 Pm and arrived at the location at 10:15 PM.
Upon arrival at the location I spoke with Mr. Ken Jones and Mr. Tony Zeito,
drilling foreman for ARO0. I then went to the rig and inspected the diverter
system. Mr. Jones told me that after the initial kick was received the
annular preventer was closed and the two 6 inch diverter lines were opened.
The well blew 9.3 to 9.6 lb/gal drilling mud and gravel out of the well bore.
9~en I arrived the well was blowing gas with 6 to 10 psig on the two 6 inch
diverter lines.
.Saturday, August 25, 1979
At 4:30 AM 125 hbls of 15.0 lb/gal mud was pumped into the annulas of the well
thru a four inch opening below the six inch diverter lines ~hich w~re left
open. Approx/mately 35 to 50 bbls of mud went down the well bore and the rest
vas blown, by the gas, out the diverter lines.
At 5:30 AM 508 hbls of 14.5 lb/gal, mud was pumped into the annulas with
apprcmimately one half going into the well bore and the other half was blG~n
out the diverter lines. At 8:00 AM the pressure on the diverter system was
three to four pounds. Mr. Zeito informed me that an Otis snubbing unit had
been called for and should arrive at 2:30 PM Sunday, August 26, 1979.
The pressure on the diverter gradually decreased during the day and at ll:00~e
PM. the pressure gauge was shcwing 1/2 lb. to 1 psig. At .~:30 PM I was ~_~,~
notified that gas bubbles were escaping up around the outi~sde of the s~ inch
conductor pipe which was set at a depth of 80 ft. I went under the rig floor
and observed the gas bubbles which were escaping but were very weak. A member
of the rig crew, who was stationed under the rig floor to record the diverter
pressure every 15 minutes and to keep a close surveillance on the well
situation, infon~ed me that the bubbles begn to appear shortly before I was
notified.

After the rig'crew c0mpie~ washing out the frozen material in the drill
pipe, ~ hot 14.5 lb/gat, mud was circxLlated until the AR(I) representatives
were satisfied that all of the conta~-~nated material was out of the drill
pipe. -~ mud was then displaced out of the drill pipe with 50%/50% m/xture
of glycol ~u~d w~ter. The i 1/4" wash pipe was then PUlled out of the drill
pipe and laid dc~n. An attempt was made to pu~p down the drill pipe and
circula~ was establi~, but ~ 10-12 bbl$. was tromped the jet in the
drilling ~t plugged. Schl~ was rigged up and a perforating gun was
run in the drill p~ to 600'" 'feet. Three gun runs ~ made before enough
holes ~ere ~ to be ~ throu_~h at an acceptable rate. The mud was
circu/ated ~1 all gas and cut~ were washed out of well bore and the rig
cr~ ~ pulling the drill pipe..
At tlr~s ~, after having witnessed a BOPE test on SOHIO's R-5, I boarded
Wien flight 24 and returned to Anchorage.
~ reported that th~ drill pipe ~d been reran to ~1500 and the well bore
circulated wi~h 13 lb/gal ~ and t_hat they would continue to hottcm.

Enclosed 'is the approved application for permit to drill the
above referenced well.
Well samples, core chips and a mud 1°g are not required. A
directional survey is not required. If available, a tape
containing the digitized log information on all logs shall be
· submitted for -coPying except experimental logs, velocity surveys
and dipmeter surveys.
Many rivers in Alaska and their drainage systems have been
classified as important for the spawning or migration of
anadromous fish. Operations in these areas are subject to AS
16.50.870 and the regulations promulgated thereunder (Title 5,
Alaska Administrative Code). Prior to commencing operations
you may be contacted by the ~abitat Coordinator's office,
Department of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
Chapter 3, Article 7 and the regulations promulgated thereunder
(Title 18, Alaska Administrative Code, Chapter 70) and by the
Federal Water Pollution Control ACt, as amended. Prior to
commencing operations you may be contacted by a representative
of the Department of Environmental Conservation.

Permit to drill.
To aid us in scheduling field work, we would appreciate your

sentative of the commission may be present to witness testing
of blowout preventer equipment before surface casing shoe is
drilled.

ARCO Oil and Gas Company proposes to drill this well to 3500'MD (straight hole)
to provide for disposal of produced water and oily waste in the formation as
found between 2700' and 3500'MD. We foresee the most likely injection intervals
to be between the CP-2 (2720'MD) and CP-1 (3248'MD) as seen on the West Sak #7
log. A 20-3/4" spherical BOP and diverter system will be used on the 16"
conductor (sketch attached). A 13-5/8" 5000 psi spherical BOP, in addition to
blind and pipe rams, will be used on the 10-3/4" casing. The BOP stack will be
pressure tested weekly and operationally tested each trip. Logs will be run
over selected intervals and 7" casing will be set from surface to TD. No
coring is anticipated. A 1-1/4" string for circulating Ethylene Glycol will be
run from 1800' to surface. A completion string consisting of a packer and
landing nipple will be run on 2-7/8" tubing.. A non-freezing fluid will be left
in the annuli within the permafrost. A 5000 psi tree will be installed and the
completed well will be perforated at a later date. A certified plat will be
submitted before spud.

ARCO Oil' and Gas Company proposes to drill this well to 3500'MD (straight hole)
to provide for disposal of produced water and oily waste in the formation as
found between 2700' and 3500'MD. We foresee the most likely injection intervals
to be between the CP-2 (2720'MD) and CP-1 (3248'MD) as seen on the West Sak #7
log. A 20-3/4" spherical BOP and diverter system will be used on the 16"
conductor (sketch attached). A 13-5/8" 5000 psi spherical BOP, in addition to
blind and pipe rams, will be used on the 10-3/4" casing. The BOP stack will be
pressure tested weekly and operationally tested each trip. Logs will be run
over selected intervals and 7" casing will be set from surface to TD. No
coring is anticipated. A 1-1/4" string for circulating Ethylene Glycol will be
run from 1800' to surface. A completion string consisting of a packer and
landing-nipple will be run on 2-7/8" tubing. A non-freezing fluid will be left
in the annuli within the permafrost. A 5000 psi tree will be installed and the
completed well will be perforated at a later date A certified plat will be
submitted before spud.
