Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped January 18t" and January 24t", 2009. The corrosion inhibitor/sealant was pumped February 21St through February 23rd , 2009. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Box 100360 Anchorage, AK 99510-0360 Dear Mr. Dubuisson: The Alaska Oil and Gas Conservation Commission ("Commission") performed an inspection of your well safety valve system (SVS) at Kuparuk River Unit 2W ("KRU 2W") pad on October 2, 2005. Attached is a copy of the Inspection Report. Regulatìon at 20 AAC 25.265 in conjunction with Conservation Order 348 requires all wells in the Kuparuk River field to be equipped with a Commission approved fail-safe automatic surface SVS capable of preventing uncontrolled t10w by shutting off t10w at the wellhead and shutting down any artitìciallift system where an overpressure of equipment may occur. If installed, a subsurface safety val ve must be maintained in working order and is subject to performance testing as part of the SVS. An SVS con1ponent failure rate of 15 percent was identified during the above-noted inspection. Commission test interval requirements provide that the testing frequency will be increased when a tàilure rate exceeds 10 percent. EfIective October 2, 2005 and until an acceptable level of performance is achieved, the safety valve systems associated with wells located on KRU 2W must be tested every 90 days. When testing is scheduled, you must notify the Commission Field Inspector (907-659-2714) at least 24 hours in advance to provide an opportunity to witness the SVS test. RIH with Standing Valve and set in CAMCO 'D' nipple at 7824'. Pressure test tubing to 4500 psi. RIH and pull SV. 4. RDMO Slickline Unit. NOTE: Fill was last tagged on 05/16/86 at 7964' - partially covering the 'A' Sand perforations. Consult with Operations Engineering when fill is tagged to evaluate if a pre-refrac CTU cleanout is required. 5. Obtain a 24 hour well test. 6. RU fracture equipment. Install "Tree Saver". Pressure up 7" annulus to 3500 psi. 7. Carry out all fluid quality control procedures (Onsite Operations Engineer). Obtain samples of: Clean proppant Pre-pad, pad, 2, 6, 10, & 15 ppg stages, flush Record proppant weight before and after the frac. . Pump the 100 bbl pre-pad. Shut down for ISIP and 10 minute pressure fall-off. Calculate tubing and perforation friction from step rate data. 9. Pump fracture treatment per the attached schedule' WHTPE = 4000 psi Fracture Gradient = 0.70 - 5930' TVD Tubing Friction = 160 to 250 psi/1000' - 7830' MD (Dependent on vendor selected) If treating pressure during the job exceeds 6000 psi, reduce the rate to 15 bpm. The treatment is designed such that the slurry stages could be pumped at 15 bpm without a screenout. 10. Rig down frac equipment. Follow attached flowback schedule. 11. After well has completed flowback, obtain a 24 hour test. The well should be left on 'A° Sand only production for about 3 weeks. Obtain a 24 hour well test at one week intervals.
