We were able to replace 12 of the 16 under engaged bolts. Attached are two photos showing the wellhead and the 4 bolts we were not able to change. We were not able to change the two bolts on each side under the casing flanges and while there is excess threads on the bottom nut, we were not able to break the bottom nut free to allow us to adjust the threads in place. In order to change the last 4 bolts we would have to kill the well, set a tubing tail plug, load the tubing with brine and pull the casing flanges off. We do not see any mechanical integrity risk to having 4 of the 16 bolts with 1 to 2 threads of under engagement and seek permission to leave the well in its current configuration. The 30-02A SVS test report lists the Set PSI and L/P Trip numbers for a gas injector, 30-02A is an oil producer.The reported numbers in the 30-02A report appear to be for the test on 3Q-02. Enclosed is the test report for 30-02. We have inspected both 30-02A and 3Q-02 and could not identify the reported deficiency. Enclosed are pictures showing that all of the bolts are engaged on well 30-02A. We believe that the deficiency noted in your report is most likely associated with the 2C-01 test for 7/25/15. Enclosed are pictures of 2C-01. The 2C-01 deficiency was turned over to our Well Integrity group and scheduled for review. Currently they are working to replace the bolts and if not determine what other actions can be taken. I am awaiting their response, if you could revise your deficiency report to the correct well I will have it updated as soon as we get the information. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped September 20~' and October 13~` 2009. The corrosion inhibitor/sealant was pumped October 15th and 31 St + November 1~ and 2"d 2009. The atta.ched spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. On May 19, 1984, the "A" sands of Kuparuk we l I 2C-1 were hydraul ical ly fractured and propped to improve production. The treatment consisted of 414 BBLS of gel led diesel as pad and carrying fluid and 21 ,300 LBS of 20/40 sand as proppant. ARCO A]aska, Inc., proposes to complete this well with the I//estward Well Service Rig //1. The procedure will be as follows: 1.) A mast truck will be utilized to run the CBL/VDL/GR and Gyro. 2.) The rig will be moved on location; the BOPE will be nippled up and tested. 3.) Selected intervals will be perforated within the Kuparuk formation and reported on subsequent reports._ 4.) The well will be completed with 3-1/2", 9.2#/ft., J-55, BTC tubing. A 13-5/8", 5000 psi, SRRA BOP stack will be used on well work. The BOP equipment will be pressure tested weekly and operationally tested each trip. A non-freezing fluid will be left in all uncemented annuli within the permafrost interval. A 5000 psi WP wireline lubricator will be used and tested on all wireline work. The subsurface safety valve will be installed and tested upon conclusion of the job.
