The well passed a non-witnessed MIT-IA to 3300 psi to prove integrity of the tubing and production casing strings. The well no longer meets the criteria of operations under AIO 2C.015. Therefore, CPAI intends on leaving the well shut in until further evaluation of the well status can be completed. The proper notification for repair or continued injection will be submitted as required. Attached is a current 90 day TIO plot, well bore schematic, and a 10-426 form for the passing MIT-IA. Please let me know if you have any questions or disagree with the plan. They attempted to charge the OA to 150 psi and could not charge to over 40 psi. 2H-15 is a water injection well with a waiver for water injection only due to TxIA on gas injection (AIO 2C.015). The well has been shut in since 2023 for reservoir pressure management. A non witnessed, shut in MIT-IA to 3300 psi passed on 5/20/24. CPAI intends to conduct further pressure testing and diagnostics on the OA and confirm OA integrity. The TIO plot, bleed report, and schematics are attached. Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. Attached is a copy of the SVS Test Report. Regulation at 20 AAC 25.265 requires all wells to be equipped with a Commission approved fail-safe automatic surface SVS capable of preventing uncontrolled flow by shutting off flow at the wellhead and shutting down any artificial lift system where an overpressure of equipment may occur. If installed, a subsurface safety valve must be maintained in working order and is subject to performance testing as part of the SVS. AnSVS component failure rate of 11.1 percent was identified during the above-noted inspection. Commission policy, "as outlined in a letter dated November 14, 1995, states that the testing frequency will be increased when a failure rate exceeds 10 percent. The AOGCC-AOGA Safety Valve System Task Force report issued June 13, 2003, supported retention of the 100/0 failure threshold. Effective November 10, 2004 and until an acceptable level of performance is achieved, the safety valve systems associated with wells located on KRU 2H must be tested every 90 days. A 20", 2000 psi annular diverter will be installed on the 16" conductor while drilling the surface hole. The 9-5/8" casing will be set through the West Sak Sands at approximately 2643' MD(2525' TVD). A 13-5/8", 5000 psi, RSRRA BOPE stack will be installed on the 9-5/8" surface pipe. It will be tested upon installation and weekly thereafter. Test pressure will be 3000 psig (1800 psi for annular). Expected maximum surface pressure is 2776 psi. Pressure calculations are based on latest reservoir pressure.
