West Sak Injector 1H-109 (PTD# 218-013) has completed the 30 day IA pressure monitor period. The IA passed
the diagnostic MIT -IA, IA DDT, T POTS and IC POTs. The IA did not show any further indication of pressure fall off during
the monitor period and is suspected that the initial IA pressure loss was thermally influenced. The well will be returned
to normal injection status. As seen on the 90 Day TIO plot, operations charged the IA post a decrease in well head
temperature on the 21n of Oct. They then re -bled the IA on the 25th of Oct to establish a 500 psi differential between
the T and IA. If the well indicated any further issues, the well will be reported to the AOGCC as required per
A102C. Please let me know if you have any questions or disagree with the plan.

off. CPAI's intentions are to leave the well on injection while perfor ' the initial set of diagnostics. If the initial MIT -IA
passes, the IA will be left with an elevated pressure and monitored for 30 to verify IA integrity. Depending on the
results of the testing and monitor period, the proper paperwork for continued in ion or repair will be
submitted. Please let me know if you have any questions or disagree with the plan.

Enclosed please find a spreadsheet with a list of wells from the Kuparuk field (KRU). Each of
these wells was found to have a void in the conductor. These voids were filled with cement if
needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As
per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that
the treatments took place and meets the requirements of form 10-404, Report of Sundry
Operations. The attached spread sheet presents the well name, top of cement depth prior to filling
and volumes used on each conductor.
