Form 10-407 Revised 10/2012 SUNDRY NOTICE 10-404 ConocoPhillips Well CPF-02 Plug and Abandonment January 15, 2015 ConocoPhillips Alaska, Inc. performed Plug and Abandonment operations on CPF-02, commencing operations on October 24, 2014 and completing the Plug and Abandonment on on January 15, 2015. Operations involved pumping 37.8 bbls 15.7 AS1 through a cement retainer set at 1,934 ft KB; 20 bbls were squeezed into the Ugnu perforations and an additional 1.6 bbls were placed on top of the cement retainer. A second cement operation was performed by pumping 135.6 bbls of 15.7 AS1 cement down tubing, through tubing perforations at 1,790-1,794 ft KB, and up both inner and outer annuli. The tree and wellhead were removed and a"top job"performed or tubing (2 gallons) and OA (0.5gallons)to bring cement to surface. State Inspector determined that cement was in good condition and the well was capped at 6 ft, 7 inches below tundra level. Casing strings found to be cut off at a depth of approximately 6 feet below tundra level. Cement was hard in all casing strings cut off at surface. Well abandonment marker plate installed with accurate well information bead welded on the plate as per 20 AAC 25.120. I. Supervisor FROM: Bob Noble SUBJECT: Surface Abandonment Petroleum Inspector KRU CPF-02 - ConocoPhillips Alaska Inc. PTD 1840680 - Sundry 314-480 - January 13, 2015: I traveled to ConocoPhillips Alaska's KRU CPF-02 to inspect the surface abandonment- casing cut-off, cement to surface and marker plate. This well was found to have split conductor pipe during an attempted excavation repair, so CPAI obtained approval to abandon the well. I met with CPAI rep Perry Greenwood and was told that the tubing was cemented to surface this morning. We went to the well to confirm hard cement. I pushed on it with my thumb and it went - all the way in. The outside diameter of the marker plate was the same as the inside diameter of the conductor. The plate apparently was made based on the nominal size of the conductor, not taking into account the deformation due to the split. Perry said they will make another marker plate. - I told him to send out another notification for inspection when the cement was set up. - Summary: I inspected the surface abandonment of KRU CPF-02. The well was not ready to- complete abandonment operations. I witnessed pressure tests of the tubing and cement plug isolating the injection perforations (MITT), and a combination pressure test of the IA and OA which were in communication (CMIT IAxOA). - The first attempt at MITT failed the 1500psi pressure test, losing 125 psi during the first 15 minutes and 50 psi during the second 15 minutes of the test (175 psi total pressure loss; failed since >10% loss). The following is a summary of the reasons for abandoning said well and a brief review of the procedure we will follow. The CPF-2 Water Disposal Well was drilled to a depth of 2589' in May 1984 and completed in the Ugnu formation as a dedicated water disposal site (see attached well sketch). It was no longer needed for excess water disposal after 1987 and at that point the well was SI and pickled. It has been dormant since that time but was MIT'd every four years as required by State regulations. Last MIT was done in Oct. of 2000. CPF-2 expansion plans have moved this well P & A to a high priority as new modules are being added which would severely limit access to this well site for future wellwork (including P & A operations). As there are no future plans to reactivate this well it will be permanently dismantled. The Ugnu formation is not anticipated to be overpressured from previous injection but a bottomhole pressure will be taken before starting P & A work. Plans for Plug and Abandonment are as follows: 1. Rig up Slickline: Tag PBTD and obtain Static BHP and temperature. 2. Rig up Pump Unit: Perform MIT of 9 5/8" casing. Follow with injectivity test down tubing. 3. Pump Unit: Rig up to IA , pump 9.6 brine to displace glycol down IA and up OA annulus taking returns to open tank at surface. Verify pathway is open from IA to OA by volume of glycol displaced. 4. Rig up Cement Unit: Pump initial cement plug to cover perfs. Cover perfs to within 50' (2010'md) of tubing tail. We find the black smoke summary to be incomplete. It should include the actual times for each date that flaring is less than 24 hours, and the amount and dew point of gas flared. We would also appreciate information on the flare tip- size, shape, and means of combustion assist (air or steam injection, etc.). The report raised more questions than it answered. More specifically: I) Was the stock tank vapor (STY) compressor the only source of black smoke (when it was down)? Submit a process flow diagram showing the function of the STV compressor. 2) On November 25, 1984 what caused the plant shutdown and could the "A" STV compressor have been kept on line? Could the "B" STV compressor have handled the load? 3) On November 26 and 27, 1984 what where the motor problems? How is the motor related to the STV compressor? 4) On NoVember 28, 1984 whY wOuld checking the motor cause the "B" STV compressor to be down? 5) On November 29, 1984 what instrument tripped and why did it cause flaring? 6) On December 6 - 11, 1984 what kind and type of maintenance was involved? Why did it cause the STV compressor to be down? 7) On December 12, 1984 what valve was adjusted and why did it cause the STV compressor to be down? RECEIVED AN 2 1985 Alaska 0il & 6as 0OHS. Commission Anchorage 8) On December 16 and 17, 1984 what vibration was checked and why would the check cause the STY compressor shutdown? 9) On December 17 - 28, 1984 what was the motor problem or alignment and why would the STY compressor be shutdown? ! 10) On December 29, 1984 what was the instrument, what was its problem and why did it shutdown the STY compressor? 11) On December 30, 1984 what is the "B" STY piston? What is meant by "wreck" and why did this cause the STV compressor to shutdown? 12) On December 31, 1984 and January 1, 1985 what valve was wrecked and why did this cause the STV compressor to be down? 13) On January 2, 1985 what where the motor problems? How is the motor related to the STV compressor? 14) On January 3, 1985 why would starting "A" STV compressor cause gas to be flared? 15) On January 5, 1985 what valve was replaced and why would it cause gas to be flared? For each date we need to know the steps that were taken to prevent reoccurrence. Your answers to all of the above questions might help clear up the confusion about the large volume of gas flared during start-up of CPF #2.
