Anctxxa~Q t a~ II b~ Enclosed please find a spreadsheet with a list of wells from the Kuparuk field (KRLn. Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped January 16, March 21, 2010. The corrosion inhibitor/sealant was pumped Apri115, 16 and 17, 2010. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Fluid level is @ 66" below top of flutes. Overall conductor length removed is 99" from top of flute. Removed cement from surface casing and cleaned for inspection. No other holes was visually noted. T/I/O = SI/0/0. Welded 4' "A" band from bottom of flutes to collar. Welded 6" and 4" double "B" band @ 92" below flutes. All bands fabricated from 10" X65.500 wall pipe. Called for inspection by Kakivik. Welded second part of "A" band, fabricated from 10" X65 .500 wall pipe. Second part of "A" band MT inspected by Kakivik.Welded 48" sleeve fabricated from 12" X65 .500 wall pipe. Overall length of patch is 92". Welding was performed per M.E. specs. Kakivik was called for inspection. D MIT-OA (passed), Pre T/I/O = SI/0/0, pressured up to 300 psi on OA. No leaks observed. Pressured up to 1800 psi with 1 bbl of diesel. Rigged up triplex to injection manifold and established several LLR with 1 -1.5 bbl diesel, 1 liter of diesel 880 psi for 18 seconds; 1 liter of diesel 830 psi for 18 seconds; 1 liter of diesel 790 psi for 19 seconds; 1 liter of diesel 740 psi for 19; 1 liter of diesel 700 psi for 19 seconds; 1 liter of diesel 690 psi for 20 seconds. Start to inject platelets, 100 5 mm cubes pumped for 10 seconds; 1200 2 mm cubes for 20 seconds; 30 mesh crumb. After 1 min the pressure didn't exceed 450 psi and installed check valve. Monitor for a short period of time OA reached 1210 psi; 15 min 1200. Triplex pump bad and swapped out with another and pressured up OA to 1500 psi, locked in pressure for 5 min. MITOA to 1800. Start MITOA 1800 psi, T /I /O= SI/0/1800, 15 min T /I /O= SI/0/1765, 30 min T /I /O= SI/0/1745, 45 min T /1 /0= SI/0/1740. Bleed OA down to 530 psi. Pumped approximately 1/2 bbls of gel diesel. An~ho~ap~ SUNDRY NOTICE 10-403 APPROVAL REQUEST 11-06-09 This application for Sundry approval for Kuparuk well 2B-16 is for the repair of a surface casing leak. The well is a gas lifted producer and was drilled in Aug. 1984 and completed in April 1985. On 03/03/08 a surface casing leak was confirmed and reported to the AOGCC while preparing the well for cement top out and sealant. Diagnostics confirmed a leak in the surface casing approximately 7" below the outer annulus valve. With approval, repair of the surface casing will require the following general steps: 1. Shut-in and secure well. Set positive pressure plug and fluid pack the IA as needed to remove all gas. Freeze protect the T and IA. Perform mechanical integrity test and draw down test on tubing plug to verify barrier. 2. Excavate wellhead location. 3. Install nitrogen purge equipment. 4. Cut away the conductor casing and remove cement around surface casing leak for inspection. 5. Notify AOGCC Field Inspector of repair operation so they may witness the event. 6. Repair damaged surface casing integrity. Repair options include sleeve patch, seal weld, lap patch, deposition weld and will be determined after inspection of the casing damage. 7. QA/QC the repair welds and MIT-OA to verify integrity 8. Install anti-corrosion coating on exposed surface casing. 9. Patch the conductor and top off the conductor void with cement and corrosion inhibiting sealant. 10. Backfill gravel around the wellhead. 11. Remove plugs and return the well to production. 12. Tag fill to insure perforations are clear. b. Insert dummy valves in all mandrels. c. Fill backside with diesel. d. Pressure test tubing. e. Obtain 24 hr. stabilized welltest off gas lift. 2. SI well for at least 24 hours before pumping spear. 3. Pump diesel/Musol spear per attached schedule. 4. Record ISIP. Be SI well overnight. Pressure up annulus to 2750 psi. Note: Annulus pressure will rise as the treatment is pumped. The annulus pressure should not be allowed to rise above 3500 psi during the job. The maximum allowable WHTP will be 5700 psi with 3000 psi on the annulus. At 3500 psi on the annulus the maximum allowable WHTP would be 6200 psi. 7. RU treating equipment, including treesaver. Be Pump fracture treatment per attached schedule. Expected maximum WHTP is 4800 psi. Monitor and record the wellhead tubing pressure during and after treatment. Well should remain shut-in for 5 hours after treatment to allow the fracture to heal and prevent flowback of proppant. 11. Bring well back 100 psi per day. Record tank straps, choke settings, and WHPs during entire flowback period. 12. Take BS&W samples after every choke change. Visually inspect samples for presence of frac sand. If frac sand is observed, increase WHP 100 psi and contact Operations Engineering. Well can be turned into CPF when BS&W is below 2%. 13. Many rivers in Alaska and their drainage systems have been classified as important for the spawning or migration of anadromous fish. Selected intervals will be logged. A 20", 2000 psi annular diverter will be installed on the 16" conductor while dril]ing the surface hole. The 13-5/8", 5000 psi, RSRRA BOPE stack will be installed on the 9-5/8" surface pipe. It will be tested upon installation and weekly thereafter. Test pressure will be 3000 psig (1800 psi for annular). Expected maximum surface pressure is 2683 psi. Pressure calculations are based on virgin reservoir pressure. Surface pressure is calculated with an unexpanded gas bubble at surface,.including temperature effects. Casing will be tested to 2000 psi prior to releasing the drilling rig. Non-freezing fluids will be left in unoemented annuli through the permafrost interval. 2B-16 will be 60' away from 2B-15 at the surface. There are no closer wellbores anywhere downhole.
