Jim,
Historically the 4-year MITs were completed within the calendar year that the test was due. It wasn`t too Tong ago that we
changed the system to track the 4-year tests by due date to complete by that time. My only explanation for this wel9 is that
the MIT was planned to be completed before December of 2003, but the well was shut-in in Juiy. This type of tracking
issue and oversight will not happen now given the way we currently track 4-year M1Ts.
Please let me know if there is any action that you would like me to take.

Just completed a review of MITs due and found just one questionable well: PBU PWDW1-1 (PTD 176-062). Our info
shows the last MIT was 3/29/2003; test was due 3/29/2007. Injection records indicate the well continued injecting until SI
July 2007. Do you have a record of an MIT done between the previous (3/29/2003) and the due date? Thank you.

Tast"s Details
I traveled to BPXs Flow Stations 1 & 3 and witnessed the 4 year MIT on wells PWDW 1-1 & PWDW 3-1. The pretest
tubing and casing pressures were observed and found to be stable. The casing was then pressured up and observed for
30 minutes with both wells demonstrating good mechanical integrity.

Well passed a state witnessed annular communication test on the tubing
x 9-5/8" annulus.
Static bottom hole pressure survey (SBHPS) - 2721 psig @5990'
Ran a spinner survey. A channel was identified 168' up from the
perforations
Well passed 13-3/8" annular communication test
4-1/2" tubing was changed out with 7". Well was reperforated 100' (5926'-
5936', 5943'-5960', 5969'-6004', 6157'-6167', 6296'-6136', 6376'-6388')
SBHPS and temperature survey (TEMP) - 2846 psi @5900'SS, TEMP
indicated injection from 5700'-6050'MD which was interpreted as either a
channel or fracture.
SBHPS - 2795 psi @5925'SS.
SBHPS & TEMP - 2772 psi @ 5825'SS. Indicated a channel to
5600'MD.
Added 30' of perforations (5250'-5280').
9-5/8" passed mechanical integrity test (MIT) at 2000 psi, 13-3/8" failed
MIT.
Passed state-witnessed MIT
Ran a spinner survey. Showed lower 4 sets of perforations covered in
schmoo.
Passed 9-5/8" MIT.
Tagged obstruction in tubing @3227'. Washed it down to SSSV. Pulled
SSSV, but lower part fell downhole.
Fished flapper housing and SSSV coil spring.
Attempted to fish spear that was being used to fish SSSV.
Milled spear and SSSV. Remainder fell downhole.
Well passed inner annulus MIT (MITIA) at 1500 psi.
Tagged lower part of SSSV @ 321 I'SLM.

PROPOSED WORK PROCEDURE
CTU FILL CLEANOUT & MUD ACID STIMULATION
A brief description of the proposed work on this well follows'
(1) RU CTU & perform a Fill Cleanout down to PBTD at 6505' MD, then wash
up one pass across the perforations with 75%/25% Diesel/Xylene,
followed by one upward pass washing with 15% HCl. POOH w/CT,
while displacing with one well volume of 1% NH4CI Water. RD CTU.
(2) RU equipment & PT to 3000 psi. Perform a Mud Acid Stimulation.
Pump fullbore as follows:
* 25 bbls 1% NH4CI Water @ 25 bpm, followed by
* 250 bbls 12% HCI- 3% HF w/0.6 ppg bezoic acid flakes
for diversion @ 25 bpm, followed by
* 560 bbls 1% NH4CI Water @ 25 bpm.
(3) Rig down & release pumps & equipment.
Return well to disposal injection service & report stimulation results.

APPLICATION FOR SUNDRY APPROVALS

WORKOVER PROCEDURE
OBJECTIVE: The purpose of this workover is to remove the existing 4-1/2"
production tubing and 7" glycol/water circulating string and replace them
with a 7" production tubing string and 7" 5M tree. The existing perfora-
tions will be isolated via wireline set blanking plug in the existing packer
tailpipe. The packer will not be replaced, however, the 9-5/8" wellhead
packoff will be replaced. Perforations will be added within the existing
perforated interval following the workover. Fill in the 9-5/8" casing below
the packer will be circulated out via coiled tubing prior to the workover.
1) RU slickline. Pull the 4-1/2" SSSV.
2) RIH w/Otis "PXN" blanking plug on "XN" lock and lock plug in "XN"
nipple at 3317'MD. Pressure test plug to 3000 psi.
3) POOH, RD slickline.
4) RU electric line. RIH, perforate 4-1/2" tubing above Baker packer at
3210'MD. POOH, RD electric line.
5) MIRU. Displace the 4-1/2" x 7" x 9-5/8" annul i with seawater. Set
BPV. ND tree. NU BOPE and test. Retrieve BPV.
6) Release 4-1/2" tubing from packer and POOH. LD 4-1/2" tubing and
tubing hanger.
7) Pull 7" mandrel hanger packoff and engage 7" mandrel hanger. POOH, LD
7" hanger and 7" circulating string.
8) Close blind rams, pressure 9-5/8" casing to 2500 psi for 30 min. If
9-5/8" casing will not hold pressure, PU 3-1/2" DP and 9-5/8" RTTS and
isolate 9-5/8" casing leak. Check FOs at 2336 'MD and 2466 'MD first.
9) MU new seal assembly on 7", 26#, L-80 LTC 8rd tubing and RIH. Sting
into Baker packer. Space out and land hanger. Test annulus and tubing
to 2500 psi and 3000 psi, respectively.
10) Set BPV. ND BOP. NU new 7" 5M tree and test.
11) RD, RR. Pull BPV.
12) RU slickline. Retrieve "PXN" blanking plug. Run SSSV.
13) Production will add perforations after rig release.

BOP STACK TEST
1. FILL BOP STACK AND MANIFOLD WITH ARCTIC DIESEL.
2. CHECK THAT ALL HOLD-DOWN SCREWS ARE FULLY RETRACTED.
PREDETERMINE DEPTH THAT TEST PLUG WILL SEAT AND SEAT
IN WELLHEAD. RUN IN LOCK-DOWN SCREWS IN HEAD.
CLOSE ANNULAR PREVENTER AND CHOKES AND BYPASS VALVES
ON THE MANIFOLD.
4. TEST ALL COMPONENTS TO 250 PSI AND HOLD FOR 10 MINUTES.
5. INCREASE PRESSURE TO 2500 PSI AND HOLD FOR 10 MINUTES.
BLEED TO ZERO PSI.
6. OPEN ANNULAR PREVENTER AND CLOSE TOP SET OF PIPE RAMS.
7. INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 10 MINUTES.
8. CLOSE VALVES DIRECTLY UPSTREAM OF CHOKES. BLEED DOWNSTREAM
PRESSURE TO ZERO PSI TO TEST VALVES. TEST REMAINING UNTESTED
MANIFOLD VALVES, IF ANY, WITH 5000 PSI UPSTREAM OF VALVE AND
WITH ZERO PSI DOWNSTREAM. BLEED SYSTEM TO ZERO PSI.
9. OPEN PIPE RAMS. BACKOFF RUNNING JT AND PULL OUT OF HOLE.
CLOSE BLIND RAMS AND TEST TO 5000 PSI FOR 10 MINUTES. BLEED
TO ZERO PSI.
10. OPEN BLIND RAMS AND RECOVER TEST PLUG. MAKE SURE MANIFOLD AND
LINES ARE FULL OF ARCTIC DIESEL AND ALL VALVES ARE SET IN THE
DRILLING POSITION.
11. TEST STANDPIPE VALVES TO 5000 PSI.
12. TEST KELLY COCKS AND INSIDE BOP TO 5000 PSI WITH KOOMEY PUMP.
1:3. RECORD TEST INFORMATION ON BLOWOUT PREVENTER TEST FORM. SIGN
AND SEND TO DRILLING SUPERVISOR.
14. PERFORM COMPLETE BOPE TEST ONCE A WEEK AND FUNCTIONALLY
OPERATE BOPE DAILY.

Atlantic Richfield Company proposes to perforate the following intervals:
5926'-5936'; 5943'-5960'; 5969'-6004'; 6157'-6167'; 6296'-6316'; and 6376'-6388'
instead of interval 3505' to 3596' All intervals are still in the Cretaceous
Sand intervals. By starting with ~eeper intervals than originally specified we
should be able to stay further away from any possible shallow fresh water zones
as well as have the chance of perforating higher intervals at a later date in
case these lower intervals should plug off during injection of produced waters.
This perforating could be done without a workover rig whereas if the original
interval were used and then plugged off, a workover rig would be required. A
5000 'psi WP wireline lubricator, tested to working pressure, will be utilized
with all wireline work. All perforations will be made with hollow carrier
type guns at 4 shots per foot. A short term injection test will be done 1i9 i'~

Permission for disposal of produced Sadlerochit Formation water
and utility wash down water (no more than 5% of total injected
fluids) is hereby requested as per stated in llAAC 22.250.
Injection would start at the time when the field comes on
production at start up of flow to the pipeline.
Injection would be into tertiary aged Z sand and geologic
data indicates that there are no fresh water sands in the
area which might be contaminated.
A copy of the Department of Environmental Conservation
letter from Mr. Ronald G. Hansen concerning their ruling
on these wells is attached for your convenience.

Arco to Drill Site W. D No. I-1.~o witness another BOP test for Arco.
J had called Hoyle Hamilton for confirmation of this, Monday morning,
September 13, 1976~ from Dritl Si.re No. 9-7.
They started BOP tests on W.~D No. I-1. The manifold was tested First,
they have 7 Rucker Shaffer wheel type valves on manifold and 10 flanges
which were tested to 5000 psi, They also tested the manual choke to 5000 psi.
Then the Swaco superchoke was tested to 5000 psi. All the valves on the
manifold were tested to 5000 psi and held OK. Then they tested the annular
preventer to 5000 psi. They then tested the pipe rams and one inside choke
valve and check vatve was tested'to 5000 psi.. Then the inside kill valve
was tested to 5000 psi. The test stem was pulled and blind rams were tested
to 5000 psi. They also tested upper and lower kelly to 5000 psi, and also
tested the drill stem safety valves on floor to 5000 psi. These were dart
and bait valves, AIl the valves on the manifold and stack are Rucker Shaffer
valves except the superchoke. The two gates and annular preventer are also
Rucker Shaffer type. The ~ell sign was up with all data OK. The master
gauge on the accumulator was 2750 psi. The N2.backup bottles were all
1900 psi. They had actuated the master controls and panel controls on floor
which work fine. They have a stee] fuel tank. not bermed. Their mud pit
is a steeJ enclosed box so they use the pump truck when it needsccleaning.
The drilt site was in very good shape.

equipment working properly and al1 tests good and rig in very good shape.

Enclosed is the approved application for permit to drill the above referenced
well at a location in Section 31, Township llN, Range 1SE, UM.
Well samples and core chips are required. A mud log is not required. A
directional survey is not required.
Many rivers in Alaska and their drainage systems have been classified as
important for the spawning or migration of anadromous fish. Operations in
these areas are subject to AS 16.50.870 and the regulations promulgated
thereunder (Title 5, Alaska Administrative Code). Prior to commencing
operations you may be contacted by the Habitat Coordinator's office, Depart-
ment of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46, Chapter 03,
Article 7 and the regulations prolmulgated thereunder (Title 18, Alaska
Administrative Code, Chapter 70) and by the Federal Water Pollution Control
Act, as amended. Prior to commencing operations you may be contacted by
a representative of the Department of Environmental Conservation.
Pursuant to AS 38.40, Local Hire Under State Leases, the Alaska Department
of Labor is being notified of the issuance of this permit to drill.
To aid us in scheduling field work, we would appreciate your notifying this
office within 48 hours after the well is spudded. We would also like to be
notified so that a representative of the Division may be present to witness
testing of blowout preventer equipment before surface casing shoe is drilled.

APPLICATION FOR PERMIT TO DRILL OR DEEPEN
Produced Water Disposal Well # 1-1
The Atlantic Richfield Company proposed to drill this disposal well to
6600'MD (straight hole)to provide shallow zones from 4100' to 6000'
for produced water disposal. A 20-3/4" spherical BOP will be used on
the 20" conductor. A 13-5/8" 5000 psi spherical and 5000 psi blind
and pipe rams (RSRRA) will be used on the 13-3/8" casing. The BOPE
will be pressure tested weekly and operationally tested each. trip.
Logs will be run over selected intervals and a maximum of 120' of
cores will be drilled from zones of interest. A 7" circulating string
with ethylene glycol will be run from 2400' to the surface. A com-
pletion string comprised of a Baker Oil Tool Packer and Locator tubing
seal assembly and Otis 4½" RQB landing nipple with RQ lock and PX prong
will be run on 4½" Hydril CSCB tubing. A non-freezing fluid will be
left in all annuli within the permafrost interval. A 4" 5000 psi tree
will be installed and the completed well left unperforated. This well
will serve for disposal of all produced water at Flow Station #1.
