This memo will remain at the front of the subject well file.
Our adopted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies in
the treatment of these kinds of applications for permit to ddll. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The APl number and in some instances the well name reflect the number of preexisting
reddlls and or muitilaterais in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to ddll.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95.
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the APl numbering methods described in AOGCC staff
memorandum "Multi-lateral (weiibore segment) Ddlling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

Enclosed is the approved application for permit to dr~.ll the
above referenced well.
If coring is conducted, a one cubic inch chip from each foot of
recovered core i~ required. Samples of well cuttings and a mud
log are not required. A continuou~ directional survey is
re,uired a~ per 20 AAC 75.050(b)(5). If available, a tape
co'raining the digitized log information shall be submitted on
all logs for copying except experimental logs, velocity surveys
~nd dipmeter s~rveys.
Many rivers in Alaska and their drainage systems have been
classified as important for the spawning or migration of
anadromous fish. Operations in these areas are subject to
AS 16.05.870 and the regulations promulgated thereunder
(Title 5, Alaska Administrative Code). Prior to commencing
operatton~ you may be contacted by the Habitat Coordinator's
Office, Department of Fish and Game.

tion equipment so that a representative' of the Commission may be
present to witness testing of the equipment before the surface
casing shoe is drilled. ~¢aere a diverter system is required,
please also notify this office 24 hours prior to co~mmencing
equipment installation so that the Co, mission may witness testing
before drilling below the shoe of the conductor pipe.
In the event of suspension or abandonment, please give this
office adequate advance notification so that we may have a

22. Describe proposed program:
ARCO Alaska, Inc., proposes to directionally drill this well to 10,169'MD (9196'TVD) to produce from the
Sadlerochit zone. A surface diverter will be used on this well. A 13-5/8", 5000 psi, RSRRA BOP stack will be
installed. The annular preventer will be tested to 3000 psi and rams tested to 5000 psi. The BOP equipment
will be pressure tested each week and operationally tested each trip. Logs will be run across selected
intervals. A string of 5-1/2", 17#, L-80 tubing, packer, gas lift mandrels, and necessary provisions for
subsurface safety equipment will be run. The packer will be set above the Sadlerochit. A non-freezing fluid
will be left in all uncemented annuli within the permafrost interval. A separate Sundry Notice will be
submitted when the well is to be perforated. DS 14-37 will be 120' away from DS 14-36 at the surface. THere
are no other wellbores anywhere down hole within 200' of the proposed DS 14-37 well course.

Accumulator Capacity Test
1. Check and fill accumulator reservoir to proper
level with hydraulic fluid.
2. Assure that accumulator ~ressure is 3000 psi ~///
with 1500 psi downstream of the regulator.
3, Observe time, then close all units simultane-
ously and record the time and pressure remaining
after all units are closed with charging pump
off.
4. Record on IAOC report. (The acceptable lower
limit is 45 seconds closing time and 1200
psi remaining pressure.
13-5/B" BOP Stack Test
1. Rill BOP stack and manifold with a non-freezing
liquid.
Z. Check that all hold down screws are fully
retracted. Nark running joint with predeter-
mined plug seating depth. Run test plug on
drill pipe and seat in wellhead. Run in lock
down screws.
3. Close annular preventer and chokes and bypass
valves on manifold.
4. In each of the following tests, first hold
a ZSO psi low pressure test for a minimum of
3, minutes, then test at the high pressure.
S. Increase presure to--and hold for a
minimtm~ of 3 minutes.--Bl~eed pressure to zero.
$. Open annular preventer and close t~'
pipe rams. Increase pressure to~uuu~_J
· and hold for at least 3 minutes. Blee~F~res-
sure to zero.
7. Close valves directly upstream of
open chokes. Increase pressure to~
& hold for at least 3 minutes. Bleed pressure
to zero.
8. Open top set of pipe rams then close bottom set
of pipe rams. Increase pressure to 5000 psi
below the bottom set of pipe rams and hold for
at least 3 minutes.
9. Test remaining untested manifold valves (if
any) with 5000 psi upstream of valve and zero
pressure downstream.
10. 81eed pressure to zero. Open bottom set of pipe
rams.
11. Back off running joint and pull out of hole,
Close blind rams and test to 5000 psi for at
least 3 minutes.
12. Bleed pressure off all equipment. Open blind
rams. Make sure manifold and lines are full of
non-freezing liquid and all valves are set in
drilling position.
13. Test standpipe valves to 5000 psi.
14. Test Kelly cocks and inside BOP to 5000 psi with
Koomey pump.
15. Check operation of Degasser.
16. Record test information on blowout preventer
test form. Sign and send to Drilling Super-
visor.
17. Perform comolete BOPE test once a week and
functionally operate BOPE daily.

& hold for at least 3 minutes. Bleed pressure
to zero.
8. Open top set of pipe rams then close bottom set
of pipe rams. Increase ~ressure to 5000 ~si
below the bottom set of pipe rams and hold for
at least 3 minutes.
g. Test remaining untested manifold valves (if
any) with 5000 psi upstream of valve and zero
pressure downstream.
10. 81eed pressure to zero. Open bottom set of pipe

Close blind rams and test to SO00 psi for at
least 3 minutes.
12. Bleed pressure off all equipment. Open blind
rmms. Make sure manifold and lines are full of
non-freezing liquid and all valves are set in
drilling position.
13. Test standpipe valves to 5000 psi.
14. Test Kelly cocks and inside BOP to 5000 psi with
Koomey
15. Check operation of 0egasser.
16. Record test information on blowout preventer
test form. Si'gn and send to Drilling Super-
v i sor.
i7. perform complete BOPE test once a week and
functionally operate 8OPE daily.
