Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in anY attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported

PLUG AND ABANDONMENT
SUMMARY OF OPERATIONS
Summary of Operations:
4-21-92: RU WLU and RIH with SB. Tagged PBTD @ 9670' SLM. POOH.
5- 12-92:
RU pumping and cementing equipment. Displaced CaCI2 water
from annulus and tubing with 845 bbls SW, 50 bbls diesel, 267
bbls MW, and 20 bbls FW by pumping down the tubing x casing
annulus @ 6.1 bpm. Circulated 24 bbls cement down tubing and
took returns on annulus, leaving a cement plug in the tubing and
tubing x casing annulus from 8270' - 8822'.
5-13-92: RU WLU and RIH with SB. Tag TOC @ 8270'.
5-14-92:
Pressure tested tubing and 9-5/8" casing to 1500 psi for 30 min.
with no bleed off. .
10-14-92: RU ELU and cut tubing @ 170'. POOH.
10-18-92: Set BPV. NU BOP's. Pulled BPV. Attempt to pull tubing hanger.
10-19-92:
Pulled tubing hanger and 170' of tubing, RU ELU and tagged top
of tubing stub @ 169'. POOH. MU and ran 9-5/8" BP and set
same @ 159'. POOH.
10-20-92:
RU and RIH with 1-1/2" tubing (6 jts @ 2 l'/jt). Mixed and
pumped 17 bbls cement. Used vac hose and sucked out cement
to 22' below top ram (3' below tundra). RD.
10-21-92: Set up for fire watch. Welder cut 9-5/8" casing off at 92.5"
below the top ram. 9-5/8" dropped 17" after cutting.
10-22-92: ND BOP's. ND casing spool. Attempt to pull tubing spool.
10-23-92:
Excavated cellar and pad to 5' below tundra level. RU welder
and cut 20" casing at tundra level. Cut 13-3/8" casing and cut 9-
5/8" casing stub at 5' below tundra level.
10-24-92:
Made final cuts on 20", 13-3/8" and 9-5/8". Installed and sealed
a plate on top of the 13-3/8" casing stub. Installed and sealed
the abandonment plate on top of the 20" casing @ 3' below the

pressure test the 9-5/8" x 13-3/8" annulus, cut and pull
the 5-1/2" tubing at 150' and circulate a cement plug from
150'- 0' MD in the tubing and tubing x 9-5/8" annulus to
permanently plug and abandon the well.

Contact AOC~C representative a minimum of 24 hours prior to
commencement of Plugging Operations.
RU pump unit and reverse circulate the CaCL2 out of the tubing and the
5 x 9" casing annulus with 8,5 ppg SW.
Circulate a cement plug from 8822' - 8500' MD. WOC for I2 hours,
RU slickllne and tag and test cement plug to 1500 psi on both the
tubing and annulus..
Test the 9-5/8" x I3-3/8" annulus to 1500 psi.
RU ELU. Cut and pull the 5-1/2" tubing at 150' MD,
Circulate a cement plug from 150' to surface in the 9-5/8" casing.
RU. slickline and tag TOc. Test cement plug to 1500 psi.
Remove welIhead and cut off ~l! casing strings at a point 6'
below pad level, Weld a steel plate across the conductor. The steel
plate will have the following information bead welded on top:

TRACT WELL T-3C
APPLICATION FOR SUNDRY APPROVALS
ARCO Alaska, Inc., requests an extension of "suspended" status for Tract
Well T-3C. Economic evaluations are currently underway to determine the
feasibility of re-entry into this well for the purpose of exposing the Wahoo
interval and completing it for production.
We al so request an exception to the surface plug requirement of
20AAC25.110(C). Tract Well T-3C contains a non-freezing, kill-weight fluid
and has not been perforated. We are requesting this exception until such
time the well is either deepened or plugged and abandoned.

Dear Mr. Ruedrich:
A review of our records .shows that the above referenced wells are
suspended. All of the wells have tubing installed, and KRU West
Sak SW-PT No. 1, East Bay State No. 1 and West Sak No. 24 have
perforations open to the well bore.
Section ll0(a) of the regulations states the Commission will, in
'its discretion, ap. prove the suspension of a well if the well is
capable of producing hydrocarbons in paying quantities or is
reasonably demonstrated to have future value as a service well.
Section 105(e) states that "[e]ach well to be abandoned or
suspended ~rust be plugged .... to prevent the movement of fluid
either into or between freshwater and hydrocarbon sources and to
prevent all other fluid migration within the well bore."
In compliance with Section 170(a) of the regulations, "[t]he
location of an abandoned well onshore or upon a historically
stable island must be cleared within one year following well
abandonment .... " Section 170(b) provides the means for an
extension of time if a valid reason exists, and the wellbore is
properly plugged,

bring the status of these wells into compliance with the new
regulations~ (2) justification for keeping the wells in suspended

No 341 for Deadhorse arriving at 7:10 pm Weather- +36°F
light wind, fog.
M~onday,. August 15, 1983: Inspection of forty-one abandoned and
suspended 'wells began at 9:00 am and continued until 3:30 pm on
Wednesday, August 17. Separate reports cover abandoned wells and
.a BOPE test on ARCO's DS 4-20 Monday evening.

information shall be submitted on all logs for copying except

Accumulator Capacity Test
1. Check and fill accumulator reservoir to proper
level with hydraulic fluid.
2. Assure that accumulator pressure is 3000 psi
with 1500 psi downstream of the regulator.
3. Observe time, then close all units simultan-
eously and record the time and pressure re-
maining after all units are closed with
charging pump off.
4. Record on IADC report. (The acceptable lower
limit is 45 seconds closing time and 1200 psi
remaining pressure.)

13-5/8" BOP Stack Test
Fill BOP stack and manifold with a non-
freezing liquid.
Check that all hold down screws are fully re-
tracted. Predetermine depth plug will seat &
mark running joint. Run test plug on drill
pipe and seat in wellhead. Run in lock down
screws in head.
3. Close annular preventer and chokes and ~ypass
valves on manifold.
4. In each of the following tests, first hold a
250 psi low pressure test for a minimum of 3
minutes, then test at the high pressure.
5. Increase pressure to 3000 psi and hold for a
minimum of 3 minutes. Bleed pressure to zero.
6. Open annular preventer and close top set of
pipe rams.
7. Increase pressure to 5000 psi and hold for at
least 3 minutes. Bleed pressure to zero.
8. Close valves directly upstream of chokes, open
top set of pipe rams and chokes, then close
bottom set of pipe rams. Increase pressure to
5000 psi and hold for at least 3 minutes.
9. Test remaining untested manifold valves (if
any) with 5000 psi upstream of valve and zero
pressure downstream.
10. Bleed pressure to zero. Open bottom set of
pi pe rams.

ll. Back off ~unning joint and pull out of hole.
Close bl£~ rams and test to 5000 psi for at
least 3 minutes.
