3/2/2013 Performed CMIT ?poops!, passed. Sat down at 6,201 -ft RKB with Baker 2.69" Bo mill, milled stringers to
6,290 -ft RKB and soft cement to 6,420 -ft RKB. Milled out Composite Bridge Plug at 6,420 -ft RKB and pushed
out tubing tail to 6,820 -ft RKB. Freeze protect well. Ready for MIT -IA.

2U -15 Cement Packer Repair
Permit to Drill #: 184 -248
The objective of the proposed 2U -15 Cement Packer Repair is to restore IA integrity and allow the well to be
put back on production. An ultrasonic leak detect log identified fluid movement past the top packer, indicating
that it was leaking. If the cement packer repair restores IA integrity and allows the well to be brought back
online, a RWO would be postponed, and an average 1 year rate of 315 BOPD could be realized.
Proposed Cement Packer Repair Procedure: .
Slick Line / Little Red: (Set Bridge Plug)
1. Pull catcher @ 6,483' KB and WRP @ 6,485' KB.
2. Dummy drift for Baker Quik -Drill composite bridge plug.
3. RIH and locate GLM @ 6,396' KB. Record Depth.
4. Using the DPU, and necessary depth correlations, RIH w/ composite bridge plug and set @ 6,420'
KB. (Middle of full joint of tubing below GLM @ 6,396' KB).
5. MIT -T to 2,500 psi, to test tubing and ensure plug is holding.
6. RIH and pull DMY valve from GLM @ 6,396' KB.
7. Pump down Tubing and up IA: 250 bbls of Seawater, followed by 71 bbls Diesel F.P.
8. Use jumper hose to U -tube Diesel from Tubing to IA for Freeze Protect.
Coil Tubing: (Cement Packer Repair / Milling / FCO)
1. Ensure that the 10 -403 Sundry has been approved by the AOGCC.
2. Pump 20 bbls of Diesel F.P. to ensure Tubing and IA are still fluid packed. U -tube F.P.
3. RIH with cement nozzle and entl tag top of bridge plug 6 420 KB. Fla Pi

4. PUH approx. 10', close in the IA, and open Tubing x CT Annulus. Pump the following:
- 2bbls fresh water, foam ball, 4 bbls Cement, foam ball, 2 bbls fresh water
- Displace cement with Seawater for the remaining coil volume.
5. Pump 100' of cement above the nozzle, and lay in cement 1 for 1 into the tubing. Estimate WCTOC
in tubing after all cement has been layed in is @ 5,960' KB, approx. 57' above GLM #7 @ 6,017' KB.
6. PUH laying in Seawater, and park coil tubing nozzle @ approx. 5,660' KB.
7. SI the Tubing x CT annulus, open IA, and slowly open choke to take returns to tanks.
8. Displace 2.7 bbls of cement into the IA. Close in the choke. Shut in the IA. Estimated TOC in IA and
Tubing to be approx. 6,353' KB (100' above packer). Record Pressures (T /I /O).
9. RIH and wash across GLM #7 @ 6,017' KB with Seawater, to clear GLM of residual cement.
10. POOH replacing coil space with SW and 1,900' of diesel F.P. to maintain a balanced wellbore.
11. WOC for 12 hours to ensure the cement is set, then RIH with milling BHA.
12. Mill cement from approx. 6,353' KB down through the composite bridge plug @ 6,420' KB. Push any
bridge plug remains downhole, out the tubing tail, and into 7" casing.
13. POOH with milling BHA, washing tubing to ensure it is free of cement stringers.
14. Perform MIT -IA to 2,000 psi and IA DDT. Record Results.
15. Retool and Perform N2 FCO to remove any fill /debris from across A -sand perfs. RDMO CTU.

1. RIH with RCBL and log from packer @ 6,453' KB to TOC in the IA. TOC est. at approx. 6,353' KB.

Dear Mr. Maunder:
Enclosed please find a spreadsheet with a list of wells from the Kuparuk field (KRU). Each of
these wells was found to have a void in the conductor. These voids were filled with cement if
needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As
per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that
the treatments took place and meets the requirements of form 10-404, Report of Sundry
Operations.
The cement was pumped January 18th and January 24th, 2009. The corrosion inhibitor/sealant
was pumped February 21St through February 23`d , 2009. The attached spreadsheet presents the
well name, top of cement depth prior to filling, and volumes used on each conductor.

Saturday, December 2, 2000: I traveled t° the KuparUk River Field and witnesSed
the semi annual safety valve system testing oni2U Pad.
As the attached AOGCC Safety Valve System Test Report indicates I witnesSed the
testing of 11 wells and' 22 components with three failures observed. The Iow
pressure pilots on wells 2U-5, 2U-14 and 2U-16 tripped well below the required set
point. These three pilots Were adjusted and retested at the time of this test. In
addition to the three failed pilots several other pilots tripped very near the minimum
pressure, these were reset and operated-properly when retested.
The 13.6% failure rate will require a retest of this Pads safety valve SYstem in 90
days rather than the normal ~12'0 day cycle.
Earnie Barndt conducted the testing today he demonstrated.good test ~rocedure
and was a pleasure to work with

NOTE: If the fill level is confirmed above 6894', a CTU cleanout is recommended
before fracturing. The last tag was at 6875' on 1/13/90.
Pull 1.5" HFCV's from mandrels at 6476', 6547' and 6740' and replace with DV's.
Set SV in 'D' nipple at 6775'. Fill annulus with diesel. Pressure test tubing and
annulus. Pull SV. RDMO slickline.
Obtain a minimum of two 24 hour A Sand only well tests.
MIRU fracturing equipment.
Carry out all fluid quality control procedures (will be done by onsite Operations
Engineer). Conduct a sieve analysis on several proppant samples. Obtain samples of
proppant and gelled water with breaker prior to pumping, and from the pad and
slurry stages while pumping. Verify and record proppant weight before and after job
execution.
Install wellhead isolation tool. Pressure test lines. Pressure up 7" x 3 1/2" annulus
to 3500 psi and maintain throughout job.
Pump the 200 barrel pre-pad at 25 BPM. Shut down and determine the ISIP. Stop
adding crosslinker after 140 barrels to avoid leaving crosslinked gel in the tubing.
Pump the fracture treatment at 25 BPM as detailed in the attached pumping schedule.
Expected treating pressures are'
WHTPE (calculated) = 3500 psi
Frac gradient = 0.70 psi/ft (6010' TVD)
Tubing Friction = 210 psi/1000 ft (6796' MD, 25 BPM)
If treating pressure exceeds 5000 psi during the pad, shut down and confer with
Operations Engineering. The maximum wellhead treating pressure is 6000 psi.
Reduce the pump rate in 2 BPM increments to a minimum of 14 BPM to maintain the
treating pressure below 5000 psi in the proppant laden stages. Go to flush if the rate
of treating pressure increase is excessive or if the treating pressure exceeds
5700 psi.
Rig down frac equipment. Leave well shut in overnight.
Proceed with the attached flowback schedule.

MIRU 1.5" CTU, pump truck, acid transport, and N2 unit. Pressure test lines, BOP's,
and pack off.
RIH with coiled tubing to +6700'. Circulate until 1.5" x 3.5" annulus is full of diesel.
Start pumping xylene and continue circulating until the CT is full of xylene. Close
return line and pump the following treatment while reciprocating the CT between
+6450' and +6750'
a) 1000 gallons of xylene
b) 1000 gallons foamed 7.5% HCI (normal additives+foaming agent+2000 scf/bbl N2)
c) 1000 gallons nitrified 7.5% HCI (normal additives+500 scf/bbl N2)
d) 1000 gallons foamed 7.5% HCI (normal additives+foaming agent+2000 scf/bbl N2)
e) 1000 gallons nitrified 7.5% HCI (normal additives+500 scf/bbl N2)
f) 1000 gallons foamed 7.5% HCI (normal additives+foaming agent+2000 scf/bbl N2)
NOTE:
A pressure transducer should be installed to monitor the 1.5" x 3.5" annulus
pressure during all phases of the treatment. Pump the treatment at as high a
rate as possible without exceeding the CT burst pressure limitations or
1500 psi on the 1.5"x 3.5" annulus.
Flush coiled tubing with N2. Open well to flow. N2 lift until approximately 200 barrels
of fluid are recovered. Adjust nitrogen rate to maintain a nitrogen/fluid ratio of
approximately 700 scf/bbl. POOH with coiled tubing.
NOTE:
1) On site Operations Engineer should collect the following fluid samples:
a) Immediately after first spent acid passes sample point.
b) At midpoint of spent acid passing sampling point.
c) After final spent acid passes sampling point.
5. RDMO CTU and acidizing equipment. Allow well to clean up for a minimum of 48 hours·
6. MIRU slickline. Pull standing valve from 'D' nipple at 6827'. RDMO slickline.
7. After well stabilizes, obtain a 24 hour well test.

Enclosed is the approved application for permit to drill the
above referenced well.
If coring is conducted, a one cubic inch chip from each foot of
recovered core is required. Samples of well cuttings and a mud
log are not required. A continuous directional survey is
required as per 20 AAC 25.050(b)(5). If available, a tape
containing the digitized log infor~_ation shall be submitted on
all logs for copying except experimental logs, velocity surveys
and dipmeter surveys.
Many rivers in. Alaska and their drainage systems have been
classified as important for the .spawning or migration of
anadromous fish. Operations in these areas are subject to
AS 16.05.870 and the regulations promulgated thereunder (Title 5,
Alaska Administrative Code). Prior to com~nencing operations you
may be contacted by the Habitat Coordinator's Office, Department
of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
Chapter 3, Article 7 and the regulations promulgated thereunder
(Title 18, Alaska Administrative Code, Chapter 70) and by the
Federal Water Pollution Control Act, as amended. Prior to
commencing operations you may be contacted by a representative of
the Department of Environmental Conservation.
To aid us in scheduling field work, please notify this office 24
hours prior to commencing installation of the blowout prevention

equipment so that a representative of the Commission may be
present to witness testing of the equipment before the surface
casing shoe is drilled. I~here a diverter system is required,
please also notify this office 24 hours prior to commencing

In the event of suspension or abandonment, please give this
office adequate advance notification so that we may have a
witness present.

ARCO Alaska, Inc., proposes to drill a Kuparuk River Field development well to approximately 7217'MD (6229'TVD).
A 20", 2000 psi annular diverter will be installed on the 16" conductor while drilling the surface hole. The
9-5/8" casing will be set through the West Sak Sands at approximate]y 3121'MD (3026'TVD). A 13-5/8", 5000 psi,
RSRRA BOPE stack will be installed on the 9-5/8" surface pipe. It will be tested upon installation and weekly
thereafter. Test pressure will be 3000 psig (1800 psi for annular). Expected maximum surface pressure is 2653
psi. Pressure calculations are based on latest reservoir pressure. Surface pressure is calculated with an
unexpanded gas bubble at surface, including temperature effects. Selected intervals will be logged. The well
will be comp]eted, using the workover rig, with 3-1/2", 9.2#, J-55, EUE 8rd tbg. Non-freezing fluids will be
left in uncemented annuli through the permafrost interva]. Selected intervals wil] be perforated within the
Kuparuk formation and reported on subsequent reports. A downhole safety valve wi]l be (see attached sheet)

installed and tested upon completion of the job. 2U-15 will be the third well drilled on the pad. It is 60'
away from 2U-14 at surface. There are no other close wellbores anywhere downhole. Top of cement for the
production string will be placed at 5617'MD, approximately IO00'MD above the top of the Kuparuk Sands. Reserve
pit fluids will be disposed of down the 9-$/8" x 7" annulus. The 9-5/8" x 7" annulus will be left with a
non-freezing fluid during any interruptions in the disposal process. After the completion rig has moved off
location, the well will be fracture stimulated.
