The corrosion inhibitor is engineered to prevent water from entering the annular space and causing external corrosion that could result in a surface casing leak to atmosphere. The attached spreadsheet represents the well name, top of cement depth prior to filling and volumes of corrosion inhibitor used in each conductor. As per previous agreement with the AOGCC, this letter and spreadsheet serve as notification that the treatments took place and meet the requirements of form 10-404, Report of Sundry Operations. Norman, Chairman ALASKA OIL AND GAS CONSERVATION COMMISSION 333 W. 7th Avenue, Suite 100 Anchorage, AK 99501-3539 RE: SVS Testing at S-19 & Y-33, Corrective Action Plan Dear Commissioner Norman: This letter is a follow up to our September 29,2005 response to your September 13, 2005 request regarding SVS test and production data for the S-19 and Y -33 wells. As noted in our prior letter, our review indicated that production continued on well S-19 after the failed SVS tests. As a result, an incident investigation was conducted to identify the root cause of the event and develop corrective actions to prevent its re- occurrence. The investigation team consisted of a Prudhoe Bay Unit Area Manager, Drill-site Lead, Operations Superintendent, two PACE HSE (Health Safety Environment) representatives, and a Safety engineer. The team was assisted by two well-pad operation leads and the well integrity engineer. Background Incident Description for S-19: On February 14, 2005, routine SSV state tests were being conducted on Well Pad S by an AOGCC Inspector, and a BP WOA Lead Operator. The SSV on Well #19 failed to hold differential pressure across the valve. The grease crew was contacted and the tree was serviced. On February 15, the S-19 SSV was re-tested by a Lead Operator and again failed to hold differential pressure across the valve which constituted a failure. The AOGCC Inspector waived to be present for re-test. A work-order (35309516) was written by the Lead Operator to rebuild the SSV and the well was left on-line. Page 2 The scaffolding was built on S-19 to facilitate SSV maintenance work on February 17, 2005. On February 25 the well was taken off-line, a back pressure valve was installed and the valve in question was rebuilt along with the swab and wing valves. The well was brought back on-line February 26 and the valve was pressure tested by the grease crew to ensure that it held pressure. ) ') On March 26, 2005, S-19 SSV was retested and failed. The BP WOA Lead Operator contacted the grease crew to service the valve then the well was brought back on-line. AOGCC Inspector waived to be present for re-test. Note: the data was incorrectly entered into SVS (Safety Valve System) database as a miscellaneous test instead of a re-test. On March 31 the SSV was re-tested and passed. AOGCC Inspector waived to be present for re-test. Incident Description for Y -33: The well-head shelter on Y -33 had been removed due to rig work-over on the adjacent Y-06 well. On, February 10,2005, routine SSV state tests were being conducted on Well Pad Y by an AOGCC Inspector, and a BP WOA Lead Operator. The SSV on Well #33 failed to close all the way and the well was shut-in. On February 13 the well-head shelter was reinstalled. Heat was applied and the grease crew serviced and tested the SSVon February 14. It appears the problem was ice in the valve. On February 17, I&E reconnected to the well head and the well was brought back on-line. SSV was retested and passed on February 22. AOGCC Inspector waived to be present for re- test. Investigation Findings The investigation team found root causes in the areas of behavior and accountability, management systems, communications, and training and procedures. . Behavior/Accountability (S-19) - The employee was concerned about the potential for freeze problems if the well were to be shut in and felt other priorities were more critical. The employee did not perceive that a minor leak presented a serious safety issue, even though it was the cause of the test failure. The relevant BPXA procedure, State Test Production Wells states" Any SSV that fails a State Test must be repaired and re-tested within 24 hours and the pad manned, or the well must be shut-in until repaired and re-tested" . Management Systems (S-19, Y-33) - Safety Valve Management System failed to identify that there was a problem with continued production following the failure of the state test. Specifically, the SVS Management system did not have the capability to identity or flag wells that are incorrectly on line after failed tests. In this case, the management system did not provide a backup, or redundant process to identity this problem and alert field management. Page 3 · Training & Procedures (Y -33) - There was inadequate guidance on the timeframe required for re-testing a well following shut-in and repair of a SSV that failed a routine test. (S-19) Additionally, the investigation revealed that additional training is required to ensure accurate data entry of state test classifications (miscellaneous, routine, or re-test). ) ) · Communication (S-19) - There was a failure to adequately communicate the status and follow up action required for this event in handover notes between the operation shifts. Corrective actions for these events include: · Coaching of individuals involved in the incident to reinforce the importance of adhering to all SVS requirements. · Reinforce expectations and accountability of all staff through slope-wide communication emphasizing the importance of following procedures and reviewing state SVS test requirements. · Enhance SVS Management reporting systems to identify SVS compliance problems such as wells with failed tests that remain on line. · Evaluate the installation of signs or tags at wells with failed tests, reinforce importance of handover with lead Operators. · Review and modify procedures, and ensure clarity and consistency. Specifically revise procedures to clarify SVS requirements during well shut-in periods and provide follow-up training. The following detailed action plan has been implemented. Expectations and Accountability Due: Nov. 1 . Require all Area Managers to communicate with all Well Pad and Drill Site 30, 2005 operators to clearly reinforce company expectations regarding adherence to SVS policies and procedures Expectations and Accountability Due: Nov. 30, 2. Conduct individual coaching sessions with the Lead Operators involved in 2005 the events to ensure understanding of requirements and reinforce expectations regarding the importance of adhering to procedures. Communications Due: Dee 31, 3. Evaluate the requirement and benefit of utilizing well head signs or tags to 2005 identify wells with failed SVS tests. Modify the report to clearly identify SVS compliance issues including wells with failed tests that remain on production Policies, Procedures Training 6. Review and modify SVS Procedures to clarify requirements regarding SVS testing. As noted in your letter of Aug 8, 2005 some requirements have been identified as confusing. Many of these issues were addressed in the AOGCC/AOGA joint Safety Valve System Taskforce in June 2003 which contains sensible recommendations. We share the Commission's desire to clarify these issues and anticipate active participation in the industry reviews. BPXA has taken this incident seriously and I remain committed to ensure that we conduct our field operations in a safe manner, and in compliance with all applicable regulations and orders. The drilling fluid program will consist of an 8.5-9.5 ppg spud mud in the 17-1/2" hole, a 9.0-9.7 ppg water base mud in the 12-1/4" hole and a 9.3 ppg water base mud in the 8-1/2" hole. Drill 17 1/2" hole to 2700' and cement 13 3/8" L-80 Buttress casing to surface. Install blowout prevention equipment consisting of 5000 psi W.P. pipe rams, blind rams, and annular preventer. Maximum possible bottomhole pressure is 4390 psi at 8800' TVDss based on original resevoir pressure. Maximum possible surface pressure is 3330 psi based on a gas column to surface from 8600' TVDss and an oil column to 8800' TVDss. Directionally drill 12 1/4" hole to the 9-5/8" casing point above the Sadlerochit Sandstone. Run 9 5/8" 47# L-80 mixed string of Buttress and NSCC to within 5' of 12 1/4" TD and cement in two stages: First stage using Class G cement around the 9 5/8" shoe. The second stage to be downsqueezed in the 13 3/8" x 9 5/8" annulus followed bynonfreezing fluid. Directionally drill 8 1/2" hole to TD. Run 7" liner from TD to 250' inside 9 5/8" casing shoe and cement using Class G cement. Clean out to 7" float equipment. Change over to 9.6 ppg. NaCl completion fluid. Test casing and lap to 3000 psi. Run 9 5/8" production packer on internal plastic coated tubing with gas lift mandrels and a sliding sleeve. Set packer 50' above top of liner. A subsurface safety valve with dual control lines will be placed 50' below base of permafrost. Install and test Xmas tree. Displace tubing to diesel. Set and test packer. Suspend well for perforating subsequent to rig release.
