This memo will remain at the from of the subject well ~e.
Our adopted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies in
the treatment of these kinds of applications for permit to ddll. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The APl number and in some instances the well name reflect the number of preexisting
reddlls and or multilaterals in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to ddll.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95.
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the APl numbering methods described in AOGCC staff
memorandum "Multi-lateral (wellbore segment) Ddiling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

As discussed with Mr. Harry Kugler, Shell Western E&P Inc. requests that
Permit No. 84-169, issued September 19, 1984, to drill Middle Ground Shoal
well A-22A-01 be cancelled. This will enable your office to process our
application for a permit to drill well A-41A-11, which will use the same
slot as well A-22-01. Well A-22-01 will be plugged and abandoned as
specified in the Sundry Notice submitted January 14, 1986.

Enclosed is the approved application for permit to drill the
above referenced well.
If coring is conducted, a one cubic inch chip from each foot of
recovered core is required. Samples of well cuttings and a mud
log are not required. A continuous directional survey is
required as per 20 AAC 25.050(b)(5). If available, a tape
containing the digitized log information shall be submitted-on
all logs for copying except experimental logs, velocity surveys
and dipmeter surveys.
Many rivers in Alaska and their drainage systems have been
classified as important for the spawning or migration of
anadromous fish. Operations in these areas are subject to
AS 16.05.870 and the regulations promulgated thereunder (Title 5,
Alaska Administrative Code). Prior to commencing operations you
may be contacted by the Habitat Coordinator's Office, Department
of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
Chapter 3, Article 7 and the regulations promulgated thereunder
(Title 18, Alaska Administrative Code, Chapter 70) and by the
Federal Water Pollution Control Act, as amended. Prior to
commencing operations you may .be contacted by a representative of
the Department of Environmental Conservation.

To aid us in scheduling field work, please notify this office 24
hours prior to commencing installation of the blowout prevention
equipment so that a representative of the Commission may be
present to witness testing of the equipment before the surface
casing shoe is drilled. I.~ere a diverter system is Cequired,
please also notify this office 24 hours prior to commencing
equipment installation so that the Commission may witness testing
before drilling below the shoe of the conductor pipe.
In the event of suspension or abandonment, please give this
office adequate advance notification so that we may have a
witness present.

Attached please find three copies of an Application for Permit to Drill
Well. No. A-22-A1 in the Middle Ground Shoal Field, Cook Inlet, Alaska.
The well will be drilled from Shell's Platform "A". A-22-A1 is a re-drill
of A-22-01 with the existing 13-3/8" and 9-5/8" casing strings to be
re-used on A-22-A1. This is noted on the graphic and rough prognoses
attached. A permit for drilling as deep as 14,100 feet is requested,
although the current prognosis calls for a total depth of only 12,973 feet.
We would appreciate your usual expeditious handling of this application.
The expected spud date is September 25.

ATTACHMENT
The maximum pressure gradient is expected to be .552 psi/ft.
Maximum BHP is expected to be .552 X 10,000 = 5520 psi. Maximum surface
pressure is expected to be 1,920 psi assuming a full column of .360 psi/ft
oil. The well will be drilled in compliance with the Alaskan Oil & Gas
Conservation Commission's regulations issued July, 1981.
As noted in the drawing of the diverter, both 6" mechanical
valves will be kept in an open position until the preventer is closed. The
mechanical valves will be wired open and tagged with instructions
concerning proper procedures for closing a valve in the event that the
preventer is closed.

Plug back existing A-22-01 w/cement retainer at 9450' and 700'
column of cement.
Drill out cement to 9050'. 'Mill window in 9-5/8" casing from 8950'
to 9010'.
Dyna Drill 8-1/2" hole from KOP at 8990' to 9660' turning to the
right from N9°E to N22.4°E and dropping angle from 48.3° to 44.5°
(total of 670' of open hole).
Directionally drill 8-1/2" hole to TD target at 9200' TVD of 7050'N
and 2800'E of surface location (total of 3333' of open hole).
Attempt to run the following wi reline logs'
DIL, BHC, GR, SP/FDC, CNL, GR/HRD.
Run 7" casing from TD to surface and cement back to 7000'
(9930' MD).
