Box 196612 Anchorage, AK 99519-6612 19-t-6~S Dear Mr. Williams: The Alaska Oil and Gas Conservation Commission ("Commission") received well safety valve system (SVS) test reports from BP Exploration (Alaska) Inc. at Prudhoe Bay Unit Lisburne sites as follows: Location Date of SVS Test Failure Rate PBU L1 April 23, 2004 30.0% PBU L2 April 26, 2004 16.7% PBU L3 April 27, 2004 50.0% Copies of the SVS Test Reports are attached. Regulation at 20 AAC 25.265 requires all wells to be equipped with a Commission approved fail-safe automatic surface SVS capable of preventing uncontrolled flow by shutting off flow at the wellhead and shutting down any artificial lift system where an overpressure of equipment may occur. If installed, a subsurface safety valve must be maintained in working order and is subject to performance testing as part of the SVS. Commission policy, as outlined in a letter dated November 14, 1995, states that the testing frequency will be increased when a failure rate exceeds 10 percent. The AOGCC-AOGA Safety Valve System Task Force report issued June 13, 2003, supported retention ofthe 10% failure threshold. Effective the date of SVS test, and until an acceptable level of performance is achieved, the safety valve systems associated with wells located on PBU L1, L2, and L3 must be tested every 90 days. The 12-1/4" intermediate hole was drilled to 14374'MD/8682'TVD. While running 9-5/8" 47# L-80 BTC intermediate casing, the string was stuck at 13545'MD/8207' TVD. Attempts to free the casing were unsuccessful, so the 9-5/8" casing was cemented with 800 sx Class "G" cmt. The 9-5/8" casing was tested, drilled out and the hole cleaned out to the original 12-1/4" TD (14374 MD/8682' TVD). A 7-5/8", 29# L-80 FL45 contingency drilling liner was run from 13223'MD/8022'TVD to 14336'MD/8660'TVD. The liner was cemented with 550 sx of Class "G" cement. The well was drilled to a planned TD of 15311'MD/9278'TVD with a 6-3/4" drilling assembly. A 5-1/2", 17# L-80 FL45 liner will be run to bottom and set with approximately 300' of liner lag into the 7-5/8" casing.
