The wells need to be brought to a mechanically sound condition with plugs in accordance to 20 AAC 25.112. A surface
plug may not be required at this time if the wells are otherwise mechanically sound. However,these wells must be
included in the development of a P&A plan for suspended wells with no future utility that must be approved through
the Commission by 12/31/16.

Victoria,
In reviewing our Not Operable well stock I came across 3 wells that we would like to convert to suspended status. I need
a bit of clarification on what we need to do to change them over.
• 03-05, PTD#176057
o Failed RWO in 1991-Tubing pulled, EZSV with cement cap set in the 9.625" casing. For some reason this
well did not have a PT in 1991 when the rig moved off of it. I assume that if we apply for a suspension
we will need to get a PT.
o No future utility
• G-03A, PTD# 197070
o RWO suspended on 10/25/06 when the rig found holes in the production casing between 5262-5291'. A
EZSV was set at 5096' and a 4.5" kill string was run to 5015'.
o PT passed to 3500 psi on 10/24/06

o No future utility
• Y-08A, PTD# 188056
o This well has a P&A sundry open (314-595), but we are unable to close it out because we have not been
able to get a PT on the cement job that was performed in late 2014. TOC was tagged at 8597', but there
is a tubing leak at 4390' and a production casing leak at 2700'.
o No future utility
Please advise on what is required for each of these to get them moving towards SU status.

Subject: Corrosion Inhibitor Treatments of GPB Y Pad 8
Dear Mr. Maunder, \ I — 3
Enclosed please find multiple copies of a spreadsheet with a list of wells from Y Pad
that were treated with corrosion inhibitor in the surface casing by conductor annulus.
The corrosion inhibitor is engineered to prevent water from entering the annular space
and causing external corrosion that could result in a surface casing leak to atmosphere.
The attached spreadsheet represents the well name, API and PTD numbers, top of
cement depth prior to filling and volumes of corrosion inhibitor used in each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10 -404,
Report of Sundry Operations.

Wednesday, June 15, 1988: I traveled this date from AAI PBU DS7-34
weil'to'S~C PBU~YfsAWell to witness the tagging of a balanced
cement plug set across the 7" x 9 5/8" liner lap. At 1:00 p.m. RIH
with open-ended drill pipe and tagged the plug with 30,000 pounds of
drill string weight at 10,897' (ft). This gave a cement plug of 23'
above the liner lap. POH and prepare to set second plug on top of
first.
Thursday, June 16, 1988: At 1:30 p.m. after waiting on cement for 16
hfS,' RIH and~tagged the second liner lap plug with 20,000 pounds of
drill string weight at 10,833'. This was 74' above the liner lap,
because there was not the required 100' of cement. I contacted
Mike Minder and was given his approval to accept this plug and to let
the operator continue with their operations.
Attached is a copy of the Wellbore schematic for SAPC PBU Y-8A well.
In summary, i witnessed the tagging of the balance cement plug across
the 7, x 9 5/8" liner lap at SA_PC PBU Y-SA well.

PROPOSED REDRILL AND COMPLETION PROGRAM
WELL Y-8A (~2-0;5-10-13} PBU
The drilling fluid program will consist of a 9.2 ppg oil base mud for
drilling the 8-1/2" hole from 9308' MD to 12573' MD.
Install blowout prevention equipment consisting of 5000 psi W.P.
pipe rams, blind rams, and annular preventer. Maximum possible
bottomhole pressure is 4390 psi at 8800' TVDss based on original
reservoir pressure. Maximum possible surface pressure is 3300 psi
based on a gas column to surface from 8600' TVDss and an oil
column to 8800' TVDss.
Kick-off of cement plug at 9308' MD and directional drill 8-1/2"
horizontal hole to TD of 12573' MD. Run a combination 5-1/2", 17#
solid/4-1/2", 12.6# slotted liner from TD to 250' above kick-off point
(9058'MD) and cement solid portion of liner with Class G cement.
Clean out 5-1/2" liner to slotted portion and displace slotted liner
with diesel. Change over to 9.6 ppg NaC1 completion fluid.
Run 9-5/8" production packer on 4-1/2" tubing with a sliding sleeve
and GLM's. Set packer 120' above top of liner. A subsurface safety
valve with dual control lines will be placed 50' below base of
permafrost. Install and test xmas tree. Set and test packer. Displace
tubing to diesel.

Prudhoe Bay Well Y-8A
Close Approach Analysi~
The existing well Y-8 is to be plugged and sidetracked to a new bottom-
hole location. This new bottom-hole location (well) will be called Y-8A.
Well Y-8A will be a horizontal drainhole with a proposed bottom-hole
location close to the old Y-8 location. From the plan view, the new
wellbore (Y-8A) will be displaced approximately 300 feet from the old
wellbore (Y-8) and from the section view, the new wellbore will drop
below and cross beside the old wellbore (see attached directional map).
The closest well other than Y-8 is the PLAGHM State #1 well which is 1600
feet to west of the proposed well Y-8A (see attached circles map).
Based on these facts, there will be no close approaches with other
wellbores in the area. A maximum allowable distance will be maintained
from the old wellbore of Y-8.
