ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation,
injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any
attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the
producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data

Biozan and cimulate bottoms up. Lay in 25 bbl FIo-Pro pill. PUH. IA gas influx to tubing. Unable to tell
if pill is holding. Prep to circulate gas from tubing. RIH, BH SW well on vacuum. Looks like FIo-Pro pill
had no effect. Order more FIo-Pro.

The permit to drill does not exempt you from obtaining additional permits required by law from
other governmental agencies, and does not authorize conducting drilling operations until all other
required permitting determinations are made.
The permit is for a new wellbore segment of existing well P-09, Permit No. 193-058, API 50-
50-029-22362. Production should continue to be reported as a function of the original API
number stated above.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the Noah Slope pager at 659-3607.

P-09L1 Sidetrack

Summary of Operations:
P-09 is a Zone 4 producer that suffers from Iow rate, high water cut. The sidetrack, P-09L1, will exit the 5
1/2" liner in the Shublik interval and target Zone 4 bypassed oil to the west of the P-09 completion. A flow-
through whipstock will be used in order to retain production from the existing P-09 completion. This
sidetrack will be conducted in two phases.
Phase 1: Drift, set whipstock on E-Line. Planned for Nov. 23, 2000. · Wellhead seals will be tested.
· All GLM's will be dummied off, the IA will be loaded and pressure tested.
· A dummy whipstock drift will be run on slickline.
· CTE-Line will set the whipstock at approx. 12,760' MD (8806ss).
Phase 2: Mill window, Drill and Complete sidetrack: Planned for Nov. 25, 2000.
Drilling coil will be used to mill the window then drill and complete the directional hole as per attached
plan. The sidetrack will be completed with a slotted/solid liner. Top of 2 7/8" liner to be approx.
12,400' MD (8513ss).
Mud Program: · Phase 1: Seawater
· Phase 2: Seawater and FIo-Pro (8.6 - 8.7 ppg)
Disposal: · No annular injection on this well.
· All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
· All Class I wastes will go to Pad 3 for disposal.
Casing Program:
· 2 7/8" slotted/solid liner will be run from TD to approx. 12,400' MD (8513ss). Slotted liner will be used
within Zone 4 and solid liner will be used from the top of Zone 4 to the top of the liner at 12,400'.
Well Control: · BOP diagram is attached.
· Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 3500 psi.
· The annular preventer will be tested to 400 psi and 2000 psi.
Directional
· See attached directional plan. BHL (State Plane): 622,039; 5,952,111. Max. hole angle is 98 deg.
· Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging · An MWD Gamma Ray log will be run over all of the open hole section.
· A real time Resistivity Log will be run in open hole.
Hazards
H2S on P-09 was measured at 10 ppm on 8/3/2000. The highest H2S recorded to date on P Pad
is 48 ppm.
Lost circulation risk is Iow. The existing perfs will be sealed with high vis FIo-Pro while drilling the
P-09L1 sidetrack.
Reservoir Pressure
Res. press, is estimated to be 3368 psi at 8800ss. Max. surface pressure with gas (0.12 psi/fi) to surface
is 2312 psi.
