Ramirez, Well Intervention Engineer Q Subject: Sundry Notice 10-403 Approval Request BPXA Well P-15 This application for Sundry approval for BPXA Well P-15 is for the inspection and repair f of surface casing. Currently, part of the surface casing on this well is exposed to atmosphere, most likely due to corrosion. P-15 has been determined to have a surface casing leak at an estimated 4 feet from . surface. BPXA requests approval to inspect and repair the surface casing. With approval, inspection and repair of the surface casing will follow the general, outlined procedure below: 1. Secure the well from the reservoir with plug(s), kill weight fluid and BPV. 2. Remove the cellar, wellhouse, tree, flowline and other equipment necessary for repair. 3. Survey existing utilities and other critical infrastructure, including the wellhead. 4. Drive piles to support the wellhead and well support frame. Drive sheet piling for excavation around well. 5. Install wellhead and well support frame. 6. Excavate inside the parameters of the sheet piling, to an estimated bottom depth of 20 feet. 7. Install nitrogen purge equipment for conductor (and OA if needed) so that hot work may be conducted in a safe manner. 8. Cut windows in the conductor casing and remove cement from the outside of the surface casing for inspection. Remove additional conductor as necessary. Notify AOGCC inspector for opportunity to inspect. 9. Install repair patch needed to return the casing to operational integrity. 10.Patch will be inspected for acceptance. 11.Perform MIT-OA. 12.Install external wrap on casing to safeguard against future, potential corrosion. 13.Backfill and compact around well and install cellar. 14.Demobilize wellhead and well support frame. 15.Reinstall tree, flowline and wellhouse. Remove plug(s), BPV and hand well over to well integrity for evaluation. 16.File 10-404 with AOGCC post repair, complete with pictures demonstrating repair. 1 li t.:;h1VtU STATE OF ALASKA AKA OIL AND GAS CONSERVATION COMMOON JAN 1/2013 REPORT OF SUNDRY WELL OPERATIONS AOGCC 1. December 18. 1994: I was contacted by Jeff Eckstein, BPX P.E., requesting me to witness the No Flow Test on wells P-04, P-12, P - 15, P-16, X-21 and X-25. I met Jeff this morning at his office and we went to P pad where the APC crew was in the final stages of bleeding the wells down.. The crew had began bleeding the wells, tubing and artificial lift gas, down on the afternoon of 12-17-94. When we arrived the American 425 gas measurement meter was hooked up to each well and a reading was takenapproximately every hour for 3 hours with the following results.
