Hi all,
Well 16-09A (PTD #1910380) has been found to have sustained casing pressure on the IA. The wellhead pressures were
T/I/O/OO equal to 1990/1990/vac/vac on 08/08/07. A subsequent TIFL failed that same day. The well has been classified as
Under Evaluation and placed on a 28 day clock. The well may be produced once Flow Station 2 begins operations again.
The plan forward for this well is as follows:
1. D: TIFL - FAILED
2. S: Set DGLVs in STA# 1, 2, & 4
3. D: Re-TIFL

May 1 ~ 2001: I traveled to BP's Prudhoe Bay Field to witness the semi-annual
SVS tests on D5-16.
I witnessed SVS tests on BP's 05-16. Testing went very well. No failures were
witnessed. We had more than adequate manpower to test this Pad in a timely
manner. Flow 2 Supervisor Roy Wells checked in wI the Pad Operators & Leads
ooD5-16 that were performing tests to make sure operations were being
performed in a safe & efficient manner. Preventive maintenance had been ./
performed on SVSfor this Pad two weeks before State testin~ was conducted. -.. ',r, .

HYDRAULIC REFRACTURE TREATMENT OF ZONE 4
7/28/94:
RIH & tagged PBTD at 10479' MD.
7/31/94:
MIRU & PT equipment, then logged an Injection Profile w/680 bbls of 20# Pre-Pad Gel, to
prepare for a Hydraulic Refracture Treatment of Zone 4 with open Sag perforations present.
Spinner log showed about 15% of injection going into the Sag perfs. Made adjustments to the
Fracture Treatment Pad volumes, then PT equipment to 10000 psi. Proceeded pumping the
Refracture Treatment using 20/40 mesh proppant, through the open perforations at:
10289' - 10309' MD (Sag)
10389' - 10415' MD (Z4)
10430' - 10440' MD (Z4)
Ref. Log: DIFL 4/26/91.
Pumped:
a) 330 bbls 30# Crosslinked Pad @ 40 bpm, followed by
b) 425 bbls Crosslinked Slurry w/proppant concentrations @ 2 - 10 ppg, followed by
c) 162 bbls 20# Gelled Flush, followed by
d) Shutdown.
Placed a total of 74,600 lbs of proppant into the formation. Rigged down.
8/1/94:
RIH w/CT & jetted a proppant fill cleanout to 10410' MD.
8/2/94:
RIH & tagged bottom proppant fill at 10382' MD.
Placed well on production w/gas lift & obtained a valid welltest.

DS 16-09A Perforating Operations:
ARCO Alaska, Inc. perforated DS 16-09A at selected intervals in the
Sadlerochit Sand Formation of the Prudhoe Bay Unit on 12/28/93. The
intervals perforated were the following: 10410' - 10415' and 10430' -
10440' MD (Reference Log: AWS-DIFL/GR dated 4/26/91). Produced fluids
were in the wellbore during perforating operations. All wireline work was
done through a 5000 psi WP BOP/Lubricator tested to 1.2 times the
maximum expected surface pressure.
A short summary of the work is given below:
Began perforating operations on 12/28/93. RU perforating unit,
pressure tested BOP's and lubricator, okay. SI well, RIH, opened well
to flow and perforated 10430'-10440' MD at 4 SPF, SI well, POOH,
and confirmed all shots fired. RIH, opened well to flow and
perforated 10410'- 10415' MD at 6 SPF, SI well, POOH, and confirmed
all shots fired. (ref: AWS-DIFL/GR of 4/26/91). Rigged down and
secured well.

POOH w/perforating gun, then performed a Hydraulic Fracture Treatment, using 16/20 mesh
proppant, through the perforations located above the sand plug (listed above).
Pumped 160 bbls Pre Pad @ 40 bpm, followed by Shutdown. ISIP @ 2005 psi.
Resumed pumping 80 bbls 40# Gelled Pad @ 40 bpm, followed by 210 bbls Crosslinked
Slurry, w/proppant in I - 10 ppg stages, followed by 150 bbls Flush, followed by Shutdown.
Placed a total of 40,600 lbs of proppant into the formation. Rigged down.
10/21/92:
Performed a proppant/sand plug fill cleanout to 10635' MD.
10/22/92:

Returned well to production w/gas lift & obtained a valid production test.

pertaining to Approval # 90-519. Verbal approval was granted
6/5/90 to perforate Well 1-30, and the work was reported on a 10-
404 on 6/29/90. The Application was sent 7/2/90. Both reference
the same approval number.

Regarding Approval # 9-692 on Well 16-20, please find enclosed a
10-404 Report Sundry for Bridge Plug Installation and Removal. In
this case, the approved Application of 8/14/89 was for a Choke
Packer Installation that was never performed. Per your request, the
Bridge Plug Installation (11/90) and its removal (3/92) is reported
referring to the 9-692 Approval #.

Regarding Approval # 91-090 on-Well 3-11i, of approval date
3/15/91, requesting approval for an Annular Communication
Evaluation, I have directed your request for a 10-404 Report
submittal to Julie Heusser / James Styler (at 659-5102), who are the
Annular Communications Engineers responsible for filing this
Report.

407 Completion Report is enclosed for the P&A of the old wellbore

1. Rig up a slick line unit. Pull the SSSV from the landing nipple at 2228'.
2. Rig up a CTU. Squeeze off the old production perfs at 10,796 - 10,881 '. Leave at least 100'
of cement above the perfs.
3. Rig up a slickline unit. RIH and tag the TOC to confirm its depth. RD.
4. Rig up an electdc line unit. RIH and cut the 4-1/2" tubing at 10,100'. RD.
Rig Oper'ations
1. MIRU. Pull BPV. Kill the well by circulating seawater through the tubing cut and down the
flowline. Set BPV.
2. ND tree. NU BOPE. Pull BPV and set test plug. Test BOPE. Pull test plug.
3. RU landing joint. Pull tubing hanger free. POOH and LD the tubing string.
4. RIH and set a CIBP at 8200'. Test the CIBP to 3000 psi. Displace the well with drilling
mud.
Operations to be Approved Under Drillinq Pemlit
Rig _O~ons
1. Cut an 80' window in the 9-5/8" casing from 8000' - 8080'. Set a cement plug from the
CIBP at 8200' to above the window. Re-test the BOPE while WOC.
2. Run a steerable assembly and dress off the plug. Kickoff through the window and continue
drilling to the top of the Sadlerochit at 10,455' MD 8976' TVD.
3. Run and cement a 7-5/8" liner in the 8-1/2" hole. Run 300' of liner inside the 9-5/8" casing.
Cement the liner and set the liner top packer. Reverse out the excess cement. Test the
liner to 3000 psi.
4. Drill out of the 7-5/8" casing and run a PIT test on the casing shoe. Drill a 6-3/4" hole to TD
at 10,751' MD 9223' 'I'VD. Condition the hole for logs.
5. Run GR/SP/DIL/LDT/CNL logs in the open hole. Condition the well to run casing.
6. Run and cement a 5-1/2" liner. Run 300' of liner inside the 7-5/8" liner. Drill out the cement
to the top of the 5-1/2" liner. Test the liner lap to 3000 psi. Clean out the 5-1/2" liner to
PBTD. Test the liner to 3000 psi. Displace the well with seawater.
7. Rig up electric line. Run a CBT/CET/GR inside the 5-1/2" liner to evaluate the cement job.
Run a GCT/Gym survey. Run the survey 500' back inside the 9-5/8".
8. Run the 7-5/8" packer with a 3-1/2" tailpipe. Set the packer in the 7-5/8" with the tailpipe
extending into the 5-1/2" liner. Rig down the electric line unit.

Run the 4-1/2" L-80 ,~3mpletion with GLM's and a SSSV. I,, JIM will be run.
10. Space out the tubing. Freeze protect the well with diesel. Land the tubing string.
11. Test the tubing and casing to 3000 psi.
12. Set the BPV. ND the BOPE. NU the tree and test it to 5000 psi. Pull the BPV. Shear out
the SOS.
13. Set the BPV. Release the rig. Pull the BPV.
NOTE:
.
Excess non-hazardous drilling fluids developed from the well operations will be disposed of
by annular injection into the DS 16-23 well. This is in accordance with our ADEC General
Permit No. 8740-DB002. Drill cuttings will be disposed of in the approved lined reserve pit
atDS 16.
2. There are no surrounding wells, other than the existing DS 16-9, within 300' of the proposed
DS 16-9A well bore.
3. The drilling fluid program and surface system will conform to all regulations set forth in
20AAC25.033.
4. The maximum anticipated surface pressure is based on BHP and a full column of gas to
surface (0.1 psi/ft).
5. No potentially hazardous levels of H2S are anticipated in this well.

1500 PSI DOWNSTREAM OF THE REGULATOR.
3. OBSERVE TIME, THEN CLOSE ALL UNITS SIMULTANEOUSLY AND
RECORD THE TIblE AND PRESSURE REMAINING AFTER ALL UNITS
ARE CLOSED WITH CHARGING PUMP OFF.
4. RECORD ON THE IADC REPORT. THE ACCEPTABLE LOWER LIMIT
IS 45 SECONDS CLOSING TIME AND 1200 PSI REMAINING
PRESSURE.
BOP STACK TEST
1. FILL BOP STACK AND MANIFOLD WITH ARCTIC DIESEL IN WINTER OR
SEAWATER DURING SUMMER MONTHS.
2. CHECK THAT ALL HOLD-DOWN SCREWS ARE FULLY RETRACTED.
PREDETERMINE DEPTH THAT TEST PLUG WILL SEAT AND SEAT
IN WELLHEAD. RUN IN LOCK-DOWN SCREWS IN HEAD.
3. CLOSE ANNULAR PREVENTER AND CHOKES AND BYPASS VALVES
ON THE MANIFOLD.
4. TEST ALL COMPONENTS TO 250 PSI AND HOLD FOR 10 MINUTES.
5. INCREASE PRESSURE TO 2500 PSI AND HOLD FOR 10 MINUTES.
BLEED TO ZERO PSI.
6. OPEN ANNULAR PREVENTER AND CLOSE TOP SET OF PIPE RAMS.
7. INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 10 MINUTES.
8. CLOSE VALVES DIRECTLY UPSTREAM OF CHOKES. BLEED DOWNSTREAM
PRESSURE TO ZERO PSI TO TEST VALVES. TEST REMAINING UNTESTED
MANIFOLD VALVES, IF ANY, WITH 5000 PSI UPSTREAM OF VALVE AND
WITH ZERO PSI DOWNSTREAM. BLEED SYSTEM TO ZERO PSI.
9. OPEN PIPE RAMS. BACKOFF RUNNING JT AND PULL OUT OF HOLE.
CLOSE BLIND RAMS AND TEST TO 5000 PSI FOR 10 MINUTES. BLEED
TO ZERO PSI.
10. OPEN BLIND RAMS AND RECOVER TEST PLUG. MAKE SURE MANIFOLD AND
LINES ARE FULL OF ARCTIC DIESEL AND ALL VALVES ARE SET IN THE
DRILLING POSITION.
11. TEST STANDPIPE VALVES TO 5000 PSI.
12. TEST KELLY COCKS AND INSIDE BOP TO 5000 PSI WITH KOOMEY PUMP.
13. RECORD TEST INFORMATION ON BLOWOUT PREVENTER TEST FORM. S IG,~
AND SEND TO DRILLING SUPERVISOR.
14. PERFORM COMPLETE BOP TEST ONCE A WEEK AND FUNCTIONALLY
OPERATE BOPE DAILY FOR DRILLING OPERATIONS. FOR COMPLETION/
WORKOVER OPERATIONS, PERFORM COMPLETE BOP TEST EVERY 21
DAYS AND FUNCTION TEST BOP WEEKLY.
