INSTRUCTIONS
GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska.
ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation,
injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any
attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the
producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data
pertinent to such interval.
ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool.
ITEM 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, Other-explain.
ITEM 28: If no cores taken, indicate 'none'.

Dear Mr. McCarty:
Enclosed is the approved application for permit to drill the above referenced well.
The permit to drill does not exempt you from obtaining additional permits required by law from
other governmental agencies, and does not authorize conducting drilling operations until all other
required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 036.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.
Sincerely,
Julie M. Heusser

Y-30 L1 Sidetrack
Summary of Operations:
A CTD sidetrack of well Y-30 will be drilled. Specific prep work, (Phase 1) and drilling, (Phase 2) operations
are detailed below.
Phase 1: Isolate & integrity test inner annulus, (IA). Drift liner and set whipstock. Mill window if
service coil is available.
Planned for August 10, 2001
· Drift well with dummy whipstock to 9170' SLM.
· Perform Integrity test IA after displacing to seawater & installing dummy gas lift valves.
· Set through tubing whipstock using e-line at approx. 9120' (top of whipstock).
· Mill window with service coil if available.
Phase 2: Drill and Complete CTD sidetrack: Planned for August 17, 2001
2-3/8" coil will be used to drill a ~1600 foot, 3-3A'' open hole sidetrack, (as per attached plan) and run a
slotted, uncemented 3-1/2" x 2-7/8" liner completion.
Mud Program: · Phase 1: Seawater
. Phase 2: Seawater and FIo-Pro (8.6 - 8.7 ppg)
Disposal: · No annular injection on this well.
· All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
· All Class I wastes will go to Pad 3 for disposal.
Casing Program:
· 3-1/2", L-80, solid liner will be run from 8790'md (TOL, 8514' TVDss) to approx. 9200'md
and then crossed over to 2- 7/8",L-80, slotted liner to TD at approx 10,746' md. ~
Well
Control:
· BOP diagram is attached.
· Pipe rams, blind rams and the CT pack off will be pressure tested to 300 psi and to 3500 psi.
· The annular preventer will be tested to 400 psi and 2500 psi.
Directional
· See attached directional plan
· Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging · An MWD Gamma Ray + Resistivity log will be run over the open hole section.
· Memory CNL will be run in the cased hole after reaching TD.
Hazards
H2S was measured at 20 ppm on January 7, 2001. The well-path will cross 0 mapped faults, however
there are two sub-seismic features the well plan will cross that may represent small fractures (<10'). The
geologic risk is Iow, but it does exist. The NW-SE orientation of the sub-seismic features is parallel to the
regional maximum stress; therefore, although these features are small, they may be gapped open and at
least partially conductive.
Reservoir Pressure
Res. press, is estimated to be 3625 psi at 8800ss.
is 2569 psi.
Max. surface pressure with gas (0.12 psi/ft) to surface

~Remarks
Ail depths are Measured Depths (MD) unless otherwise noted.
Baker Hughes INTEQ runs 1 and 2 logged the hole section from 9097' MD
(8807' SSTVD) to 10072' MD (8925' SSTVD) with Ultra Slim Multiple Propagation
Resistivity tool (USPMR).Resistivity service was discontinued due to the
possible lost in hole conditions which existed in runs 3-8. Continued logging
to 10344' MD (8929' SSTVD) utilizing NaviGamma service.
Well Y-30L1 was drilled to TD @ 10351' MD (8928' SSTVD) on Run 8.Bottom
portion of the well was not logged due to Gamma Ray sensor offset.
Well Y-30L1A was kicked off @ 9105' MD (8814' SSTVD) from the
wellbore of Well Y-30.
(5) Data presented here is final. The data has not been shifted to a PDC
(Primary Depth Control) curve. As BPX instructions, the MWD Gamma Ray is the
PDC for this borehole.
(6) A Magnetic Declination correction of 26.52 degrees has been applied
to the Directional Surveys.
