MIRU & pumped 450 Mscf of Nitrogen to displace wellbore fluids to 11600' MD, in preparation
for ROPE treatment. RIH & set a shear disc in the 3-1/2" tubing tail at 11639' MD. RD.

Performed a High Energy ROPE Stimulation on the open perforations (listed above).
MIRU & PT equipment. Pumped 293 Mscf of Nitrogen & ruptured the shear disk at 8290 psi.
Rigged down.

A Cement Plugback to isolate the liner hole for Water Shutoff, was performed as follows:
MIRU CTU & PT equipment to 4000 psi. Loaded the wellbore w/Seawater, then RIH w/CT to tag bottom
& spotted 4 bbls of Latex Acid Resistive cement in the liner from 10132' to 10024' MD. POOH w/CT
spotting biozan across the perforations located above 10024' MD. Left cement below 10024' MD. RD.

MIRU CTU & PT equipment to 4000 psi. Loaded the wellbore w/Seawater & Crude, then RIH w/CT dump

POOH w/CT spotting biozan across the perforations located above 10024' MD. Rigged down.

displacement. Continued pumping 25 bbls of Superfine Cement, followed by 25 bbls Latex Acid
Resistive Cement, to squeeze the perforations (listed above). Attained 3500 psi maximum squeeze
pressure & held for one hour. Placed 2 bbls of cement behind pipe. Contaminated cement w/biozan

MIRU & PT equipment. Pumped the Perf Breakdown/Stimulation Treatment fluids through the open
perforation intervals located above the sand plug (listed above).

a) 100 bbls Slick 1% NH4CI Water @ 9.4 bpm, followed by
b) 50 bbls Xylene preflush @ 4.9 bpm, followed by
c) 60 bbls 12% HCI, followed by
d) 30 bbls 9% HCI - 1% HF @ 5.4 bpm , followed by
e) 100 bbls 1% NH4CI Water @ 0.5 - 5.7 bpm, followed by
f) 60 bbls 12% HCI @ 2.5 bpm, followed by
g) 30 bbls 9% HCI- 1% HF @ 3.9 bpm, followed by
h) 100 bbls 1% NH4CI Water @ 1.2 bpm, followed by
i) 17 bbls Saturated Benzoic Acid Flake Diverter, followed by
j) 10 bbls 1% NH4CI Water spacer, followed by
k) 27 bbls 12% HCI, followed by
I) 12 bbls 9% HCI - 1% HF @ 1.7 bpm, followed by
m) 160 bbls 1% NH4CI w/5% Musol Water Overflush @ 3 bpm, followed by
n) Shutdown.
Rigged down.
Returned the well to production w/lift gas & obtained a valid welltest.

DS 16-04A Perforating Operations'
ARCO Alaska, Inc. perforated DS 16-04A at selected intervals in the
Sadlerochit Formation of the Prudhoe Bay Unit on 3/9/93 and 3/11/93.
The intervals perforated were the following: 11954' - 11974', 11982' -
12000', 12124' - 12136', 12144' - 12150' and 12174' - 12182' MD
(Reference Log: SWS-BHCS dated 2/19/93). Completion fluids were in the
wellbore during perforating operations. All wireline work was done through
a 5000 psi WP BOP/Lubricator tested to 1.2 times the maximum expected
surface pressure.
A short summary of the work is given below:
Began perforating operations on 3/9/93. RU perforating unit,
pressure tested BOP's and lubricator to 3500 psi, okay. RIH and
perforated 12144'-12150' MD at 4 SPF, Logged up and perforated
12124'-12136' MD, POOH, and confirmed all shots fired. RIH and
perforated 12174'-12182' MD at 4 SPF, POOH, and confirmed all
shots fired. (ref: SWS-BHCS of 2/19/93). Rigged down and secured
well.
Continued perforating operations on 3/11/93. RU perforating unit,
pressure tested BOP's and lubricator to 3000 psi, okay. RIH, opened
well to flow and perforated 11982'-12000' MD at 4 SPF, SI well,
POOH, and confirmed all shots fired. RIH, open well to flow and
perforated 11954'-11974' MD at 4 SPF, SI well, POOH, and confirmed
all shots fired. (ref: SWS-BHCS of 2/19/93). Rigged down and
secured well.

Dear Larry,
I have compiled a summary of Anadr/ll's performance on DS 16-4A, Nabors 2ES.
SERVICES PROVIDED
MWD with DWOB and DTOR was picked up on 09-FEB-93 to kick off at 10207' at the
top of the window. Tripped out at 10703' and changed MWD tools on 11-FEB-93 and
continued kick off. Reached 11458' on 13-FEB-93 and pulled out of hole because of hole
packing off and low ROP. Laid down MWD on 13-FEB-93 and rigged down. There were
a total of two standby days and five operating days on this well.
,lOB PERFORMANCE
DS S with downhole weight on bit and downhole torque were used to side track DS 16-4A.
The DWOB was used effectively to assist in the sidetrack process. The 2" MEL
(mechanical efficiency log) indicated that the bit was in new formation at 10230'. The
cement plug was a good hard plug making the kickoff routine.
We began to see usable surveys around 10572' but the magnetic interference continued to
some degree to 10700'. Due to some difficulty repeating a survey and the apparent lack of
left hand turn, (6 degree planned dogleg) the company man decided to pull out of the hole
to check the BHA and change the MWD tool. The Anadrill Engineer informed him that it
was probably magnetic interference and suggested drilling ahead at least 30 feet and
survey. The Company Representative felt that, even if the surveys were accurate we were
still 18 degrees behind the curve and that it would be prudent to check the motor, bit and
change MWD tools to be sure of what the hole direction was really doing. We ran back in
with a new MWD tool, bit and the same motor. We took three check shots on the way in
and they confinned the first tools surveys. Also the DD single shot matched the first tools'
surveys.
On the second run we had to slide over 90% of the time at first to assure that we would hit
the_ target. At 10458' the motor began to stall out while rotating and the hole appeared to be
packing off and we had a pump pressure increase of 300 psi. We pulled out of the hole and
a shank and number one cone were missing. The shank was broken off clean, which
indicated that the cone never locked, this was confirmed with absence of any excess
downhole torque.
Of the 100 surveys taken 10 were used by the directional driller and 37 were good surveys.
The final survey calculation gives the following : SURVEY DEPTH: 11382' TRUE
VERTICAL DEPTH: 8584.91' NORTH: 3368.39' EAST: 4830.36' SECTION: 4577.11'

Sunday, February 21, 1993: The BOP Test on Nabors Rig 2ES began at 6:30 a.m.
and was completed at 9:00 a.m. The one failure was a value in the choke manifold.
This will be repaired and a AOGCC representative will be notified within 4 days.
SUMMARY: I witnessed the weekly BOP Test on Nabors 2ES. Test time 2.5 hours,
I failure.
I was notified February 24, 1993 the value that failed had been repaired and tested.

Wednesday, February 3, 1993: I witnessed a B.O.P. test on Nabors rig 2ES on ARCO drill site 16-4A.
The well is a redrill and the 7" casing was set at 10,100 feet.
The test started at 6:00 p.m. and was completed at 11:30 p.m. (3) valves on the choke manifold
leaked. The valves were greased and retested satisfactorily. The top variable ram failed to hold
pressure and will be changed to a 7" ram and retested with a test plug upon pulling of the tubing. The
well sign and the kill sheet were missing and are to be posted upon making them.
Summary: The B.O.P. test on Nabors 2ES was completed in 5-1/2 hours and there were 4 failures.

Enclosed is the approved application for permit to redrill the above
referenced well.
The permit to redrill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and does not
authorize conducting drilling operations until all other required
permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with
20 AAC 25 035. Sufficient notice (approximately 24 hours) must be given
to allow a representative of the Commission to witness a test of BOPE
installed prior to drilling new hole. Notice may be given by contacting
the Commission petroleum field inspector.on the North Slope pager at
659-3607.

OBJECTIVE:
The objective is to return the well to production by producing a new target 800' northwest
of the present wellbore. A redrill has been requested since a workover is needed to
repair the tubing leaks, and a more desirable location would more effectively drain the
remaining reserves, i The current wellbore has been plagued with high water production
believed to be associated with near we!lbore faulting.
.
Post-sidetrack production rates are estimated at 2000 BOPD at 50% WC and 800 GOR.
The scope of the 16-4A redrill is to plug and abandon the original perforations, pull the
5-1/2" tubing, mill a 60' section in the 9-5/8", lay a cement plug, kickoff and drill a new 8-
1/2" hole to a target 800' northwest of the present wellbore, run open hole logs, run and
cement a 7" 26# 13CR/L-80 liner, and recomplete.with 4-1/2" CR tubing and a 7" CR
packer.
CURRENT STATUS:
Well 16-4 is curreptly shut-in due to high water production.
The 16-4 well was drilled in 1981 with Rowan #35. The well is conventional design with
9-5/8" casing set at 11,845' MD (about 60' above the top of the Sadlerochit) with 10 ppg
mud. The 7" liner was set at TD with 10 ppg mud.
The well has been plagued with unsuccessful attempts to squeeze off water. A hole in
the tubing at 7591' MD was patched 3/90. In 4/90, another leak was detected deeper in
the tubing string. With pressure on the tubing and the tubing x casing annulus, the well
was plugged in 8/92.
PERMITTING:
A sundry notice covering the plugback of ~the original well was approved on 7-17-92
(apprOval #92-190). The attached drilling permit covers the 'proposed drilling program
for the redrillo

Pre-rig work: Kill well. Lay cement plug across perforations to at least 100' above
top perforation. Cut .5-!/2" tubing at 10.500' MD. Freeze protect.
MIRU workoVer rig. Test intermediate packoff to 3500 psi.
Pull BPV. Circulate out freeze protection fluid.
Set two-way check valve. ND tree.
NU 13-5/8" 5M BOP's. Test BOP's to 5000 psi. Pull two-way check valve.
Pull and lay down 5-1/2" tubing.
RIH w/~-5/~" bridge plug and set at 10,460' MD.
RIH with a 9-5/8" section mill and cut a 60' section in the 9-5/8" casing starting at
+/- 10.250" MD.
RIH with open-ended DP. Mix and pump 180 sacks of cement to spot balanced
kick-off plug from 100' below the base of the section to 200' above the top of the
section. If casing appears uncemented, increase cement volume up to 300 sacks.
Replace intermediate packoff if necessary.
RIH with 8-1/2" steerable drilling assembly and dress off cement to 5' below
section. Kickoff well and directionally drill to align well with target.
Drill with a 8-1/2" packed hole assembly to total depth of 12.342' MD / 7880' TVD
or 150' MD below the OWC at 9102' TVD.
RU wireline and log with the DLL / GR / SP / Sonic / Neutron. Make wiper trip
-depending on hole conditions and logging time.
Run 7" 26# 13CR FOX x 7" 26# L80 NSCC standard liner assembly to TD with
liner top packer and +/- 150' lap. Crossover from 13CR to L-80 at 11.530'.
Cement liner with 392 sacks based on 40% open hole washout, shoe volume, lap
volume, and 200' excess on top of liner with DP.
Clean out liner to PBTD and test to 3500 psi. Test intermediate casing packoff to
3500 psi and replace if necessary.
RIH with .4-1/2" 12,6# 13(;;R~0 TDS / 3-1/2" 9.2# 13CR80 FOX completion string w/
Z" CR packer. Circulate corrosion inhibitor down the annulus. Space out, land
completion, and set packer at 11.630' based on: 200' above the top of the
Sadlero(phit'.
Test down tubing to 4000 psi. Test tubing x casing annulus to 3500 psi.
.Set two-way check valve. ND BOP's. NU and test the 4-1/16" 5M tree to 5000 psi
with diesel. Pull two way check valve. Shear RP valve and freeze protect.
Set BPV. Release rig.
