Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped June 30, July 4, and July 20, 2008. The corrosion inhibitor/sealant was pumped June 29, June 30, July10, and July 24, 2008. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Please call MJ Loveland or Perry Klein at 907-659-7043, if you have any questions. Sincerely, ~ ~~ f M.J. Loveland ConocoPhillips Well Integrity Projects Supervisor • ~~ ConocoPhillips Alaska Inc. (PTD 88-§0). This well was previously a producer, In our effort to enhance our production we identified this well as an excellent water injector candidate. We turned this well into injector by replacing all 9 gas-lift valves with dummy gas-lift valves. We then set a plug in the xn nipple on bottom and tested the tubing and the casing, The required MIT test on the casing is as follows: The tubing was test to 3§00#'s for 30 minutes, After 30 minutes the pressure on the tubing was 3490#'s. The casing was then pressure up to 2200#'s for 30 minutes. After 30 minutes the casing pressure 2140#'s, The test was successful and at that time it was turned over to the production operator to be put on injection. The well at this time is currently injecting approximately 600 bbl's a day. If you have any questions regarding this report, please contact me on Platform C at (907) 776-5251, T, A. Selected intervals will be logged. A 20", 2000 psi annular diverter will be installed on the 16" conductor while drilling the surface hole. The 13-5/8", 5000 psi, RSRRA BOPE stack will be installed on the 9-5/8" surface pipe. It will be tested upon installation and weekly thereafter. Test pressure will be 3000 psig (1800 psi for annular). Expected maximum surface pressure is 2711 psi. Pressure calculations are based on virgin reservoir pressure. Surface pressure is calculated with an unexpanded gas bubble at surface, including temperature effects. Casing will be tested to 3000 psi prior to releasing the drilling rig. Non-freezing fluids will be left in uncemented annuli through the permafrost interval. 2V-12 will be 60' away from 2V-11 at the surface. There are no closer wellbores anywhere downhole. Reserve pit fluids will be disposed of by pumping them down the 7" x 9-5/8" annulus of this well. The 7" x 9-5/8" annulus will be left with a non-freezing fluid during any interruptions in the disposal process. (see attached sheet). 23.
