Well 5-21 was replaced by 5-21A (permit # 98-0118, APl # 50-029-21969~6i) ¢/h~'the permit was approved 7/10/98. The replacement well incorporates a deepening operation of the existing well bore. This extension, along with the original hole, will be used for production of hydrocarbons. A review of both well files indicates administrative abandonment of the originating well bore is appropriate according to current AOGCC procedures for wells where additional permitted extension is drilled from an existing well bore. ': · 20 AAC 25.005(e) requires a Permit to Drill for each hole drilled below the conductor unless drilled to straighten the original hole, sidetrack junk, or correct mechanical difficulties. In addition, AOGCC data bases require each conductor has only one permit number and APl number. Administrative abandonment of the original permit only eliminates duplication in the databases. Information for the original well and its replacement will be maintained in their respective files. The operator has been notified of this action. All furore correspondence with the AOGCC should reflect the new permit number, API number and well data. MIRU drilling rig. NU diverter and function test same. 2. Spud well and drill a vertical 17-1/2" hole to 3000'MD/TVD. 3. Set 13-3/8" casing and cement same. Test casing to 2500 psi for 30 minutes. Nipple up BOPs and test to 5000 psi. 4. Pick up 12-1/4" bit and BHA. Drill 10' of new hole and perform leakoff test. Drill vertical 12-1/4" to 7192' TVD / MD (KOP). Build angle at 3.09°/100' at S 30° 39'E to 8488' TVD / 8626' MD (10' TVD above the top of the Sadlerochit). Angle at this point should approximately 40°. 5. Set 9-5/8" casing and cement same. Test casing to 3000 psi for 30 minutes. 6. Drill 10' of new 8-1/2" hole and perform formation integrity test to 12.5 ppg EMW. Switch to oil base mud, then drill 8-1/2" hole continuing to build at 3.09°/100', to 9048' TVD / 10157' MD (EOC). The angle should be 91.5° at the end of the curve. Maintain angle and direction until TD at 9022' TVD, 11157' MD. Note: While drilling, the Sadlerochit will be logged with MWD tools as per program. 7. RIH and spot polymer/salt pill at the 7" liner point at 9048' TVD / 10157' MD. Note: 7" liner point may change due to the depth of the GOC. 8. Run and set the 7" liner with an external casing packer above the 7" shoe. 9. Clean out the liner and pressure test to 3000 psi for 30 minutes. RIH with drill pipe conveyed cement evaluation logs. 10. Clean out the open hole to TD. 11. Run and set the packer and 4-1/2" slotted liner across the open hole. 12. Run 3-1/2" x 4-1/2" tubing with 8 GLMs at appropriate depths. Pressure tubing to 3000 psi. 13. Nipple up tree. Displace the tubing with diesel and the tubing x 9-5/8" annulus with 9.0 ppg NaC1 brine down to the GLM at 9000' TVD. 14. Release drilling rig. PBU DS 5-21 OPERATIONS SUMMARY CONT. NOTE: a.
