On 4/13/97, a State-witnessed safety valve system test was performed on production well P2-12. At that time the well's low pressure pilot (LPP) and surface safety valve (SSV) passed, but the subsurface safety valve (SSSV) failed. Numerous slickline and coiled tubing operations were unsuccessful in re-establishing operation of the 4.5" Camco TRCF-4A tubing-retrievable SSSV. The valve was permanently locked open and work began to install a wireline-retrievable $SSV. However, in the course of that work, the slickline tool, used to shift open ports in the tubing- retrievable $SSV to provide control line pressure to the insert valve, was damaged. A backup tool is not available on the slope, and according to Camco, a replacement will have to be manufactured, requiring approximmely 3 months lead time. Based on the well's successful LPP and SSV test and the fact that the drillsite is continuously manned except for short time periods for crew shift changes, ARCO feels that P2-12 can be safely operated while the replacement wirelJne tool is being manufactured. We will also work closely with Camco to make every feasible effort to expedite delivery of the replacement tool. I witnessed testing of all components with the exception of the blind rams, choke manifold, and choke line valves. These component's had been tested earlier in the day and witnessed by John Spaulding (P.I.) earlier this day. There had been three failures, these being a kill line valve, the gas detectors and a hinge pin seal on the blind rams. All these were repaired and retested with the exception of the gas detectors which would register a Iow alarm but not high gas. The rig had just been activated after a one year stack and showed it, I mentioned this to the toolpusher and was assured that it would look better as he could free some hands up. The area surrounding the rig was fairly well organized and clean. SUMMARY; I witnessed the initial BOP test on EWS rig ~ on Conoco's well # D-2A in the Milne point unit. Three failures observed. I witnessed testing of all components with the exception of the blind rams, choke manifold, and choke line valves. These component's had been tested earlier in the day and witnessed by John Spaulding (P.I.) earlier this day. There had been three failures, these being a kill line valve, the gas detectors and a hinge pin seal on the blind rams. All these were repaired and retested with the exception of the gas detectors which would register a Iow alarm but not high gas. The rig had just been activated after a one year stack and showed it, I mentioned this to the toolpusher and was assured that it would look better as he could free some hands up. The area surrounding the rig was fairly well organized and clean. SUMMARY; I witnessed the initial BOP test on EWS rig #4 on Conoco's well # D-2A in the Milne point unit. Three failures observed. When I arrived on location, the rig electrician was in the process of calibrating the Hydrogen Sulfide and Methane Gas Detection Systems. I witnessed the calibration while the final stages of the BOPE stack nipple-up were completed. As the attached AOGCC BOPE Test Report indicates, there were no failures during this test. There were three new crew members being trained on the drilling crew, this training resulted in a longer than normal test time. IN SUMMARY, witnessed the BOPE Test at AAI's Pt.
