Jim,
An AOGCC witnessed MIT-IA was performed on Injector P2-29 (PTD#1931470) on 06/27/2015. Mr. Bob Noble was the
AOGCC witness on location and deemed the test inconclusive pending further review due to the IA pressure increasing
during the test. We did an extended monitor for 30 minutes total of 60 minutes for the first test. The IA pressure
increased 41 psi in 60 minutes. We bled the IA pressure back down to 2304 psi and re-started the test. The IA pressure
increased 28 psi in 30 minutes.
Enclosed is a temperature and injection graph during the test and the MIT-IA form and also a 90 day TIO plot. The
injection temperature increased —20° during the tests. Currently the well remains on injection and I wanted to get an
assessment if you deem this as a passing MIT-IA.

Subject: Corrosion Inhibitor Treatments of GPMA PM -2
Dear Mr. Regg,
Enclosed please find multiple copies of a spreadsheet with a list of wells from GPMA
PM -2 that were treated with corrosion inhibitor in the surface casing by conductor
annulus. The corrosion inhibitor is engineered to prevent water from entering the
annular space and causing external corrosion that could result in a surface casing leak
to atmosphere. The attached spreadsheet represents the well name, API and PTD
numbers, top of cement depth prior to filling and volumes of corrosion inhibitor used in
each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10 -404,
Report of Sundry Operations.

As per our conversation today, this fax represents notification of T x IA communication for seawater
injector P2-29. Ttlis notification is for records purposes only,
Information to support this notification is as follows:
Well P2-29 was completed in February 1994. On or about Apd11995 the well was diagnosed with a
small T x IA leak. It appears to be a one-way leak down deep; the well will pass an MIT, but will slowly
equalize with the tubing over time. The well has successfully passed MITIA's on 5/I/95, 5/20/95,
5/1/96, and 3/17/'98.
There is no record In the ARCO Annular Communication files of this well being reported verbally to the
State. We are currently operating this well allowing the IA to equalize with the T pressure, and are
performing annual MIT's to monitor the status of the leak, Tubing pressure operates from 1500-2300
psi. Since the leak Is very slow and the annulus will MIT, there is no way to log the well to determine Its
location.
As we discussed, tills do,es not constitute an M_IT fail_ ute and ARCO will continue to operate this well in
the above fashion monitoring the IA with annual MIT's.

Friday, February 4, 1994: I traveled to Doyon rig 14 and witnessed the initial
BOPE test on ARCO's P2- 29 well.
As the attached AOGCC BOPE Test Report shows/there was one failure of the
equipment tested. A stem packing leaked on a valve in the choke manifold,
leake~. It was greased, cycled and retested, but began to leak on another test.
! told Mort Heavirland, Doyon rep., to repair the valve and notify me when it
had been retested.
The H2S gas detectOrs did not operate properly. These are to be repaired and I
will be notified. The methane detectors worked properly.
The location was very clean and the excess equipment stored in the proper
areas. The rig was in fair shape as for as being clean and Mort said he knew
the rig needed to be cleaned up and as soon as drilling began, that would be
taken care of.
Summary: i witnessed the BOPE test on Doyon rig 14 - test time 2.5 hours -
one failure.

Note · I was notified 2 - 6 - 94 by Mort that all the items that were not in
compliance at the test time were now repaired and in good working order.

witness the Diverter System Function Test and to conduct a Diverter System
Inspection.
As the attached Diverter Inspection Form indicates, the Diverter system was in
compliance,
The Diverter Systems had a 30" Annular Preventer with one 16" Hydraulically
Operated Full Opening Knife Valve and one 40' 16" Vent Line facing in a
Westerly direction.
The location was in good condition and had adequate staging area's should the
need arise for additional equipment.
Summary: ! witnessed the Diverter system function test and conducted a
Diverter systems inspection. No failures

Enclosed is the approved application for permit to drill the above
referenced well.
The permit to drill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and does not
authorize conducting drilling operations until all other required
permitting determinations are made.
The blowout prevention equipment (BOPE) must be tested in accordance
with 20 AAC 25.035; and the mechanical integrity (MI) of the injection
wells must be demonstrated under 20 AAC 25.412 and 20 25.030(g)(3).
Sufficient notice (approximately 24 hours) of the MI test before
operation, and of the BOPE test performed before drilling below the
surface casing shoe, must be given so that a representative of the
Commission may witness the tests. Notice may be given by contacting the
Commission petroleum field inspector on the North Slope pager at
659-3607.

Operations Summary
MIRU drilling rig.
Spud well and drill 16" hole. Kick off at 900' MD and build hole angle to 40°,
68° azimuth. Continue drilling to 4097' md / 3700' tvd.
Set 13-3/8" casing and cement same. Test casing to 3000 psi for 30 minutes.
Nipple up BOPs and test same to 5000 psi.
Pick up 12-1/4" BHA and drill 10' of new hole and perform leakoff test. Drill 12-
1/4" hole to TD of 11,303' md / 9200' tvd. Run wireline open hole logs.
Set 9 5/8" L-80 casing and cement same, displacing with 10.5 ppg brine. Test
casing to 4000 psi for 30 minutes. Displace the top 2200'tvd of casing to diesel.
Install a master valve and tree cap. The well will be completed at a later date.
Freeze protect the annulus.
Release rig.
Run cased hole logs: cement bond log.
Note:
This well will be completed after DOYON #14 moves off location.
