GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska.
ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation,
injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any
attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the
producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data

Description of Operations
Arrived to X pad. Spot rig and pits.
Rigging up. Check tree cap for pressure. NIP tree cap. N/U
adapter spool/BOPE/kill and choke line. Rig accepted at 22:00
Hook-up Koomey lines. Pressure up. Function test BOPE.
Install lubricator. Pull BOPE stack over and align it. Measure
stack. Set in cutting box. PT hardline to 4000 psi.
RU for BOP test.
Safety meeting.
PT BOPE. Witness waived by AOGCC's John Crisp.
Stab pipe.
Pull TWC. WHP = 200 psi.
Bring inj to floor. Install CTC. Pull and PT. MU stinger with
3.5" nozzle (BHA #1).
Circulate methanol out of coil.
Open master. RIH, bleeding WHP.
Circulate bottoms up. Got back crude and gas. Lost 20 bls in
1 hour.
POH. Have KCI back on surface. 2.6/2.3.
OOH. M/U BHA #1.
Continue making up BHA.
RIH wlBaker whipstock. Pumping minimum rate.
RIH to 14020'. Increase pump rate to 2.5 bpm. No apparent
losses. Tie in. Not getting GR readings -- pegged out at 126
API units. Trouble shooting on surface. Don't want to cycle
pumps down (to "reboot" MWD), for fear of setting whipstock if
screws are sheared.
POH. Stop at 8600'. GR working again. Re~log to confirm.
RIH. GR stopped working again at 13000'.
POOH
Change out probe and FAIV.
RIH.
Attempt tie in log up at 2.7 BPM at starting at 14020'. GR still
pegged out at 126 units (scale???). Try PUH. Still pegged at
13600', 13300', and 13000'.
RIH. Tag IBP at 14046' down. PUH to full string weight =
14038' up. Correct depth (+5') to 14043'.
PUH to whipstock setting depth 13982' (TOW at 13970'). SD
pump and bleed to attempt "reboot" of MWD. Bring pump back
up and pressured up -~ must have sheared screws while
logging. Pressure to 4000 psi and shear setting tool. Stack
10k down. Whipstock set with top at 13970' and 28R
orientation (no RA tag).

OOH. LD setting tools. P/U milling assembly.
RIH wlmilling assembly.
RIH. Corrected -2' at flag.
Tag XN nipple @13760'. Mill thru (1 stall).
Tag whipstock pinch point @ 13967'. Several stalls getting
started. Then milled 0.6' in 1 hour. 2.6/1600.
Continue time milling to 13968.3' with 2 stalls.
Mill 2.7/1600 FS. Bump in CTP at 1397 4' ~- maybe popped out
here. Mill to 13974.5' and stall.

Try to work BHA past 14714' bit depth. Repeated tags at same
depth ~- again no motor work. Then skipped thru. RIH tag
14733' wlo MW. Work past, then tag a few feet later wlo motor
work. Work to 14750'. Brick wall here wlo motor work.
Continue working to bottom.
Directionally drill f/14799 to 14808 looking good then hit
another brick wall. Wiper trip back up through to 14700' drill
ahead to 14810. Pump Lubetex pill wo effect. Try to put Torq
into constant rotation mode not taking commands. Work area
from 14700 to 14790', took off drilling, put Torq in constant
rotation, drilled to 14837' with good motor work then same
problem, acts like bit is balling up. Continue working coil and
drilled to 14859'.
Short trip to 14000'
Drill f/14859' to 14890'. Last samples reflected 40% soft wht
slst wI clay & pyrite. Mud losses 5 bph
Directionally drill f/14890' to 14935'. Struggling to drill ahead
with clays and pyrite in samples. Pumped remaining
reconditioned mud (160 bbls) into system and seen some
improvement in PR. Making frequent small short trips. This is
only adding to coil life problems. Have ordered out 500 bbls
new mud for 1100 hr delivery.
Making wiper trip to 14100'. Work back to bottom but unable to
drill ahead. With Torq in constant rotation back ream slowly
while waiting on mud

Dear Mr. Sherwood:
Enclosed is the approved application for pennit to drill the above referenced well.
The pennit to drill does not exempt you from obtaining additional pennits required by law from other
governmental agencies, and does not authorize conducting drilling operations until all other required
pennitting detenninations are made.
The pennit is for a new wellbore segment of existing well Prudhoe Bay Unit X-35, Pennit No. 193-
199, API 50-029-22436. Production should continue to be reported as a function of the original API
number stated above.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given by
contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

X-35L 1 Sidetrack - Extension

Summary of Operations:
A sidetrack lateral from production well X-35 is currently being drilled with Nabors 381 Schlumberger Unit4
coil tubing drilling rig. We have now drilled the permitted plan and logging suggest that the oil in our primary
target has been effectively swept. Rather than stopping, we would like to drill onto another banked oil target
some 550' further on. Prep work, (Phase 1) and drilling, (Phase 2) operations are detailed below.
Phase 1: Screen & Prep Well: Screen - Install dummy GlV's, fluid pack IA & MIT. Fluid pack OA & MIT.
Pressure test wellhead seals & tree valves. Completed May 1ih, 02

E-Line set permanent isolation packer with modified check vlv at 14,050'md, (8945'tvd)
Rig-Coil set mechanical thru-tbg, flo-by whipstock at 13,970' md, (-8,883' tvd).

Note: a 10-403 Sundry Permit was not submitted as the existing production intervals are being retained
Phase 2: Drill and Complete CTD sidetrack: Started May 18th, 02.
2%" coil has drilled -1480ft of 3%" OH sidetrack from a window cut in the 5%", 13Cr liner at 13,970'. This is
already 320' beyond planned TO. As per the attached plan the well will be extended a further 260' to
15,713', with the aid of jointed pipe, added to the BHA. This will bring the additional footage above 500ft,
(583' to be exact). The original completion plan remains, that is uncemented 33/16" solid & 2~ slotted liner.
Mud Program:
. Phase 1: Seawater
. Phase 2: Seawater and Flo-Pro (8.6 - 8.7 ppg)
Disposal:
. No annular injection on this well.
. All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
. All Class I wastes will go to Pad 3 for disposal.
Casing Program:
. 33/16" l-80 solid liner run from 13,750'md (TOl, 8,692' tvd) to 100' outside of the window at 14,075' md
. 2~" l-80 solid & slotted liner from 14,075' to TO at approx. 15,710' md - solid joints will be run from
T8AO back to the liner deployment sleeve.
Well Control:
. BOP diagram for Nabors 3S rig 1 Schlumberger CT Unit 4 is attached.
. Pipe rams, blind rams and the CT pack off will be pressure tested to 300 psi and to 3,500 psi.
. The annular preventer will be tested to 400 psi and 2,200 psi.
Directional
. See attached directional plan.
. Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging
. Real time MWO, Gamma Ray will be run in the open hole section. Memory Resistivity may be logged
after reaching TO
Hazards
. X-35 was last tested for H2S on the 3/11/02, and reco.rd~d a 20ppm sample concentration.
. No fault to be crosses prior to TO has a low lost circulation risk.
Reservoir Pressure
Reservoir pressure is estimated to be 3,325 psi at 8,800ss. Maximum surface pressure with gas (0.12
psi/ft) to surface is 2,269 psi.

Summary of Operations:
A sidetrack lateral from production well X-35 will be drilled with Nabors 3S / Schlumberger Unit4 coil tubing
drilling rig. Specific prep work, (Phase 1) and drilling, (Phase 2) operations are detailed below.
Phase 1: Screen & Prep Well: Screen -Install dummy GlV's, fluid pack IA & MIT. Fluid pack OA & MIT.
Pressure test wellhead seals & tree valves. Prep - as detailed below is planned to start April 8th, 02 .,/

Set permanent isolation packer with modified check v/v at 14,050' md, (8945'tvd)
Set a mechanical thru-tbg, flo-by whipstock at 13,975' md, (-8,883' tvd). /

Note: a 10-403 Sundry Permit has not been subitted as the existing production intervals are being retained ~'
Phase 2: Drill and Complete CTD sidetrack: Planned for April 22"d, 02.
2%" coil will be used to drill -1150ft of 3%" OH sidetrack from a window cut in the 5Y2", 13Cr liner at
13,975'- as per the attached plan. The lateral will be completed with an uncemented 33/16" solid & 2Y8 solid
& slotted liner which is designed tol preserve production from the existing perfs, via the PIP with modified
check v/v.
Mud Program:
. Phase 1: Seawater
. Phase 2: Seawater and Flo-Pro (8.6 - 8.7 ppg)
Disposal: /
. No annular injection on this well.
. All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
. All Class I wastes will go to Pad 3 for disposal.
Casing Program:
. 33/16" l-80 solid liner run from 13,750'md (TOl, 8,692' tvd) to 100' outside of the window at 14,075' md ./'
. 2ì8" l-80 solid & slotted liner from 14,075' to TD at approx. 15,130'md - solid joints will be run over
high Sw intervals. ".-
Well Control:
. BOP diagram for Nabors 38 rig / Schlumberger CT Unit 4 is attached. /
. Pipe rams, blind rams and the CT pack off will be pressure tested to 300 psi and to 3,500 psi.
. The annular preventer will be tested to 400 psi and 2,200 psi.
Directional
. See attached directional plan.
. Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging
. Real time MWD, Gamma Ray & Resistivity logs will be run in the open hole section.
Hazards
. X-35 was last tested for H2S on the 3/11/02, and recorded a 20ppm sample concentration.
. No faults are prognosed along the planned wellbore, however there is a risk of loosing circulation to
open Z4B perfs above the whipstock. In this event conventional lCM will be used to manage fluid loss.
Reservoir Pressure
Reservoir pressure is estimated.to be 3,325 psi at 8,800ss. Maximum surface pressure with gas (0.12
psi/ft) to surface is 2,269 psi. ./
