ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation,
injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any
attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the

Arrive to Apad, Rig on stand by at 18:00 hrs. Waiting on DSM
to level the pad.
Wait on Pad to be leveled up.
Rig off stand-by. Spot Nabors 3S over well.
Not able to spot mud tanks next to rig. Angled too much to that
side. Re-spot rig over well.
Spot mud tanks. Continue with R/U. DS CTU #4 on location at
1100 hrs and started their R/U. Notified AOGCC of pending
BOP test. Witnessing wavied by John Spaulding.
Accept rig. Start nipple up BOPE. Double valved IA
BOPE test.
Completed BOPE test,
Pull TWC. Well has 250 psi
Bleed off pressure. Monitor well
P/U injector. Install coil connector. Pull test CTC. M/U milling
BHA
RIH w/BHA #1. Tagged SWN nipple at 11350'.
Crew change. PJSM review plans of drilling window. Discuss
wellbore gas problem.
Circulate well to tiger tank to clear up wellbore gas.
RIH start milling nipple. Free spin 1730 psi at 2.6 bpm. Mill
through nipple. Pick-up run though w/o pump. Looks good.
RIH tag whipstock (top @ 11530'). Pinch point at 11531'.
Correct coil depth -12'. FS 1700 psi @ 2.5 bpm
Time milling window. Top of window 11531' Bottom of window
11536.8' Work window area with string mill.
Drill formation from 11536.8' to 11548' (starting swap to Flo-Pro
at 11548')
Continue to swap hole to Flo-pro ( working window area, looks
good w & wlo pump)
TOH. Flag pipe at 11300'.
OOH. LID milling tools. P/U build assembly.
RIH w/BHA #2. Corrected 12' @EOP
Sent command to TORC. Didn't response. Struggling with
TORC. Got tool faces.
Struggling getting TORC to work. Finally got it. Back on
bottom.
Problem with electronics on pump controls. Unable to
establish communication with orienter. POH to 11517'. Call a
technician. Pump pressure had exceeded default setting of 4k.
Unknown to operator, technican, and DD all that was needed
was to reset "touch" screen reset button. Pump parameters
reset to 4500 psi cut-out.
Orient RIH
Directionally drill f/ 11548' to 11589'. FS 2.4 2770
Shutdown to check suction valve on Nabors pump. Something
still wrong swap to Dowell pump.
Screw with Dowell pump and finally get it right. Orient.
Directional drill fl 11589' to 11598'. Frequent stalls at 11591'.
FS 2.5 2700 psi OB 2850. TF drifting fl 60R to 90R. P/U to
orient, sent command for 60R RIH waiting for TF reading.
Tagged bottom and stalled motor. P/U & reduced pump rate
(normal operation) brought pump backup to 2.5 bpm noticed

and seen slight increase in pump pressure.
10:45 - 13:20 2.58 DRILL N DFAL PROD1 TOH. Found TORO tool radially broken 26.29' below top of
tool at the final drive housing. Fish in hole: bit, motor,UBHO,
3-flex collars, flow sub, XO, 1.33' of the final drive housing for
total of 42.41'. Within the final drive is a 1" drive shaft lossely
held in place that may/may not still be sticking up. If so it would
be +/- 8" up.
Plan is to RIH with LIB to determine if this "shaft" is still in
place.
13:20 - 16:00 2.67 DRILL N DFAL PROD1 Cut 300' of coil while waiting on fishing tools. This is planned
operation,

21 :30 - 23: 15 1.75 DRILL N DFAL PROD1 RIH w/LlB on slickline. wt up 800 #s. Work to pass through
the window. RIH to 11553'. POH
23:15 - 00:00 0.75 DRILL N DFAL PROD1 POH. At 7000' wire broke. Pulling 800#s. No overpull. Made
torsion test on the wire get a clean cut and compare with the
one broke in the hole. Looks corroded. Ask for other slickline
unit to fish wire. The braided line drum on current unit has a
bad spot at 9000' and it's not an option. Not sure which will be
the condition of the remaining drum so call for other unit.
Length of the slick-line fish 32.91' of tools plus 4500' of wire.

long enough to keep .125" slickline above whipstock.

Enclosed is the approved application for pennit to drill the above referenced well.
The pennit to drill does not exempt you from obtaining additional permits required by law from
other governmental agencies, and does not authorize conducting drilling operations until all other
required pennitting determinations are made.
The pennit is for a new wellbore segment of existing well PBU A-38, Pennit No 193-032, API
50-029-22347. Production should continue to be reported as a function of the original API
number. stated above. '. .
Annular disposal has not been requested as part of the Pennit to Drill application for Prudhoe
Bay Unit A-38L.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

A-38L 1 CTD Sidetrack
Summary of Operations: /
A CTD through tubing sidetrack is planned for A-38. The coil sidetrack, A-38L 1, will exit the 5 1/2" liner in
the Sag R. formation and target Zone 4. A-38L 1 will be completed with a slotted liner therefore both the Sag
River and Zone 4 will be open for production from the new sidetrack. The existing A-38 perfs in Zone 4 will
be open to produce through the standing valve and the flow-through whipstock.
One fault splay, 10-15 ft throw, will be crossed. This fault orientation and throw suggest a minimal risk of
lost circulation. A-38L 1 will be completed with a slotted liner to enable continued production from the
existing Zone 4 perfs.
If lost circulation or high water saturation is encountered then a request will be filed with AOGCC to change -*
the sidetrack completion to a cemented liner and simultaneously change the sidetrack name to A-38a. /
Phase 1: Prep work. Planned for April 10-17, 2002.
1. E-line to set standing valve at 11,620'.
2. E-line to set flow-through whipstock at 11,530'.

Phase 2: Mill window, Drill and Complete sidetrack: Planned for April 28, 2002.
Drilling coil will be used to mill the window then drill and complete the directional hole as per attached
plan. Maximum hole angle will be 93 degrees.
Mud Program:
. Phase 1: Seawater
. Phase 2: Mill with seawater/biozan, drill with Flo-Pro (8.6 - 8.7 ppg)
Disposal:
. All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
. All Class I wastes will go to Pad 3 for disposal.
Casing Program:
The production liner will be a 3 Y2" ST -L x 3 3/16" TC II x 2 7/8" solid/slotted liner from TD to approx. ./
11 ,315'MD (8,597'ss). Only the top joint will be 3 Y2". Open hole will be 3.75". Liner will be un-cemented.
Well Control:
. BOP diagram is attached. /'
. Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 3500 psi.
. The annular preventer will be tested to 400 psi and 2500 psi.
Directional
. See attached directional plan. Max. planned hole angle is 93 deg.
. Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging
. MWD Gamma Ray & Resistivity logs will be run over all of the open hole section.

A Pad is an H2S pad. The maximum recorded H2S is 200 ppm on well A-43.
The existing completion, A-38, has been fraced. Losses may occur if the sidetrack encounters
the frac wing.
One small fault splay offers some risk of lost circulation.
Losses or high water saturation may result in abandonment of the sidetrack or a change to a
cemented completion.
