ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. ITEM 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, Other-explain. ITEM 28: If no cores taken, indicate 'none'. Well Control A. The Schrader Bluff in this fault block is expected to be normal pressured at 8.6 ppg EMW. This pressure is estimated based on the initial static on W-200 (Oec-99). ' II. Hydrates and Shallow gas A. Hydrates have not been a significant issue thus far on W-Pad during the recent drilling of W-211 and W-212, however there is always a potential. III. Lost Circulation/Breathing A. Lost circulation is unlikely. B. Breathing will not be an issue as we are not drilling below the Schrader Bluff. IV. Kick Tolerance/Integrity Testing A. The 9-%" casing will be cemented to surface. The top of the 15.8 Ib/gal tail cement slurry is planned to be at -3,292' MD. B. The 7" casing will be cemented with 15.8 Ib/gal tail cement slurry to 500' MD above the Ma Sands -5,668' MD C. The Kick Tolerance for the surface casing is 53.3 bbls with 9.2 ppg mud, 11.6 ppg FIT, and an 8.7ppg pore pressure. D. The Kick Tolerance for the intermediate casing is infinite with 9.2 ppg mud. E. W Pad is not designated as an H2S site, however Standard Operating Procedures for H2S precautions should be followed at all times. Water injection on W-Pad was initiated in late 2001, and H2S levels are expected to remain low until the waterflood matures. Recent H2S data from the pad is as follows: B. Adjacent producers at Surface: a. W -12: less than 8 ppm C. Adjacent producers at TO: a. W-200: 0 ppm O. The maximum recorded level of H2S on the pad was under 20 ppm. VII. Faults A. The planned directional trajectory crosses two faults. These faults have been previously crossed at similar TVO's by W-01 with no problems associated with them. B. MWD RUN 3 IS DIRECTIONAL WITH DUAL GAMMA RAY (DGR) UTILIZING GEIGER MUELLER TUBE DETECTORS, ELECTROMAGNETIC WAVE RESISTIVITY PHASE 4 (EWR4), COMPENSATED NEUTRON POROSITY (CNP), AND STABILIZED LITHO-DENSITY (SLD). 4. MWD RUN 5 IS DIRECTIONAL ONLY FOR SETTING WHIPSTOCK AND MILLING WINDOW AND IS NOT PRESENTED. 5. MWD RUN 6 IS DIRECTIONAL WITH DUAL GAMMA RAY (DGR) UTILIZING GEIGER MUELLER TUBE DETECTORS, ELECTROMAGNETIC WAVE RESISTIVITY PHASE 4 (EWR4), COMPENSATED THERMAL NEUTRON POROSITY (CTN), AND STABILIZED LITHO-DENSITY (SLD). 6. THE GAP IN SROP FROM 7363' MD, 5118' TVD TO 7393' MD, 5127' TVD IS DUE TO THE MILLING RUN. 7. THE GAP IN STABILIZED LITHO-DENSITY (SLD) DATA FROM 7363' MD, 5118' TVD TO 7380' MD, 5123' TVD IS DUE TO THE SENSOR BEING NEXT TO CASING WHILE EXITING THE WHIPSTOCK. 8.
