Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).

and seen slight increase in pump pressure.
10:45 - 13:20 2.58 DRILL N DFAL PROD1 TOH. Found TORO tool radially broken 26.29' below top of
tool at the final drive housing. Fish in hole: bit, motor,UBHO,
3-flex collars, flow sub, XO, 1.33' of the final drive housing for
total of 42.41'. Within the final drive is a 1" drive shaft lossely
held in place that maylmay not still be sticking up. If so it would
be +/- 8" up.
Plan is to RIH with LIB to determine if this "shaft" is still in
place.

torsion test on the wire get a clean cut and compare with the
one broke in the hole. Looks corroded. Ask for other slickline
unit to fish wire. The braided line drum on current unit has a
bad spot at 9000' and it's not an option. Not sure which will be
the condition of the remaining drum so call for other unit.
Length of the slick-line fish 32.91' of tools plus 4500' of wire.

Enclosed is the approved application for permit to redrill the above development well.
This permit to redrill does not exempt you from obtaining additional permits or approvals
required by law from other governmental agencies, and does not authorize conducting drilling
operations until all other required permits and approvals have been issued. In addition, the
Commission reserves the right to withdraw the permit in the event it was erroneously issued.
The permit is for a new wellbore segment of existing well PBU A-38, Permit No 193-032, API
50-029-22347. Production should continue to be reported as a function of the original API
number stated above.
Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter 25 of the
Alaska Administrative Code unless the Commission specifically authorizes a variance. Failure
to comply with an applicable provision of AS 31.05, Title 20, Chapter 25 of the Alaska
Administrative Code, or a Commission order, or the terms and conditions of this permit may
result in the revocation or suspension of the permit. Please provide at least twenty-four (24)
hours notice for a representative of the Commission to witness any required test. Contact the
Commission's North Slope petroleum field inspector at 659-3607 (pager).

Summary of Operations:
Prudhoe Bay well A-38 is scheduled to have an additional horizontal lateral added utilizing coiled tubing
drilling. Prep work will be limited to installing dummy gas lift valves and demonstrating mechanical integrity
of wellbore. Sidetracking will occur off a mechanical whipstock, out of 3-3/16" liner at 11600' MD, in the
Shublik interval. The target for the lateral is upper Zone 4.
Approximately 1550' of 3" bi-center open hole is planned to access the target. The well will be completed /
with an uncemented, pre-drilled liner. Production from the existing wellbores will be past the whipstock.
Pre-rig Prep work. Planned for mid November, 2003 startup.
1. Install dummy GL V s. Drift tubing/liner with dummy whipstock to KOP.
2. MIT tubing/inner annulus to 3500 psi
Nordic 2 CTD - Mill window, Drill and Complete sidetrack: Planned for December 07, 2003 startup.
1. RU Nordic 2, with 2" e-line coil and 7-1/16" BOP stack.
2. Install mechanical whipstock inside 3-3/16" liner for an 11600' MD KOP.
3. Mill 2.74" window from whipstock through the 3-3/16" liner /
4. Directionally drill -1550' to 13150' MD of 3" bi-center hole to access Zone 4 reserves.
5. Run 2-3/8" pre-drilled liner from TD up to 11500' md, approximately 3 joints above window.
6. RD Nordic 2. Install gas lift. Place well on production.

Disposal:
· All drilling and completion fluids and all other Class II wastes will go to Grind & Inject or Pad 3.

Casing Program:
· 1650' of 2-3/8" STL 4.6# L-80 pre-drilled liner from TD up into the 3-3/16" liner to 11,500' md. /
Well Control:
· BOP diagram is attached. /
· Pipe rams, blind rams and the CT pack-off will be pressure tested to 400 psi and to 3500 psi.
· The annular preventer will be tested to 400 psi and 2500 psi.
Directional
· See attached directional plan, 1550' planned horizontal. Max. planned hole angle is approx. -92 deg.
· Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging
· An MWD Gamma Ray will be run over the entire open hole section.
· MAD pass openhole Resistivity for determining water saturations.
Hazards
· The most recent H2S test on A-38Ll was 30 ppm. Prudhoe A Pad is a designated H2S pad. ./
· One mapped fault will be crossed. Lost circulation risk is considered low based on offsets.
Reservoir Pressure
· Pore pressure is estimated to be 3477 psi at 8800'TVDss (7.6 ppg emw).
· Maximum surface pressure with gas (0.1 psi/ft) to surface is 2600 psi ./

branches are proposed from a single conductor, surface, or production casing, the applicant shall
designate one branch as the primary wellbore and include all information required under (c) of this section
