Subject: Corrosion Inhibitor Treatments of GPMA PM -2
Dear Mr. Regg,
Enclosed please find multiple copies of a spreadsheet with a list of wells from GPMA
PM -2 that were treated with corrosion inhibitor in the surface casing by conductor
annulus. The corrosion inhibitor is engineered to prevent water from entering the
g p g
annular space and causing external corrosion that could result in a surface casing leak
to atmosphere. The attached spreadsheet represents the well name, API and PTD
numbers, top of cement depth prior to filling and volumes of corrosion inhibitor used in
each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10 -404,
Report of Sundry Operations.
If you require any additional information, please contact me or my alternate, Gerald
Murphy, at 659 -5102.
Sincerely,
Mehreen Vazir

INSTRUCTIONS
GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and
leases in Alaska.
ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water
supply for injection, observation, injection for in-situ combustion.
ITEM §: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces
on this form and in any attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item
16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional
interval to be separately produced, showing the data pertinent to such interval.
ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of
the cementing tool.
ITEM 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In,
Other-explain.
iTEM 28: If no cores taken, indicate 'none'.

Dear Mr. Schultz:
Enclosed is the approved application for permit to drill the above referenced well.
The permit to drill does not exempt you from obtaining additional permits required by law from
other governmental agencies, and does not authorize conducting drilling operations until all other
required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25.035.
Sufficient notice (approximately 24 hours) of the BOPE test performed before drilling below the
surface casing shoe must be given so that a representative of the Commission may witness the test.
Notice may be given by contacting the Commission petroleum field inspector on the North Slope
pager at 659-3607.

Abnormal differential reservoir pressure should not be encountered in this well. The BHL is offset
by two producers and one injector: 1800' SE of P1-05; 1200' W of P2-54 and 2500' SSW of P2-
42i. Production/injection logs have not been run in offsets. The Terrace Fault is located 400' S of
the proposed top Kuparuk formation. Original reservoir pressure is 4350 psia at 8852' TVD. The
most likely reservoir pressure is estimated at 4150 psi (9.0 ppg), but could range from 4050 to
4250 psi at 8852' TVD.
Two close approach well has been identified which will be shut in while drilling the close approach
zone (see above).
Well Control:
Well control equipment consisting of 5000 psi W.P. pipe rams, blind rams, and annular preventer
will be installed and is capable of handling maximum potential surface pressures.
Diverter, BOPE and mud fluid system schematics on file with AOGCC.

Operations Summary
1. MIRU drilling rig. NU and function test diverter.
,
Spud well and drill 12 1/4" hole to 300' md; begin eastward nudge per attached directional
plan. Drill to 325 l'---~ck off and build angle to 44.2° inclination at 137.58° azimuth.
Hold angle to 5303' MD surface hole TD.
,
Run and cement 9 5/8" casing; perform top job. Test casing to 3000 psi for 10 minutes.
Nipple up BOP's and test same to 5000 psi. Note AOGCC witness or waiver on the
morning report.
4. PFU 8 1/2" BHA, drill 10' new hole and perform LOT.
.
Drill ahead and hold 44.2° to 9963'; build angle to 47.13° at 137.5° azimuth. Hold angle
and azimuth to top of HRZ at 10461'. Perform open-hole change over to 3% KC1 polymer
mud at 10.2 ppg. Drill to 11037' TD, holding angle and azimuth. Obtain LWD logs
through Kupamk and MAD pass on trip out of hole.
,
POH and stand back drill pipe. Run 4 1/2" NT-13Cr-80 liner and cement per attached
cement program~displace with seawater. Bump plug and set liner top packer. Release
and displace 9 5/8" casing to filtered seawater.
,
Run dummy seal assembly and test the casing to 4000 psi for 30 minutes. Run 4 1/2" NT-
13Cr-80 tie-back string/completion assembly. Sting liner tieback seals into liner top. Test
tubing and annulus to 4000 psi.
,
Shear RP valve in top GLM. Displace tubing to diesel, U-tube to freeze protect the
annulus.
9. ND BOP and NU tree assembly. Test tree assembly to 5000 psi.
10.
Secure well, release rig.
POST RIG WORK
1. Replace RP valve with dummy or glv as required. Perforate and flow test well.
