GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Altemating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
ITEM 20: True vertical thickness.
ITEM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the
cementing tool.
ITEM 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in
item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional

Summary of Operations
Well W -05 is scheduled for a rig work over with Doyon 16 followed by a through tubing
sidetrack with coiled tubing drilling rig Nordic 1. The work over will recover the 3-1/2" I
L80 tubing and packer, plug and abandon the existing perforations with class G cement .3/1 (þ v4
and run a 4-1/2" 13Cr-80 tubing string and packer. The sidetrack, W -05A, will kick off
from a window milled off a mechanical whipstock set above the abandoned perforations.
Upper Zone 4 sands will be the target for this sidetrack. Initial plan is to run a pre drilled
liner but if water is encountered a solid cemented liner may be set instead.
Pre Rig Prep Work
i) Complete mechanical integrity test of inner and outer annulus.
ii) Pressure test wellhead and tubing packoffs
Nordic 1 CTD - Set whipstock, mill window, drill and complete sidetrack:
iii) Run whipstock through 4-112" tubing into the 5-112" liner and set at 14210' /J
MD.
iv) Mill 3.8" window at 14210' MD, through the 5-112" liner. ,
v) Directionally drill 1565' of 3-3/4" to 4-1/8" hole to a planned TD of 15775' ~

No faults are expected in this well though one nearby well suffered severe losses.
