The complete 403 approved packages are attached to this e-mail for reference. Our rationale is as follows: 1. The MITOA typically will not pass conventional pressure test criteria (30 minute test with 10% pressure loss) due to the extremely small volume we are trying to test in a fully cemented OA. The 30 minute test criteria was designed for a large volume (un-cemented,fluid packed tubing or casing and associated annuli). 2. Due to point#1, BP does not consider a passing"MITOA"to be the appropriate measure of success or failure of the repair. 3. BP proposes that the indicator for job success will be bringing the well online under evaluation and monitoring the conductor for any leak,weep, etc. as well as monitoring any thermal pressure buildup in the small void space in the OA daily. 4. In Sept'96, CTD drilled out the toe of the slotted liner and crosscut the Zone 4 section. The drill out was completed with 2-7/8" slotted liner. S-40A started having problems in 2002 when the Pad Operator noticed diesel coming up the conductor pipe. The failing MIT-T and IA tracking indicates communication from the tubing to the IA. This will be resolved with the new tubing completion. The failing MIT-OA to N2 confirmed the SC leak and will be resolved by cementing the 7-5/8" production casing to surface. Scope of Work: The primary objective of this RWO is to restore well integrity and return the well to / operable status. The tubing leaks were logged with a leak detect log on 3/3/15 and found at 7780' and 8439' MD.The production casing is good as the well passes a CMIT-TxIA to 3000 psi. So the pressure is on the OA and we will repair that by cutting and pulling the 9-5/8" casing and cementing new casing in place to surface. . Dry tag td at 11440' ctm, pulled 5k over. Pump 206 bbls 15/10 HCL/Xylene as per program reciprocating coil over entire slotted liner interval, good acid response, were able to get down to 11575' ctm without any sticking or over pulls. Sat down one time at 10449' ctm, picked up and went right down hole without any problems. Made one quick pass at tail pipe jetting acid to clean any scale build up. Pumped 63 bbls FSW while pulling out of hole to keep well bore full. Rig down and turn well over to Trico for flowback and acid neutralization. Fluids Pumped BBLS. Came loose w/ 31k overpull at fish. POOH. Recovered knuckle jr. And 2 weight bars. RU wireline,run in hole w/wire grab. Recovered 15' of wire. Continue run wire grab-no recovery. Run in hole w/lib. 9/2/96 3 11366 Run in hole w/lib on wireline. Picture of jar fn. Ran 3.8"od wire finder. Trouble getting past tight spot @ 4602 which appeared after first ball of wire was pulled up. Probably piece of wire stuck in buttress collar there. Worked finder over fish. Appeared to get a few bites. Sheared GR @ 4602 on way out. Picked up GS and stabbed back in. Pulled out finder-no recovery. Picked up 3.8" by 74" extended wire finder. Could not get finder past tight spot @ 4602. Pulled back out. Picked up 3.8" broach and cleaned up tight spot with little effort. Picked up long wire finder again and worked same over fish. No bites. Ran finder w/ one arm off with decentralizer. No bites. Tool sheared in SSSV nipple on way out. RIH and retrieve same. Ran recessed lib to prove we can get over jars and spangs. Got good impression of top of oil jars. TIH with washover shoe. Washover fisfl, appearred to be cutting to 9411 CTD. Possibly top of GS spear. POOH. 913196 4 11366 Pulled out washover shoe. Good wear. Wire marks on outside of barrel. Picked up 1-3/4" overshot. Tried to plant same on fish. Pulled out w/overshot. No marks on grapple. Picked up 3.55" bell guide and 2.6" lib. Ran in and took impression. Lib shows broken end of WL jars- polished rod could be extended on fish. Added extension to overshot and re-ran. Had no problem planting overshot. Picked up GS and latched fish. Try to jar free without success. Sheared off and beat back down-no movement down either. Pulled out. Picked up Baker hydraulic GS. RIH and latch fn on first attempt. Dead pull W/0 jars to 10K over, no movement. Began jarring w/ 10K overpull at'fish. Came free while pulling for 7th jar lick. Pulled out to check tools. Grapple had slipped off fish. All teeth on grapple rolled. Grapple had probably gone past oil jars and was jarring on shooulder. Re-dress overshot w/1.75" grapple and shorter extension. TIH. Work over fish and jar to 78,400#. Came free, POOH. Recovered oil jars.sucker rod pin had parted in top of spang jars. Re-dress overshot w/1.75" grapple and change out extensions to space out over spang jars. TIH. 9/4/96 $ 11366 Tried to latch fish w/ 1-3/4" overshot without success. Pulled out. Rig up wireline, run in w/overshot w/1.875" grapple. Planted but pulled off. POOH. Run wire grab, could not get past 4600'. Run in and plant overshot w/1.75" grapple. TIH w/hydraulic release GS on CT. Latch into fish and jar. Came free at 80,000#. POOH, recovered spang jars and quick connect. RU wireline, run wire grab. Left wire grab in hole. Retrieve wire grab continue making attempt to clear out wire 9/$/96 6 11366 Ran lib, POOH, found clean 1" sucker rod pin indentation in lib, no wire marks, R/D wireline, R/U lubricator and washover pipe / motor bha. RIH with CT. Mill with washover shoe for 3 hrs on fish at 9427 ft. POOH. Shoe worn 1/4" on btm face, ID of shoe showed that fishing neck was swallowed completely. No wireline recovery. R/U wireline unit. Run Kinley power jar. Set off charge, no indication of movement. Run in w/blind box and jar down-no movement. Run Kinley power jar #2. Set off charge, no indication of movement. Run in hole w/blind box and jar down-no movement. Run in hole w/pulling tool, attempt to latch fish, unable to latch. Rig back wireline. LD CT tools. Cut ___75' off CT. Re-head CT. gl6/96 7 11366 Fish 9/7/96 8 11366 Run in hole w/down jars and swage to jar down flow tube. Jar down and move fish down hole to 9468' (42'movement). RU wire line and run wire grab, no recovery. Run lib, indicated wire around outside of ~ishing neck. TIH w/hydraulic t, elease GS spear and jars. Latch fish and begin to work uphole. Pulling to 25,000#over pipe weight and ymppping free. Work up to 9404', quit moving up or down. Begin jarring. Hit 25 jar cycles-no movement. Got indication of wire on 1st run-overpull to 2000#. No recovery. Continue attempt to recover loose wire. Work down to 9920' WLM-hole angle to great to get deeper. tl9/96 1 0 11366 P,/D wireline unit, RIH with 3 arm wire grab on CT to 10,199 ft. Pick up with overpull, RIH and set down at same depth again, POOH with 5k overpull to surface, no wire on grab. Modify tool and RIH again. Set ·own at 10,191 ft, six times, no progress. Possible ID problem in perfed / slotted liner. POOH with wire ~rab. No recovery. TIH w/motor and mill. Mill thru obsructions in liner, very touchy milling at 10,190' and ~0,816'. Clean out to pack off bushing. Aluminum insert in bushing seems to be blanking off mill cutting ~rface. POOH ~110/96 I I 11439 FOOH with mill, layout milling BHA, M/U directional drilling BHA. Pull past liner shoe, also ~lled tight with bit at liner, POOH chasing trash, numerous overpulls, until we left the trash in the 7 5/8" ~sing interval between the tbg and liner. POOH with no more overpulls. Check BHA-no marks or damage. ~ BHA #10 (reverse circ junk basket). RIH to liner shoe and clean up junk. Recovered a few steel chips ·nd one 1" piece of WL. RIH with BHA #11 (mill and string reamer). Had a few tight spots in tubing. Ream ~iher from top to 11273' where wt loss occurred. Mill to 11274'. all 3/9 6 I 4 I 1 465 ·ill 4.5" liner with 3.80 mill and string reamer, mill out liner shoe and work shoe area. POOH. Layout ~illing BHA. Found burst disc on Baker circ sub had failed causing weak motor run. M/U drilling BHA with .t~0 deg bent motor. RIH. Test Anadrill- tool failed, POOH. Change pulsar. Test Anadrill 3 times while ~H. Tag TD to verify depth (CTU counters have been slipping). PUH to GR tie-in point (with original ~lling log) @ 10980'. Unable to tie-in because GR was showing max counts. RIH and verified GR worked in non slotted liner and open hole. PUH to solid liner at 10868' and Icg GR to 11000' with nothing definitive. The extension drops the well down throughout Zone 4 and provides offtake for conformance. Drill Directional Hole: Planned for July 15-20, 1996. - Directional drilling coil tubing equipment will be rigged up and utilized to ddll the directional hole as per attached plan to planned TD of 11,669' MD (8,766' TVD ss). Mud Program: - FIo-Pro (8.8- 8.9 ppg) Casing Program - 2 7/8", 6.16#, L-80, FL-4S Slotted liner will be run from TD to approx. 11,340' MD.
