Enclosed please find multiple copies of a spreadsheet with a list of wells from GPB DS
14 that were treated with corrosion inhibitor in the surface casing by conductor annulus.
The corrosion inhibitor is engineered to prevent water from entering the annular space
and causing external corrosion that could result in a surface casing leak to atmosphere.
The attached spreadsheet represents the well name, API and PTD numbers, top of
cement depth prior to filling and volumes of corrosion inhibitor used in each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10-404,
Report of Sundry Operations.

All,
Well 14-03A (PTD #1961290) has been found to have sustained casing pressure on the IA. The well broke
MOASP on 07/17/09 with TIO pressures of SSV/2020/850 psi. A TIFL was conducted on 07/19/09 that failed due
to overnight pressure build up. The well has been reclassified as Under Evaluation and has been placed under
the 28 day clock. The following corrective action has been put in place.
1. D: TIFL - FAILED
2. S: Set DGLVs in STA # 1& 2, caliper
3. F: MIT-IA to 3000 psi, LLR if fails

repressurization. Drillsite 14 Operator reported the IA of 14-03 to have sustained 1A pressure exceeding MAASP. A
subsequent TIFL passed on 12/14/07. The DSO reported the IA to repressure above MAASP. A second TIFL was
performed on 12/28/07 which failed due to IA repressurization of 110 psi in 24 hours. The well has been classified as
Under Evaluation and may be produced under a 28-day clock.
The Welt Integrity plan forward is as follows:
1. D: TIFL -FAILED
2. S: DGLV's
3. F: MITIA to 3000 psi

cement to set the liner at 10340 - 12263' MD. Left estimated TOC behind liner at 10800' MD.
Released from TOL & POOH w/CT displacing well from mud to 1% KCI Water. RIH w/stinger &
jetting nozzle. Displaced liner to 2% KCI Water. POOH & WOC.
8/1 4/96:
WOC. MU & RIH with 2-1/8" perforation guns & shot 194' of initial perforations at:
11870 - 11900' MD (Z2) at4spf.
12030 - 12194' MD (Z2) at4 spf.
POOH w/perforation guns. Set BPV, RD CTU, ND stack, NU & tested tree.
Placed the well on production w/lift gas & obtained a valid welltest.

The permit to redrill does not exempt you from obtaining additional permits required by
law from other governmental agencies, and does not authorize conducting drilling
operations until all other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with
20 AAC 25 035. Sufficient notice (approximately 24 hours) must be given to allow a
representative of the Commission to witness a test of BOPE installed prior to drilling new
hole. Notice may be given by contacting the Commission petroleum field inspector on the
North Slope pager at 659-3607.

Objective:
Directionally drill ~977' sidetrack well from the existing
wellbore. Complete with 2-7/8" cemented liner.
Intended Procedure:
· P&A existing interval.
Mill window in 7" casing at ~11,080' md. Build to high
angle (85°) at ~20°/100'.
· Directionally drill at an 85° tangent section for ~ 690'.
· Run and cement 2-7/8" liner to TD.
· Perforate selected intervals.

Potential high perm intervals through the Victor
conglomerates (KOP to ~8860' ss). This section warrants
consideration for lost circulation or high rate gas production
during underbalanced operations to address differential
sticking concerns.
