The corrosion inhibitor is engineered to prevent water from entering the annular space and causing external corrosion that could result in a surface casing leak to atmosphere. The attached spreadsheet represents the well name, API and PTD numbers, top of cement depth prior to filling and volumes of corrosion inhibitor used in each conductor. As per previous agreement with the AOGCC, this letter and spreadsheet serve as notification that the treatments took place and meet the requirements of form 10 -404, Report of Sundry Operations. As the attached AOGCC BOPE Test Report shows there were no failures of the equipment tested. The lower pipe rams could not be tested at this time due to their close proximity to the tubing hanger and the kelly had not been picked up. When the existing tubing has been laid down, both kelly valves and the lower pipe rams will be tested. This rig is very clean and the equipment appeared to be well maintained. The surrounding location was well organized allowing good access to all area's of the rig. Summary: I witnessed the BOPE test on Doyon Rig 16 - test time 2.5 hours - 0 failures. Test 9-5/8" casing to 3,500 psi for 10 minutes. RIH and set EZSV at+_8,850' MD (250' below proposed 9-5/8" section). PU section mill, TIH and spot +15 bbl (+205') of 18.0 ppg milling fluid from EZSV (_+8,850' MD) to 8,645' MD. Space out and mill section from 8,595' MD to 8,645' MD. POOH. TIH with open-ended drillpipe. Mix and spot a 17.0 ppg cement kick-off plug (per recommended practices) from 8,395' to 8,745' MD. POOH a minimum of 12 stands. Perform a hesitation squeeze. Do not exceed a final squeeze pressure of 2,000 PSI and do not displace cement any closer than 100' above section top. NOTE: Final cement volume should insure slurry placement 100' below milled section, 50' across milled section and 200' inside casing above milled section. PU directional BHA and TIH. Tag TOC. MINIMUM WOC TIME IS 12 HOURS. Dress off cement plug to 8,600' MD (5' below top of section), kick-off and directionally drill 8-1/2" hole to _+10,305' MD (+88°). POOH. Run and cement 7" (L-80 x 13 Cr) liner from +10,305' MD and tie back into the 9-5/8" casing at _+8,450' MD. POOH and LD liner running tools. Test tubing-casing annulus to 3,500 psi. 13. Set two way check and test to 1,000 psi. 14. ND BOPE. NU tree and test to 5,000 psi. 15. Freeze protect well and release rig. Drilling Hazards DS 11 is an H2S pad. Although the maximum documented H2S concentration in the last year on the DS 11-5 well is 15 ppm, appropriate H2S precautions are to be observed at all times while the drilling rig is on location. These precautions include, but are not limited to: Proper training of all onsite personnel, unescorted visitors and temporary onsite personnel; Monitors with both audible and visual alarms; Access to 5-minute emergency escape air packs; Personal H=S monitors; Working wind indicator; Posted evacuation routes (minimum of two); Muster points in designated safe areas; KOP will occur below the top of the K-lO. MW should be at minimum 9.8 ppg to maintain hole stability. Shublik B interval may be fractured resulting in lost circulation. 7" liner will be set across these intervals. Sadlerochit interval has an estimated pore pressure of +7.7 ppg EMW. Lost circulation may occur due to overbalance conditions. No faults are anticipated in the the proposed interval. Lost circulation is possible but not likely. There are no known drilling close approaches. No other known drilling hazards are expected. B ......
