ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. ITEM 4b: TPI (Top of Producing Interval). ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). ITEM 20: True vertical thickness. ITEM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. ITEM 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). ITEM 26: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, or Other (explain). ITEM 27: If no cores taken, indicate "None". ITEM 29: List all test information. If none, state "None". Winton, As per our discussion this morning, Nabors 2es is currently on K-03B. Yesterday, while attempting to build angle and land horizontal in Ivishak Zone 1, we experienced trouble with our directional assembly and were unable to build angle at our required doglegs. The plan was to build at an average 15/100'. The BHA was only delivering an average of 12-13/100'. This BHA was pulled and the motor bend was increased. We now needed 18/100' to 'catch up' and land at the geologic target. This second BHA only delivered 16/100', again getting us further behind the plan. The decision was made to plug and abandon the first 6-1/8" hole and start over from the 7" casing shoe. Open-ended drillpipe was run to TD and the production interval was filled with 26 bbls of 17.0 ppg Glass 'G' cement. Currently we are in the hole with our 6-1/8" directional BHA cleaning out cement in the 7" liner. The excess cement volume left -50' of cement in the 7" liner. The plan is to redrill the 6-1/8" section to the same geologic target, and drill the Zone 1 horizontal as originally planned. Please call with any questions. Phil, Per 20 AAC 25.035 (h) (1), alternate BOPE is hereby approved for wells C-17B (PTD 204-094) and K-03B (PTD 204-115), as described below. All other permit stipulations apply. Winton Aubert AOGCC 793-1231 Smith, Phil G (ASRC) wrote: Winton, As discussed yesterday, I neglected to request in the permit application dispensation from changing rams to 7" while running the drilling liner on C-17B. We would like to keep the drillpipe rams in place while running the liner as it is a safer, more efficient method. Please confirm your approval with a reply. Likewise the K-03B permit has been approved without the ram dispensation. K-03B and C-17B are quite similar except that K-03B has just a 550' liner planned (about half that of C-17B). We also request the same dispensation for K-03B. Apologies for the inconvenience, Phil Smi th Operations Drilling Engineer 564-4897 . [ill fÆ ~c . l ¡ , I ¡ I í ¡ I i ¡ ¡ FRANK H.
