Mr. Regg,
Producer 15-20C (PTD # 2050930) has been secured with a tubing tail plug. The well failed an MIT-T
and passed CMIT-TxIA on 03/17/22 which verifies failed primary well barrier. The well is now
classified as NOT OPERABLE and will remain shut in until well barriers are restored.

Subject: Corrosion Inhibitor Treatments of GPB DS 15
Dear Mr. Maunder,
Enclosed please find multiple copies of a spreadsheet with a list of wells from GPB DS
15 that were treated with corrosion inhibitor in the surface casing by conductor annulus.
The corrosion inhibitor is engineered to prevent water from entering the annular space
and causing external corrosion that could result in a surface casing leak to atmosphere.
The attached spreadsheet represents the well name, API and PTD numbers, top of
cement depth prior to filling and volumes of corrosion inhibitor used in each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10-404,
Report of Sundry Operations.

Alaska.
IrEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
IreM 4b: TPI (Top of Producing Interval).
IreM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
IreM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
IreM 20: True vertical thickness.
IreM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the
cementing tool.
IreM 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in
item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional
interval to be separately produced, showing the data pertinent to such interval).

location. Clean location & fold herculite.
Held pre-move safety meeting with crew.
Move rig to DS 15-20. Position rig in front of 15-20. Install dikes
around tree & lay herculite in front of well. Confirm WHP of 0
psi on 15-20.
Waiting on Veco Well Support to install working platform.
Call was made at 0900 hrs by Jerry Middendorf to request
platform.
No return calls to TP, check with Veco at 1600 hrs and left
message.
Check at 1800 hrs, Veco had no clue about it. Veco told co.
man 2000 hrs was absolute latest.
Finally showed up on location at 2200 hrs. Supposedly
searching for platform that had been on location the entire
time.
BP was charged $2,222 (3 hrs standby) for wait on Veco Wells
Support.
Rig accepted at 2200 hrs on 7/31/05. NU ROPE, RU around
rig.
RU and make ready CT equipment.
General RU around rig.
BOP testing: function and pressure per well plan.
250 psi/3500 psi for Combis, VBR, blind/shear, packoff.
250 psi/2500 psi for Hydril.
Trouble getting to test due to misconfigured stack (at SLB
shop).
Trouble getting VBR to test due to unknown causes.
BOPE test eventually good after work.
SLB took 4 hrs of downtime on ROPE tests.
Computer system work by SLB.
Alarm system work by Nordic.
Test hardline on inlet and outlet sides.
Pump coil volume.
Pull test CTC to 40K Ib.
Pump dart.

Hit obstruction at 227'.
Worked through it, samples at shakers showed fibrous
material, nut hulls, most likely LCM from previous wells' lost
circulation problems.

LD memory logging tools.
PU nozzel and flush cement from stack.
Test liner lap to 2100 psi. Bleed to 575 in 30 min .FAILED test.
Liner lap test failed. Bleed off from 2100psi. to 575 psi in 30
min.
Prep frool for perforating job. Load gfuns in pipe shed.
PJSM for perforating operation.
PU first gun run 510' gun and 342' blanks + 41 stands of CSH
RIH w/ 1st pert gun to 10650' EOP.
Drop 518" ba11. Pumping at 2.2/.75 bpm, Fire first guns:

Dear Mr. Kolstad:
Enclosed is the approved application for permit to redrill the above
development well.
This permit to redrill does not exempt you from obtaining additional permits or
approvals required by law from other governmental agencies, and does not
authorize conducting drilling operations until all other required permits and
approvals have been issued. In addition, the Commission reserves the right to
withdraw the permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20,
Chapter 25 of the Alaska Administrative Code unless the Commission
specifically authorizes a variance. Failure to comply with an applicable
provision of AS 31.05, Title 20, Chapter 25 of the Alaska Administrative Code,
or a Commission order, or the terms and conditions of this permit may result
in the revocation or suspension of the permit. Please provide at Least twenty-
four (24) hours notice for a representative of the Commission to witness any
required test. Contact the Commission's petroleum field inspector at (907) 659-
3607 (pager) .

• Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
• Distance to nearest property - 28,000 ft.
• Distance to nearest well within pool - 280 ft.

• MWD directional and Gamma Ray will be run over all of the open hole section.
• Cased hole MCNL.
Hazards
• The maximum recorded H2S level on pad s 50 pm (DS 15-21A on 02/24/01).
