Subject: UNDER EVALUATION: L -211 (PTD #2040290) , L -212 (PTD# 2050300), L -218 (PTD #
2051190) Wells Due for MITIA Compliance Testing
All,
Injectors L -211 (PTD #2040290) , L -212 (PTD # 2050300), L -218 (PTD #2051190) exceeded their 4 year MIT -IA
requirement due to being shut in for reservoir management. Injector L -211 (PTD # 2040290) passed an MIT -IA
to 4000 psi on 05/31/12. Injector L -212 (PTD # 2050300) passed an MIT -IA to 4000 psi on 05/31/12. Injector
L -218 (PTD # 2051190) passed an MIT -IA to 4000 psi on 06/02/12. These wells are ready to be put back on
injection and are being reclassified as Under Evaluation so that an AOGCC MIT -IA can be obtained once the
wells are thermally stable.
Please be aware of the potential for annular fluid expansion when putting these wells on injection.
Please call if there are any questions. Thank you,

Subject: Corrosion Inhibitor Treatments of GPB L-Pad
Dear Mr. Maunder,
0 C T 4 5 2009
blas~Ca ~ii ~C Gas Cons. Commi~sion
Anchorage
a.o5'~- ~ ~ ~
L--~---" ~ ~ ~
Enclosed please find multiple copies of a spreadsheet with a list of wells from GPB L-
Pad that were treated with corrosion inhibitor in the surface casing by conductor
annulus. The corrosion inhibitor is engineered to prevent water from entering the
annular space and causing external corrosion that could result in a surface casing leak
to atmosphere. The attached spreadsheet represents the well name, API and PTD
numbers, top of cement depth prior to filling and volumes of corrosion inhibitor used in
each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10-404,
Report of Sundry Operations.
If you require any additional information, please contact me or my alternate, Anna Dube,
at 659-5102.
Sincerely,
Torin Roschinger
BPXA, Well Integrity Coordinator

INSTRUCTIONS
GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
ITEM 20: True vertical thickness.

'/a Mile Wellbore Near Wellbore Mechanical Integrity Summary:
No wells with-in a ~/a mile radius, however, L-124 is just outside the ~/a mile radius at the top of the Na
sands. That being the case, a driller's hard-line will be established in the directional program to insure
that this injection well path does not enter the Ya mile radius of the L-124 well.
There are no near Wellbore integrity issues.
Anti-Collision Issues:
The proximity of the W KUPST-01 well does not pass BP's major risk assessment (6091.31' center to center
distance at 202.6' TMD). This well has been P&A'ed.

Centralizer Recommendations:
9 5/8" Casing Centralizer Placement
a) Run (25) 9 5/s" Bow-spring centralizers attached over the collar of each joint of casing from the
casing shoe up to 2,440' MD (1,000' MD above the shoe).
b) Run 2Bow-spring centralizers on either side of the TAM Port Collar (if run).
c) This centralizer program is subject to change due to actual hole conditions.
7" String Centralizer Placement
L218i Well Plan Summary Page 4
a) Place 2 rigid straight blade centralizers back to back at the very bottom of the shoe joint. Do not
run any centralizers on the next two full joints.
b) With the exception stated above, run one centralizer per joint, floating across the entire joint up
to 5,330' MD (500' MD above Ma sand). Zonal isolation with cement is required across the Ma
sands. The centralizer program is subject to change pending modeling and hole conditions.
Well Control
Well control equipment consisting of 5,000 psi working pressure pipe rams (2), blind/shear rams, and annular
preventer will be installed and are capable of handling the maximum potential surface pressures. Based upon
calculations below, BOP equipment will be tested to 4,000 psi. Due to test pressures not exceeding
S,000psi it will not be necessary to have two sets of pipe rams installed at all times.
BOP regular test frequency for L-2181 will be 14 days. ~'

Drilling Hazards and Contingencies
POST THIS NOTICE IN THE DOGHOUSE
Well Control -
A. The Schrader in the Polygon 1 fault block is expected to be normal to sub-normally pressured at 8.0-8.7
ppg EMW.
11. Hydrates and Shallow gas
A. Gas Hydrates are present at L-pad and have been encountered in off-set wells and throughout the SV4,
SV3, SV2, and SV1. Setting casing into the SV1 should keep hydrate exposure from extending past the
surface hole, however, some gas hydrates have been noted in the lower SV1 below the base of the
surface casing. Recommended practices and precautions for drilling hydrates should be followed in the
surface hole.
111. Lost Circulation/Breathing
A. Lost circulation is not expected at the L-Pad. The faults associate with this well have been intersected by
several other wells on L-pad at this depth with out problem.
• Note: Massive losses were encountered on L pad at L-118 due to high penetration rates (1100-1300
fph) and inadequate hole cleaning strategy.
B. We do not expect breathing on this well.
IV. Kick Tolerance/Integrity Testing
A. The 9 5/e" casing will be cemented to surface.
B. The 7" casing will be cemented with 15.8 Ib/gal tail cement to 500' MD above the Ma sand -5,330' MD.
C. The Kick Tolerance for the surface casing is 55 bbls with 9.0 ppg mud, 11.4 ppg LOT, and an 8.7 ppg
pore pressure.
D. Integrity Testing -11.4 ppg EMW
V. Stuck Pipe
A. We do not expect stuck pipe if the hole is kept adequately clean.
VI. Hydrogen Sulfide
L-Pad is nQL designated as an H2S site, however Standard Operating Procedures for H2S precautions should
be followed at all times. No measurable H2S is being produced from the formation at this time: However 10
ppm is measured regularly at both L and V pads as a result of the use of Ivishak gas lift gas. All V and L pad
gas lift wells will likely have H2S in this concentration. Note that all injection has been with produced water
not seawater. H2S levels are expected to remain low until the waterflood matures. Recent H2S data from the
pad is as follows:
Adjacent producers at Surface: Adjacent producers at TD: Max on L-Pad
L-101 5 ppm L-201 5 ppm L-107 20 ppm
L-02 10 ppm
VII. Faults
A. Two faults are expected to be crossed at V-221 i. No losses are expected to be associated with either
fault. Details are listed below for reference:
Fault 1 ("L" Pad Fault -SV2): 3067' MD 2433' ND 2360' TVDss Throw: 110' W
Fault 2 (UG3): 5342' MD 3543' TVD 3470' TVDss Throw: 105' SW
VIII. Anti-Collision Issues
The proximity of the WKUPST-01 well does not pass BP's major risk assessment (6091.31' center to center
distance at 202.6' TMD). Photo-gyro surveys were used to drill this "old" well which is currently in a P&A'
status.
