Unable to trolly in/out with block hook. Standby for mechanics. Trolly gearbox repaired. MU BKR
2-1/8" motor w/a 2.32" 5-bladed junk mill. Load well w/ 151 bbls of 2% KCI w/Safelube. RIH and
tag CIBP at 9887'. Worked the mill on the obstruction but stalled immediately. Decided to POOH
before damaging the casing. The depth of 9887' has a dogleg severity of 28 deg/100 ft. At surface,
changed the mill to a 2.30" diamond parabolic mill and RIH. RIH past 9887'with zero issues.
Tagged CIBP at 11,301'. Milled CIBP/pushed downhole from 11,301'to 11,453'. Stalled 21 times
total so far(5 stalls while milling up the CIBP, and 16 stalls at various depths while pushing CIBP
downhole). IA pressure has been bleed from 900 psi to 450 psi twice.

Producer 15 -40A (PTD #1980980) was confirmed to have sustained casing.pressure on the OA above
MOASP. The tubing /IA/OA pressures were equal to 2500/1640/1020 on 09/11/2011. A Packoff Test
passed on the Inner Casing hanger on 09/12/2011 which confirmed integrity of the wellhead seals. An
OA repressurization test was conducted from 09/12/11 - 09/14/11 and the OA build up rate was —10
psi /daytherefore this well is manageable by periodic bleeds.
This well does not require further diagnostics of the sustained casing pressure on the OA and therefore is
not being reclassified to Under Evaluation. This notification is being provided to meet the AOGCC
requirement in CO 492 for notification of any well that exhibits sustained casing pressure on the OA
above 1000 psi.

Enclosed please find multiple copies of a spreadsheet with a list of wells from GPB DS
15 that were treated with corrosion inhibitor in the surface casing by conductor annulus.
The corrosion inhibitor is engineered to prevent water from entering the annular space
and causing external corrosion that could result in a surface casing leak to atmosphere.
The attached spreadsheet represents the well name, API and PTD numbers, top of
cement depth prior to filling and volumes of corrosion inhibitor used in each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10-404,
Report of Sundry Operations.

Rig down. Lower & scope down derrick.
Move from D Pad to DS 15. Unable to Rig up on 15-43
because of 15' Well spacing. Move to 15-40a and stand-by.
Wait on Drilllsite Maintenance to install BPV & R/D from well.
Wait on prepping of location prior to acceptance. Had
subsidenece around well. Needed to fill & grade w/ gravel.
Level area around well for matting boards.
Lay Herculite & matting boards. Spot in rig sub, pipeshed &
pits. Spot camp. Begin rigging up.
Finish spotting in Rig sub, pits & pipe shed. Begin rigging up.
Raise & scope derrick.
Spot tiger tank. Lay lines to rig. Begin taking on heated
seawater for well kill. Accept Rig at 03:00 hr...
Wait on Drillsite maintenance to pull BPV. They are delayed on
Nabors 7es.
Pick-up Lubricator. Pull BPV. Lay down same.
Pre-Job Safety and Operations Meeting with all.
Test pump line at 3,500 psi. Circulate/Kill Well with 120 deg
Sea Water at 3 -5 bpm/250 - 550 psi. Monitor Well.
Install TWC and test. Blow down lines. Rig down lines.
Nipple down Production Tree. Inspect Hanger Thrds. Good.
(4 1/2" NSCT...8 turns).
Nipple-up BOP Stack. and Test Spool.
Ru Test Equipment. Perform BOPE Test per BP/AOGCC
requirements at 250/3500 psi. Good Test. State's right to
witness tesLwaived by J. Chrisp.
P/U & thaw out kelly. Pressure test, LP 250 psi, HP 3500 psi.
R/D test equip. Blow down lines. Stand back kelly. BOP test
complete.
P/u lubricator & pull TWC. LID lubricator.
M/U landing jt. BOLDS. P/U w/ 118K. Tbg came free. Pull
hanger to floor with 112K.
Break out hanger. LID hanger & landing jt. Fill hole w/ 14 Bbls.
Change out elevators & R/U tbg tools & control line spooler..
POOH & LID 4 1/2" completion. Spool up control line. Pulled &
laid down 25 jts @ midnight.
POOH & LID 4 1/2" completion & spool up control line. LId 52
jts. Recovered all 29 SS bands.
R/D spooling unit. clear floor. LID Camco TRFC-4 SSSV.
POOH and lay down 41/2" L-80 Completion. Recovered
Completion as follows: 212 Jts, a 22.71' Chemical Cut Stub,
SSSV w/Pups, 4 ea GLMs w/Pups, X-Nipple w/Pups, and 4 ea
Space-Out Pups. Chemical Cut Stub end was a clean
cuLnormalOOIlD. Single Control Line and all 29 Bands were
also recovered.
Rig down 4 1/2" Handling Equipment. Clear and Clean Floor.
Rig up 3 1/2" Handling Equipment. Install Wear Bushing.
Pick-up BHA #1 as follows: Cut Lip Guide, 6 3/8" Overshot w/4
1/2" Basket Grapple, Extention, Top Sub, Bumper Sub, Oil Jar,
9 ea 4 1/3" Drill Collars.
RIH with BHA #1 and single in with 3 1/2" Drill Collars from
Pipe Shed.

Friday, August 14, 1998; I witnessed the weekly BOP test on Nabors rig #CDR-1
redrilling Arco well DS 15-40A in the Prudhoe Bay Field.
I arrived to find the rig ready to test and we began immediately. The test went well
with all equipment being tested properly and holding its pressure test.
During the accumulator test I noticed that the air pumps failed to come on when
the pressure dropped to 2375 psi. I suggested that the regulator be adjusted to
2500 psi and cut out at 2800 psi. We bled the system down to 0 psi and checked
the precharge. Some of the bottles were below the 1000-psi recommended
precharge and these were recharged. The Pit monitoring system has not been
installed as of yet and I informed the Drilling foreman, Mark Johnson, that after
this well it would be re~q,~red. The hydraulic choke had not yet been installed but
the parts were on hand~wT:)uld be installed before the next well.
The rig is adequate for summer operations except for the above problems. I am
concerned about the lack of a PVT system during their open-hole work. It was
explained to me that a pit watcher is present during this time, but I have observed
pipe being run without one actively watching. There are not enough personnel on
hand to cover this area constantly. A trip tank and Pit level gauge is needed and is
required by APl RP-53. This rig is due for refit and winterization soon and should
be watched carefully until it is in compliance.
SUMMARY: The weekly BOPE test I witnessed on Nabors rig #CDR-1 revealed
the following;

CT Sidetrack and Completion
Intended Procedure
Objective:
Plugback existing wellbore. Directionally drill a horizontal well from the
existing wellbore. Complete with 2-7/8" cemented liner. Perforate and
produce.
Intended Procedure:
· Cement squeeze and plugback the existing completion to - 9800' md.
Mill window in 5-1/2" casing at -9800' md. Drill 10' of formation with
8.9 ppg Flopro fluid and perform Leak off test to 9.4 ppg.
Directionally drill 3.75" x 2300' horizontal well to access Lower Romeo
reserves. Build angle to -92.5° at TD.
Run and cement 2-7/8" liner to TD.
° Perforate selected intervals.
· Place well on production.
