all types of lands and leases.
Item 1' Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the
data pertinent to such interval.

COILED TUBING EXTENSION DRILLING OPERATIONS
7/28/98 - 7/30/98:
MIRU CTU. Pressure tested BOPE. Pumped 2% KCL water down hole.
7/31/98:
RIH w/BHA & swapped well to FIo Pro. Pumped LCM pill. Tagged obstruction located in liner & attempted to
mill through it but motor stalled. POOH w/BHA. RIH w/BHA & milled on obstruction located @ 10930' MD.
8/01/98:
Cont. milling on obstruction. RIH & tagged another obstruction @ 11049' MD. Milled to 11056' MD &
backreamed. RIH & milled out packoff bushing located @ 11225'. RIH to guide shoe @ 11274'. POOH while
backreaming obstruction areas. RIH w/BHA & milled POB/guide shoe area. Drilled to 11297' & backreamed.
POOH w/BHA. RIH & drilled open hole to 11359'.
8/02/98:
Drilled ahead to 11400' MD but unable to drop inclination. Decision made to PUH & drill open hole sidetrack
near the end of the slotted liner. PUH to 11320' & time drilled OH ST to 11530'.
8/03/98:
Drilled OH ST to 11546' & POOH w/BHA. Lower part of motor lost in hole. Fished motor from hole. RIH w/
BHA.
8/04/98:
Reamed OH ST entrance while RIH to BTM. Drilled from 11546' to 11966'.
8/05/98:
Drilled to 11990' & backreamed back to 11320'. Lost MWD signal so POOH. Troubleshooted MWD.
8106198:
RIH w/BHA & reamed/backreamed 11600' to 11950'. Drilled to 12003' & CT became stuck. Freed pipe w/
Crude. Drilled horizontal to 12110'. Pumped Hi Vis pill. Drilled to TD @ 12175'. POOH.
8107198:
RIH & reamed/backreamed hole. Pumped Hi Vis pill. POOH. RIH & landed the pre-perforated liner @ 12174'.
8108198:
Released from the 2-3/8" liner & displaced well to Crude while POOH. RDMOL.
Placed well on initial production & obtained a valid well test.

Drill 10' of formation with 8.5 ppg Flopro fluid and perform leak off test
to 9.0 ppg.
Directionally drill 2.75" x ~1100' horizontal extension to access Romeo
reserves. Maintain -90° to TD.
· Run 2-3/8 .... pre-perfed liner to TD.
· Place well on production.
