CTU#8 1.75" CT. Job Scope: Adperf
MIRU. Make up and RIH with 2.200" GR/CCL carrier and 2.31" JSN. Bullhead well with 1% KCI w/
safelube. Tag 11434' (top perf) jet with 60/40, No progress, Log from tag up to 9000'. Call town for
permission to pu mill/motor. Perform weekly bop test before milling. Freeze protect well and
standby for weather.

CTU#8 1.75" CT. Job Scope: Adperf
RIH with 1.69" motor and 2.33" 5 bladed junk mill. Unable to make progress due to repeated stalls
at same spot -11116' ctm (15 stalls). Trip OOH for larger 2.12" motor and different, less aggressive
mill. MU/RIH with 2.30" parabolic mill. Tagged at similar depths and stalled instantly. No progress
made, new plan forward to drift with 1.75" slick BHA and caliper. RIH with slim DJN BHA and
tagged 14' deeper and could not make hole.

CTU#8 1.75" CT. Job Scope: Adperf
POOH with slim DJN BHA. Make up and RIH with 1.77" diamond mill. Dry tag at 11458 and begin
milling. Drift with motor on down to 15525'. No milling/motonvork really. -11460-11458' was a
repeated light set down (3-5k surface wt) and pop right through area. No motorwork through this
area. Repeated it several times with motor on at 1.2bpm. Able to dress the area up with little to no
set down weights. Dry drift through area several times with little to no set down. Dry drift down to
11550' Paint safety flag at 11520'. Continue up hole and paint another flag at 11450 (10' above
known set down area). POOH. Minimal if any wear on the 1.77" mill. MU/RIH with HES
GR/CCUMIT-24. Log from safety flag 11527 up to 9500ft. POH. MU & RIH w/ pert guns.

INSTRUCTIONS
General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Submit
a well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole wail design is changed.
All laboratory analytical reports regarding samples or tests from a well must be submitted to the AOGCC, no matter when the analyses are conducted.
Item is. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is
a completion.
Item 1b. Well Class Service wells: Gas Injection, Water Injection, Water -Alternating -Gas Injection, Salt Water Disp, Water Supply for Injection, Observation or Other.
Item 4b. TPI (Top of Producing Interval).
Item 9. The Kelly Bushing and Ground Level elevations in feet above mean sea level. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.

Enclosed is the approved application for the permit to redrill the above referenced development
well.
Per Statute AS 31.05.030(d)(2)(B) and Regulation 20 AAC 25.071, composite curves for well
logs run must be submitted to the AOGCC within 90 days after completion, suspension or
abandonment of this well.
This permit to drill does not exempt you from obtaining additional permits or an approval required
by law from other governmental agencies and does not authorize conducting drilling operations
until all other required permits and approvals have been issued. In addition, the AOGCC reserves
the right to withdraw the permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter 25 of the Alaska
Administrative Code unless the AOGCC specifically authorizes a variance. Failure to comply
with an applicable provision of AS 31.05, Title 20, Chapter 25 of the Alaska Administrative Code,
or an AOGCC order, or the terms and conditions of this permit may result in the revocation or
suspension of the permit.

Approval is requested for a permit to drill a CTD sidetrack from well 01-26A with the Nabors CDR2 Coiled
Tubing Drilling rig on or around September 1", 2019.
Proposed plan for 01-26B:
The coil sidetrack 01-26B will be a 1470' U-shaped well targeting the Zone 4 GOWN oil pad where DS -01 meets DS -
03. Drilling in 2017 identified a thick oil column in the Southeast DS -01 area, which will be the target of this well. The CST
will be drilled on or around September 1't, 2019.
During pre -rig work it is proposed to cut and pull the 3-1/2" scab liner from the cut at 10,380' MD above the top of
cement at -10,700' MD, then drift and set a 4-1/4" monobore whipstock with TOWS at -10,365' MD in the 4-1/2"
production liner.
The rig will move in, test BOPE and kill the well. A 3.80" window + 10' of rathole will be milled. The well will
kick off in the Shublik, land in Zone 3 to locate the OWC and toe up into Zone 4. The proposed sidetrack will be completed
with a 3-1/4" x 2-7/8" 13Cr solid liner, cemented in place and selectively perforated.
Rig Work (scheduled to begin September 111, 2019)

• All Class I & II non -hazardous and RCRA-exempt drilling and completion fluids will go to GNI at DS 4 or Pad 3 at DS 6.
• Fluids >1% hydrocarbons or flammables must go to Pad 3.
• Fluids >15% solids by volume must go to GNI.
• Fluids with solids that will not pass through 1/4" screen must go to GNI.
• Fluids with PH >11 must go to GNI.
Hole Size:
• 4.25" for entirety of the production hole section.
Liner Program:
• 2-7/8", 6.5#, 13Cr solid liner: 11,000' MD -11,835' MD
• 3-1/4", 6.6#, 13Cr solid liner: 9,967' MD -11,000' MD
• 3-1/2", 9.3#, STL 13Cr solid liner: 9,937' MD -9,967' MD
• A minimum 26.1 bbls of 15.3 ppg cement will be used for liner cementing (TOC = TOL at —9,937' MD).
• The primary barrier for this operation: a minimum EMW of 8.4 ppg (593 psi over -balance).
• A X -over shall be made up to a safety joint including a TIW valve for all tubulars ran in hole.
Well Control:
• BOP diagram is attached.
• Pipe rams, blind rams and the Ci pack off will be pressure tested to 250 psi and to 3,500 psi.
• The annular preventer will be tested to 250 psi and 3,500 psi.
Directional:
• See attached directional plan. Maximum planned hole angle is 110°, inclination at kickoff point is 38'.
• Magnetic and inclination surveys will betaken every 30 ft. No gyro will be run.
• Distance to nearest property— 24,000'
• Distance to nearest well within pool —650'
Logging:
MWD directional, Gamma Ray, and Resistivity will be run through the entire open hole section.
Gamma Ray / CCL / RPM log planned post -rig through the entire production liner.
Perforating:
• Perforating: 50'-100' post -rig, 2" Max Force guns, 60 deg random phasing, 6 spf, perfguns.
• The primary barrier for this operation: a minimum EMW of 8.4 ppg (593 psi over -balance).
• A X -over shall be made up to a safety joint including a TIW valve for all tubulars ran in hole.
Anti -Collision Failures:
• None. All offset wells pass BP anti -collision scan criteria.

the pad: 194 ppm on 01-07A (4/9/90).
• Lost circulation risk on 03-26B is considered Medium.
