Finish BOP test. Continue RU. RIH with HES GR/CCL 2.20" carrier and 2.25" DJN.
Test kill well with 264bbl of 1%KCl. Tagged high at 11708CTM. Could not jet off.
POOH for motor and mill. OOH with logging BHA and have a +5.5' correction. RIH
with 2.30" diamond parabolic milling asembly. Correct depth at flag and tag at 11719'.
Eventually work through obstruction after multiple stalls and overpulls. Tag again at
12140' and push to 12250'. Wiper trip to above LTP to 10375'. Dry drift to PBTD at
12856'. Currently POOH.

Continue to POOH with 2.30" milling BHA. RIH with HES GR/CCL in 2.20" carrier
with 2.25" DJN. Bobble through obstruction at 11,720'. Tag PBTD at 12858'. Log
OOH at 50fpm. Have good log data with a -3' correction.
Make up and RIH with 2" HES 6spf Maxforce perf gun and perforate 12551-12582,
Reference log BKR MWD 10/6/2019. POOH and got hung up at 11958. Got free
POOH. RIH with 2.28" Venturi and venturi down to tag at 11833' (deepest perf target
is 11620'). Currently PUH for dry drift.

Dry drift from 10390' to tag at 11834' with 2.28" Venturi Junk Basket. OOH with
venturi and muleshoe left in hole. Muleshoe: 2.30" x 0.7' . Make up and RIH with
2.29" 4-blade junk mill. Tagged muleshoe at 11830 and milled/pushed to PBTD. Dry
drift from above LTP. Correct depth at PBTD and paint red flag at 11299.9.
Corrected EOP at flag 11280.84'. POOH. Currently swapping BOPs.

Brief Well Summary:
14-38A is a CTD sidetrack from October 2019. The well has been a fulltime producer but has relatively low gross
fluid rates.
Objective:
The objective of this program is to add additional perforations in the horizontal of the well to bring in more
rate.
Cerberus Modelling suggests making TD with ~471’ of 2” guns w/ 1.75” coil tubing.

Sundry Procedure (Approval Required to Proceed):
Note: The well will be killed and monitored before making up the initial perf guns. This is generally done during
the drift/logging run. This will provide guidance as to whether the well will be killed by bullheading or
circulating bottoms up throughout the job. If pressure is seen immediately after perforating, it will either be
killed by bullheading while POOH or circulating bottoms up through the same port that opened to shear the
firing head.
Coil Tubing
1. MIRU Coiled Tubing Unit and spot ancillary equipment.
2. MU and RIH with GR/CCL logging tools and ~2.2” nozzle. Pull jewelry log for tie in.
3. POOH and confirm good data.
4. Contact OE prior to perforating for final approval of tie-ins.
5. Perform gun run #1 shooting 31’ interval from 12,550 – 12,581’.
6. Bullhead kill the well w/ ~255 bbls of 8.4 ppg 1% KCl or 8.5 ppg seawater. (This step can be performed
any time prior to open-hole deployment of the perf guns. Timing of the well kill is at the discretion of
the WSS.)
a. Wellbore volume to top perfs = ~170 bbls
7. Freeze protect tubing as needed. Prepare for deployment of TCP guns.
8. Confirm well is dead. Bleed any pressure off to return tank. Kill well w/8.4 ppg 1% KCl or 8.5 ppg as
needed. Maintain continuous hole fill taking returns to tank until lubricator connection is reestablished.
9. Pickup safety joint and TIW valve and space out before MU guns.Review well control steps with crew
prior to breaking lubricator connection and commencing makeup of TCP gun string. Once the safety
joint and TIW valve have been spaced out, keep the safety joint/TIW valve readily accessible near the
working platform for quick deployment if necessary.
10. Break lubricator connection at QTS and begin makeup of TCP guns and blanks per schedule below.
Extended perforating will include perf depths between 11,149’ – 11,620’.

INSTRUCTIONS
General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Submit
a well schematic diagram with each 10-407 well completion report and 10404 welt sundry report when the downhole well design is changed.
All laboratory analytical reports regarding samples or tests from a well must be submitted to the AOGCC, no matter when the analyses are conducted.
Item 1a. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is
a completion.
Item 1b. Well Class Service wells: Gas Injection, Water Injection, Water-Altemating-Gas Injection, Salt Water Disp, Water Supply for Injection, Observation or Other.
Item 4b. TPI (Top of Producing Interval).
Item 9. The Kelly Bushing and Ground Level elevations in feet above mean sea level. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
Item 15. The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).

Per Statute AS 31.05.030(d)(2)(B) and Regulation 20 AAC 25.071, composite curves for well
logs run must be submitted to the AOGCC within 90 days after completion, suspension or
abandonment of this well.
This permit to drill does not exempt you from obtaining additional permits or an approval required
by law from other governmental agencies and does not authorize conducting drilling operations
until all other required permits and approvals have been issued. In addition, the AOGCC reserves
the right to withdraw the permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter 25 of the Alaska
Administrative Code unless the AOGCC specifically authorizes a variance. Failure to comply
with an applicable provision of AS 31.05, Title 20, Chapter 25 of the Alaska Administrative Code,
or an AOGCC order, or the terms and conditions of this permit may result in the revocation or
suspension of the permit.

Re: 14-38A Permit to Drill Request
Approval is requested for a permit to drill a CTD sidetrack lateral from well 14-38 with the Nabors CDR2
Coiled Tubing Drilling rig on or around August 1`t, 2019.
Proposed plan for 14-38A:
A 2,200' coil tubing drilling sidetrack will be drilled with the Nabors CDR2 rig. Prior to sidetrack, a flowback is planned for
"'S days to confirm well integrity when the well is warm and flowing. The whipstock will be set pre -rig in the 7" liner. Then
the rig will move in, test BOPE and kill the well. A 3.80" window+ 10' of rathole will be milled. The well will kick off in the
Zone 4, with minor undulations planned in the 46P oil column. The well bore toes down towards the interpreted OWC at
TD. The proposed sidetrack will be completed with a 3-1/2" x 3-1/4" x 2-7/8" 13Cr solid liner, cemented in place and
selectively perforated. This completion will completely isolate and abandon the parent Ivishak perfs.
Rig Work (scheduled to begin August 1, 2019)

Mud Program: •
• Drilling/Completion: a min EMW of 8.4 ppg KCL with Geovis for drilling (444 psi over -balance).
Disposal:
• All Class I & II non -hazardous and RCRA-exempt drilling and completion Fluids will go to GNI at DS 4 or Pad 3 at DS 6.
• Fluids >1% hydrocarbons or flammables must go to Pad 3.
• Fluids >15% solids by volume must go to GNI.
• Fluids with solids that will not pass through 1/4" screen must go to GNI.
• Fluids with PH >11 must go to GNI.
Hole Size:
• 4.25" for entirety of the production hole section.
Liner Program:
• 2-7/8", 6.5#, 13Cr solid liner: 11,236' MD —12,940' MD
• 3-1/4", 6.6#, 13Cr solid liner: 10,715' MD —11,236' MD
• 3-1/2", 9.3#, 13Cr solid liner: 10,430' MD -10,715' MD
• A planned 35.75 bbls of 15.3 ppg cement will be used for liner cementing (TOC = TOL at 10,430' MD).
• The primary barrier for this operation: a minimum EMW of 8.4 ppg (440 psi over -balance)
• A X -over shall be made up to a safety joint including a TIW valve for all tubulars ran in hole.
Well Control:
• BOP diagram is attached.
• Pipe rams, blind rams and the CT pack off will be pressure tested to 250 psi and to 3,500 psi.
• The annular preventer will be tested to 250 psi and 3,500 psi.
Directional:
• See attached directional plan. Maximum planned hole angle is 102% inclination at kick off point is 38'.
• Magnetic and inclination surveys will betaken every 30 ft. No gyro will be run.
• Distance to nearest property — 9135'
• Distance to nearest well within pool —510'
Logging:
• MWD directional, Gamma Ray, and Resistivity will be run through the entire open hole section.
Perforating:
• Perforating: —500' of perforations (on rig) - 2", 60 deg random phasing, 6 spf, perf guns.
• A X -over shall be made up to a safety joint including a TIW valve for all tubulars ran in hole.
Anti -Collision Failures:
• None. All offset wells pass BP anti -collision scan criteria.
Hazards:
• Drill Site 14 is considered an H2S pad. Maximum H2S reading on 14-21: 590 ppm (06/18/2002). Most recent H25
sample on 14-38: 18 ppm (10/12/1994)
• Overall Lost circulation risk is considered medium.

• The reservoir pressure on 14-38 is expected to be 3400 psi at 8800' TVDSS (7.43 ppg equivalent).
