Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.

IreM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in

WEXIT Rack back 4" HWDP and 4-3/4" steel drill collars in the derrick.
Break, Gauge and lay down mills. Upper watermelon mill full
gauge. Clear and clean rig floor.
WEXIT Pick up perforated sub and flush BOP.
WEXIT Pull wear bushing. Install test plug.
WEXIT Attempt to remove blind rams and install blind shear rams.
Bolts on ram doors had been over tightened from previous
blind shear ram change out. Inspect bolts and threads for
damage.
Change blind rams to blind shear rams.
Rig up test equipment.
Test BOP's and all associated valves to 250 psi 5 min low,
3500 psi 5 min high. BOP test witnessed by AOGCC rep Jeff
Jones. BP WSL Chris Clemens. Nabors tool pusher Larry
W urdeman.
WEXIT Rig down test equipment. Pull test plug install wear bushing.
WEXIT AIM in XP motor failed surface test. Mobilize motor from
Schlumberger yard (Back up motor with 50 hours on it OK).
While waiting on motor extra HWDP was picked up and stood
back in the derrick. Slip and cut 119' of drilling line. New motor
passed surface test.

Enclosed is the approved application for permit to drill the above referenced
development well.
The permit is for a new wellbore segment of existing well PBU W-204, Permit No.
206-158, API No. 50-029-23333-00. Production should continue to be reported as
a function of the original API number stated above.
This permit to drill does not exempt you from obtaining additional permits or an
approval required by law from other governmental agencies and does not authorize
conducting drilling operations until all other required permits and approvals have
been issued. In addition, the Commission reserves the right to withdraw the
permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter
25 of the Alaska Administrative Code unless the Commission specifically
authorizes a variance. Failure to comply with an applicable provision of AS 31.05,
Title 20, Chapter 25 of the Alaska Administrative Code, or a Commission order, or
the terms and conditions of this permit may result in the revocation or suspension
of the permit. When providing notice for a representative of the Commission to
witness any required test, contact the Commission's petroleum field inspector at

Formation pressures from surface to the Ma are calculated based on a .455 water gradient. Formation
pressures from the UG_Ma down to the SB_OA are based on W-200P61 RFT pressures taken before start
of W Pad Schrader Bluff production. Formation pressures in the SB_OBa through SB_OBd sands are
estimated and assume these sands will show moderate pressure depletion resulting from W-200 and W-201
Wellbore TD Criteria:
TD for all laterals is planned to be at end of polygon. No rathole is necessary.
No extensions beyond the end of the polygons are planned, however if extended poor rock quality is encountered
while drilling, some laterals could be ended sooner.
If structure along the laterals comes in substantially deep to prognosis, then the OBa lateral may be ended sooner
than the polygon end to avoid approaching any closer than 1500 feet to the minimum case OWC location. If O
sand structure comes in shallow to prognosis, the OBa lateral should be terminated at the point where the top OBa
marker depth is projected to reach a depth of -4970 feet TVDSS. Terminating the OBa lateral at this depth will still
leave a minimum of 1500 feet of lateral standoff to the minimum case OWC depth at -5030 feet TVDSS. O sand
structural depths along the W-204 laterals should be well established by the time the OBd lateral is drilled. Thus,
there should be between 5 and 10 days of time prior to drilling the OBa lateral to reconfigure the OBa lateral
profile, if necessary.

Drilling Hazards and Contingencies
POST THIS NOTICE IN THE DOGHOUSE
I. Well Control
A. The Schrader in the target fault block is expected to be normal pressured at 8.2-8.7 ppg EMW. This
pressure is estimated based on offset well pressures and material balance.
II. Hydrates and Shallow gas
A. Gas Hydrates are possible at W-pad from the SV5 thru the SV1 sands below the base of the permafrost.
Setting casing through the SV1 should keep hydrate exposure from extending past the surface hole
although there are previous instances of gas from the SV1 into the Ugnu on W-pad. Recommended
practices and precautions for drilling hydrates should be followed in the surface and intermediate hole
sections.
III. Fault Crossing/Lost Circulation/Breathing
A. Lost circulation is not expected on this well.
B. Fault #1, if detected, should be crossed in the SV3 formation, but should have a minimal throw at that
depth.
C. If the seismic artifact around the possible fault #2 locations does indeed turn out to be a fault then the well
bore will be sidetracked to a new BHL to the East. This new location will likely be over 500' away from our
current target and will require state approval before proceeding.
D. Breathing is not expected on this well.
E. 53 bbls with a 10.8 ppg EMW LOT, 9.2ppg mud and 8.7 ppg OBd EMW
F. Wood and associated running gravels have been caused problems on previous W-pad well surface holes.

C. Fault #2 is a possible fault - if encountered, wellbore will be re-oriented to a more easterly postion.
D. Fault #3 will not be crossed by W-204. Fault position and standoff defines western side of target polygon.

OPERATIONS SUMMARY
Pre-Rig Work:
1. A 34" x 20" insulated conductor has already been set and surveyed. Weld an FMC landing ring for the FMC
Gen 5 Wellhead on the conductor.
2. If necessary, level the pad and prepare location for rig move. Prepare neighboring wells for the rig move and
production simops.
Drilling Rig Operations:
1. MIRU Nabors 7ES. Nipple up diverter spool and riser. Function test diverter. PU 4" DP and stand back in
derrick.
2. MU 13-'/z" drilling assembly with MWD/GR and directionally drill surface hole to approximately 3,346' MD. The
actual surface hole TD will be +/-200' TVD below the top of the SV1 sand.
3. Run and cement the 10-3/a", 45.5#, L-80 BTC-SD casing from TD back to surface. ND drilling spool and riser
and NU casing /tubing head. NU BOPE and test to 3,500 psi.
4. MU 9'/e" directional drilling assembly with MWD/GR/Res/Den/Neu/PWD and a Ghost reamer 11,600' from the
bit. RIH to float collar. Test the 10-3/a" casing to 3,500 psi. for 30 minutes. Note: Ensure grade is L-80. There
has been some K-55 on the slope and 3,500 psi would be near 100% burst for K-55 casing.
5. Drill out shoe track and displace the well to new 8.8 LSND drilling fluid. Drill 20' of new formation and perform
a LOT. Directionally drill ahead to TD at ± 5,861' MD.
6. Run drill pipe conveyed MDT. Perform cleanout run if necessary.
7. MU and run 7 5/e", 29.7#, L-80, BTC-M, TC-II casing to TD. Cement in one stage to -4,446' MD (1,000' MD
from the surface casing shoe). Bump plug with seawater.
8. Clean pits for displacement. Freeze protect 7 5/e" x 10-3/a" annulus to 2,200' TVD.
9. RU a-line. Run USIT log from TD to surface.
• W-204 OBd Lateral
10. PU 6-3/a" conventional assembly with MWD/GR/Res/Den /Neu/AIM/PWD and RIH to the float collar. Test the
7 5/6" casing to 3,500 psi for 30 minutes.
11. Displace well over to 9.0 ppg Versapro MOBM. Drill out shoe track, circulate and condition mud. Drill 20' of
new formation and perform FIT to 10.5 ppg EMW.
12. Directionally drill ahead to TD geo-steering through the OBd to TD at -12,662' MD. Back ream out of the hole
to the shoe, circulate and condition mud. RIH to TD and displace well bore to SF MOBM.
13. POOH to near OBc window depth, log the top OBc (RA marker) with GR, log across the OBa window as well.
POOH.
14. MU and run 4-'h" 12.6# L-80 BTC-M slotted liner and swell packers with liner hanger and liner top packer.
Position TOL for OBc whipstock. POOH.

window is near the top of the OBb sand. Orient and set whipstock. Shear off Whipstock. Displace well over to

16. RIH with 6-3/a" conventional drilling assembly with MWD/GR/Res/Den/Neu/AIM/PWD to whipstock.
Directionally drill to the planned TO of OBb lateral. Back ream out of hole to KOP, circulate and condition mud.
RIH to TD and displace well bore to SF MOBM. POOH.
17. Pull whipstock #1.
18. Run and set 4-'/2" slotted liner and hook hanger. POOH and LD liner running string.
19. Run Baker Hook Hanger Lateral Entry Module #1 with ZXP liner top packer, POOH.

24. Run Baker Hook Hanger Lateral Entry Module #2 with ZXP liner top packer, POOH.
25. Clean pits for displacement. Displace mother bore over to 9.2 ppg KCL brine. POOH.
26. MU 4-'h" 12.6# IBT-M, L-80 completion assembly tying into and RIH. See completion schematic for details.
Land tubing hanger and run in lockdown screws.
27. Drop RHC ball and rod and set packer as per Baker recommendation and test tubing to 3,500 psi and annulus
to 3,500 psi. Shear DCK valve.
28. Install two-way check and test to 1,000 psi from below. Nipple down the BOPE. Nipple up and test the tree to
5,000 psi.
29. Pull two-way check. Reverse in brine with corrosion inhibitor to protect the annulus and freeze protect the well
to 2,200' TVD. Install VR plug in 7 5/8" casing annulus valves.
30. Install BPV. Secure the well and rig down /move off Nabors 7ES.
Post-Rig Work:
1. MIRU slickline. Pull the ball and rod /RHC plug.
2. Pull out DCK shear valve, install GLV in all of the other mandrels as per the PE.
